EE maintenance Worker for KCT, on 04/22/2013 at approximately 2:00 pm, was reconditioning roller frames on a 226 D9R Dozer. EE was driving the equilibrium bar pin out, and as it was 1/4 inch from being out, ee placed a bolt in it, and was holding onto the bolt when the equilibrium pin came out and the roller frame fell and pinched ee left thumb.

Mostrando los 22Employee was attempting to adjust broom bristle on sampler tail piece with pry bar. Pry bar snagged moving belt and smashed employee's fingers between pry bar and belt resulting in fracture to left ring finger and laceration to left pinky finger requiring 4 stitches.
Employee was performing pre-operational inspection of Loader and saw that A/C vent housing above his head in the cab was loose on one side. Employee attempted to snap vent housing back in place and shoved hand through the vent approx. 5 inches and made contact with fan blades resulting in lacerated and broken ring finger on left hand.
Employee stepped down to ground level from the auger house stairs and as he went to turn to the right to leave the area, he twisted his left ankle resulting in a fracture.
Employee was unloading empty pallet from back of mechanic's truck and when he stepped to the ground his knee gave out and felt pain. This is the same knee that was injured in 7/2011 requiring surgery where the employee was off until he was released to return to work on 4/9/2012.
Employee was trimming saplings off the river bank and while in the process of setting the chain saw down the chain saw struck the inside of left leg under knee resulting in laceration requiring 16 stitches.
Employee was walking in the Ceredo rail yard after coupling two locomotives together and stepped on a piece of ballast rock causing him to roll his left ankle resulting in a fracture.
Employee was rolling up a grease hose and in the process the grease gun struck the corner of the bed on the grease truck causing the trigger on the grease gun to depress injecting grease into the left index finger of the employee. This resulted in surgical removal of the grease.
Employee was mounting a parked locomotive on CSX Yard 1 West and slipped bruising his left side ribs and back. Employee was placed on restricted duty by doctor in that the employee couldn't climb or lift greater than 10 lbs for 48 hours. Case turned into lost time due to out company not having light duty positions.
Employee had just dismounted a loader being repaired in the shop area and after placing both feet firmly on the shop floor the employee turned to his right and felt a pop in his left knee. Employee did not seek medical attention until 8/15/2011 and was placed off work for 1 week by his physician.
Employee was working with 7 other employees in the Dump 1 Feeder Room to clean up a large coal spill resulting from a malfunction of the # 3 Feeder Belt. In the process of shoveling the spilled coal onto the belt to clean up the area, the employee sprained his ankle by stepping backwards onto a lump of coal. This resulted in a lost time accident.
Employee was loading a small electrical motor in the back of his company truck and pinched his left pinky finger between the motor and the truck bed resulting in a laceration requiring 2 stitches.
Employee was dismounting a railcar and when his left foot touched the ground he felt a "pop" in his left knee resulting in pain.
Employee was in process of pulling crabline out of water and placed hand in pinch point situation where his right wrist was pressed between the monkey line and boat, ultimately resulting in a bruised wrist.
3 employees were performing welding activities at the same time and in the process one employee was exposed to an arc flash from another welder resulting in temporary pain in the eyes. Employee sought medical attention at an eye doctor and was given one day off of work.
Employee was placing a piece of scrap metal into a loader bucket for disposal into the scrap metal bin and in the process, pulled a muscle in his right bicep area, resulting in two days away from work.
Employee was removing lodged lump of coal with slate bar. In an attempt to free the material, the slate bar & lump fell out of the bin area striking employee's right leg ultimately causing a scrape on the leg. Employee refused medical attention initially, but went to family doctor on 5/26/09 because of redness. Doctor put employee off work for rest of week for infection.
Employee was standing inside locomotive preparing to dismount when a faulty air valve on one of the railcars caused the train to enter emergency mode and stop. This unexpected stopping motion caused the employee to fall inside the locomotive and hit his head. Doctor put employee off for two days.
Employee was attempting to sit down inside company vehicle and in the process, felt severe pain in his lower back. This resulted in a strained lower back and two lost time workdays as per doctor's advice.
Employee had just finished fueling his locomotive and in the process of dismounting the loco to replace the nozzle, he stepped on the fuel hose and rolled his ankle. This resulted in the employee spraining his ankle.
Conductor mounted the cut of loaded railcars to protect the move to the hill, and as he mounted the railcar, pulled a muscle in his right shoulder. Employee was instructed by doctor to be off work and attend physical therapy for a potential muscle tear. Foreman was not made aware of incident until 11/30/2008.
Employee was attempting to enter the cab area of Locomotive 1200 and in the process bumped his head on the low clearance door facing. Employee heard his neck pop and now has a strained neck. Employee was wearing a hard hat. Employee is taking physical therapy for his neck. Didn't learn of the incident until 11/20/2008.
