Employee was helping a co-worker work on a tail pulley. Employee was turning a threaded rod while a co-worker was hitting a bearing with hammer, bearing came loose suddenly and the treaded rod took skin off EE's fingers.
EE was installing battery in Haul Truck when EE reached down to pick battery up EE stated they strained back. Did not go to Doctor & no lost work days. Decided to go to Doctor in April no loss days. Went back May 6 Doctor put EE on a modified duty but with restrictions, we had no job so EE is now out of work til next Dr. Appt on May 16.
Employee is our QC/QA Testing Technician. He picked up a 5 gal bucket of material to dump out when he felt pain in his lower back and leg.
Employee climbed into loader to start up then dismounted to perform pre-shift. When he stepped down from bottom step he turned to go round front of loader when he felt pain in lower back causing a sprain lumbar
Employee was helping set water pump in pit sump he stepped out of way, felt something pull in lower back, stated he was ok. Continued to work through 1/6/14. Due to weather was told to stay home on 1/7/14. Employee called in on 1/7/14 said he was going to ER due to the incident that happened on 12/5/13. He received no medical treatment and no lost time till 1/7/14
