Employee was changing tires on a 988 loader. He was using a hydraulic jack to brake down tire from rim. While under pressure the jack slipped and struck employee in the head.

Employee was changing fuel filters on the CAT 345 Excavator when he got fuel on his hands after changing the filter he got down on the ground and struck a cigarette lighter to lite a cigarette catching his right hand on fire. Employee was taken to the hospital and received 2nd degree burn.
EE was installing a set of chains on a 14H cat grader when he slipped and fell on snow covered ground. He landed on his backside. EE returned to on 3-7-14 he also went back to the doctor on 3-18-14 for a follow up visit and he was taken off work again for 1 week. EE work March 7th, 8th and 10th.
Employee was disassembling a D11 roller frame using an impact wrench & socket. The impact socket busted cutting employee's left hand.

The employee returned to work the following day. The employee continued to complain of pain in his shoulder and went back to the Dr, and has been put off work.
Employee was changing bit on the Highwall miner head and a rock became dislodged from the wall and hit the employee and knocked him into the miner head. The employee received a fractured right wrist and two fractured vertebrate.
He was changing the starter on a Kenworth mechanic truck when the wrench he was using broke and cut his arm. He was taken to the ER and received 14 Stitches. He returned to work that day.
A Komatsu PC360 excavator was tramming towards a burning bush fire when it caught on fire under the cab. Forensic fire investigators have not yet determined the source of ignition. This is a non-injury accident.
Employee was dumping a load of coarse refuge when the hoist pins on the frame broke turning the truck over. He was taken to Highland Regional Hospital to have his right arm looked at. Doctor recommends 2 days off to rest. He was wearing a seatbelt. The doctor only done x-rays.
EE (mechanic) was changing the left track on the D-10R dozer until #L1-1472 when the chain he was picking up the track up slipped and fell on his right foot. He drove himself to ARH in Hazard and received x-rays no other treatment. His right foot was bruised.
EE was replacing alternator on a D9R dozer (unit #L1-798) when he slipped on the dozer track that was wet. He reached for the engine compartment door to steady himself and caught the corner of the door cutting his right hand. He went to ARH in Hazard, KY and received 10 stitches.

EE contract mechanics was changing hydraulic hose when operator bumped control on Ingersoll Rand Drill unit #64-7864 causing hydraulic oil to inject under skin of index finger on left hand.
Dozer was cleaning the highwall and over extended and material gave out and caused the dozer to slide and over turn one time. The operator was thrown in the floor and suffered five broke vertebrae, two broken ribs, punctured stomach, bruised intestines, cut on the ear, and a broken foot.
Truck Driver was traveling down a ladder and slipped and fell to the ground and broke his ankle.
The employee was attempting to remove a crane from a mechanics truck when the two chains he was using one of them came off. This allowed the base of the crane to fall to the floor. The employee was then struck in the leg by the crane and received a compound fracture to the right leg.
The operator was climbing down off of his loader and stepped into a hole. When he did it twisted his back and hip. He continued to work until 08/23/2011 when the doctor he went to see put him off work for therapy. Again he did not get put off work until 08/23/2011.
The employee was putting a primer box up in the top of a magazine and the box fell down. He tried to catch the box and felt something pull in his shoulder. The doctor put him on light duty at that time. He continued to have pain and the doctor put him off of work until he could see a neurosurgeon. Date 07/26/2011. The employee was on light duty from 05/23/2011 to 07/26/2011.
The employee came down a ladder when the bottom kicked out, causing him to fall and strike his head on concrete floor. The doctor said he had a concussion.
The employee was pressure washing and he brought the wand in front of his body when it caught on his rain suit. Then the wand sprayed 180 degree water into his rubber boot. This blistered the top of his left foot.
The employee was trying to hold one end of a auger head when the weight shifted to his side and he felt something pull in his back.

The employee moved a rock truck and the bed free fell there fore shaking the employee. His back & neck was bothering him. He said he didn't need to go to a doctor. Went to doctor at later date. The employee was pulling a pin out of a highwall miner beam, when the pin got stuck and he attempted to pull harder and he didn't use his legs to pull the pin, he instead used his back and it resulted in a strained back.

The employee was using a 20 pound sledge hammer to hit the pins on a beam for a highwall miner. He raised the hammer over his head and came down with the hammer, when he felt something pull in his back. He worked the rest of the shift and went home and had to go the ER becuase he said he was in pain.
A cap from an oxygen cylinder fell off top floor of the plant fell 45' below and landed on the EE's right hand causing a laceration that required two stitches. The EE missed one day of work and returned to regular duty on his next sch. shift.
EE was hooking chains to a cab to unload it off a step deck trailer. EE forgot it was a stepdeck assumed it was flat and fell. EE landed on his back and then rolled off the lower portion to his head.

There had been a small shower and the road was slick. The water truck driver started out from the pond and up the hill. He had almost got to the top of the hill when the truck spun out. Then the truck went backwards down the hill about 130 feet. The truck went up the spoil on the driver side and turned over onto the passenger side of the truck.
Employee was unstopping oversized rock chute. A big rock was lodge in chute, it dislodged, fell down the chute, bounced and struck employee in leg. Employee received medical treatment and lost 5 days.
The employee had a come-along hooked to the nose cone of the dozer then was releasing the come-along and there was more slack came in the come-along than expected. The employee tried to get out of the way and to stop the nose cone but was unable. The nose cone rested against his leg and the blade. It had his leg until they jacked the nose cone back off.
The employee was attempting to get out of his truck and stepped down to the step on the fuel tank. His boots being muddy his foot slipped and his tail bone hit the fuel tank. He didn't go to the doctor for a couple of weeks till his neck started hurting him. He has not missed any work from 5/12/10 to 12/6/10. The doctor wants him off till after he recovers from the surgery.
Dozer pushing in shot, blade contacted undetonated booster resulting in explosion in 1-5 Pit Tip Top.
The employee was airing up a two piece rim for a mack truck. The rim broke and struck the employee in the upper part of his left arm, this broke his arm.

Employee states he was using a utility knife to clean 2" pipe. The knife slipped, cutting employee's left forearm. EE received medical treatment 7/13/09 and returned to full duty capacity 7/14/09. No lost time.
It had rained and the employee was climbing down from the service truck when he got his foot tangled up in a chain and slipped and fell. He landed on his hip. It was also muddy since it had rained.
