Mostrando los 9Employee was working on the conveyor cover on continuous miner 101 while standing on step ladder. A coworker was moving a wooden pallet and accidentally bumped the step ladder with the wooden pallet while operating a TH. The step ladder became unbalanced causing the employee to fall from second rung of step ladder landing on left leg. Medical Treatment Only.
Collar door switch at #2 shaft collar faulted out. Due to this fault the hoist operator did not know if the door had closed or not. This made it unsafe to operate.
Employee was exiting cab of dump truck. In order to exit cab safely employee needs to utilize the three vertical steps and two hand grips. Employee was on the first step when EE lost EE's grip and fell approximately four feet to the ground.
Employee was using a stationary wire wheel to remove metal burrs from a short unistrut section, +/- 5" long
Employee was repairing a 980K wheeled loader. In the process his right thumb got pinched between two components resulting in a minor laceration to his right thumb.
There was no accident injury or illness of persons. A spring cup of one of the brakes mechanism was showing signs of ware and had to be replaced. It took more than 30 min. To trouble shoot and install the hoist was back in production at 2:00pm
Preliminary Investigation: The operator turned his head to face forward while applying brakes. The braking process resulted in his nose striking the canopy.
While working to disassemble a piece of equipment, the suspended load shifted and pinched his left thumb
