While checking rotation of the piggy backs in the pit, the pig was rotating backwards. Employees switched the electrical wires inside the junction box. When turning the power back on it caused an electrical short which made the employees run into one another. Our employees fell to the ground injuring their right shoulder. The employees were located 200 feet away.

Mostrando los 20Employee had been doing maintenance on the shakers at Perry Wash Plant. EE was walking down the stairs and when EE got to the end EE stepped on a piece of gravel. This caused EE to lose balance and fall on the same level surface. EE was diagnosed with a right twisted ankle.
Injured party was in line to sign up in for a huddle up meeting. The other party broke in line and mouthed off with profanities at the injured party. The injured party rushed towards the party who started this altercation and then shoved the injured party, who then fell backwards and landed on a truck trailer hitch/ball with EE's buttocks, thus the injury. Injured employee assisted in maintenance of log washer. Employee was standing on walkway holding journal of log washer. As EE was doing this another employee was in the tub of the log washer removing the bolt. As the bolt was removed the journal became unstable, pinching three fingers of the employees left hand.
Employee was changing a roller on a field conveyor. Employee was holding one side when it suddenly fell pinching the middle finger of right hand between the roller and conveyor frame. This resulted in a contusion to right hand and also injured the fingernail of the middle finger of the right hand .
EE was pulling off a Roller from the conveyor to replace, as EE grabbed it to remove as EE and helper pulled it EE's finger was caught in roller, it crushed and cut middle right finger
Employee was assisting another employee to remove a nut and bolt from a conveyor roller frame. The injured employee was using a wrench to hold the bolt as the other employee was turning the nut. The injured employees wrench slipped resulting in the ring finger of EE's left hand being struck against some angle iron or conveyor frame.
Employee was climbing into the classfier to clean a screen. They stepped onto a brace, their feet slipped, and they fell against another brace. Employee has a contusion of the left shoulder and thorax.
Employee was standing next to an electrical panel as another employee removed a ground cable using a wrench. The wrench suddenly slipped striking a positive connection which resulted in a electrical flash burning the supervisors that was standing next to the panels right forearm.
Employee states EE and another employee were carrying a pipe. Each employee was positioned on an end of the pipe. one employee suddenly dropped EE's end resulting in EE's right shoulder being strained.
Employee was performing conveyor maintenance. Employee attempted to climb on conveyor and slipped and fell. EE's left wrist was impaled by bolt attached to the conveyor as EE attempted to grab the conveyor when EE was falling.
I had changed the cables on a crane from a single to a double. The crane operator boomed up as my hand was on one of the cables causing might pinky on my right hand to be pinched between the pulley and cable.
Employee was installing paddles on the log washer and felt back pain. EE previously reported that on 10/13/16 EE had felt similar pain when EE was washing the shaker platform while pulling on a 2 1/2 inch water hose. EE requested to seek medical treatment on 10/31/16.
This was not a Memphis Stone employee and there was no incident. A crusher operator discovered a customer(truck driver) in the cab of truck unresponsive and notified the foreman in which emergency medical help was summoned through the use of 911. Paramedics arrived and transported to the hospital where passed away. There was no event that occurred which caused this.
Employee was putting a battery on a John Deere excavator when the battery suddenly blew up. The employee received initial evaluation on the day of occurence for ringing in ears and given no diagnosis. Employee was referred to an audiologist for evaluation.
Employee states he was removing a cutting edge from a bucket. He was using an impact wrench to remove a bolt that secured the cutting edge on the bucket. As he was removing this, the nut began to spin, and he grabbed it to secure it from moving. When he did this it caused a laceration of the index finger of the right hand.
Employee was training employees on how to train a conveyor. He states he was using a hammer to tap the rollers for adjustment. He completed this and walked to his truck and as he began to open his truck door he felt back pain.
The employee was using a utility knife to cut a piece of conveyor. The knife slipped and lacerated the employee's right leg.
Employee states he was prying on a wear plate that was to be removed from a front end loader bucket. The pry bar broke resulting in it striking him above the lip.
Employee was involved in moving a 55 gallon drum from a Bobcat onto a pallet. As he was standing the 55 gallon drum up his middle finger on his left hand was caught between that drum and another drum that was on the pallet.
