Mostrando los 9North Pit crushing plant screen 1 feed belt breaker tripped during regular operations. Plant operator (injured miner) seeing no other malfunctions, opened the electrical cabinet to reset the breaker and re-engage the starter motor, which caused an electrical flash arc, causing burns to the operators right arm. after investigation, it was determined proper LOTO was not performed.
Miner 1 was holding a roller frame during the installation process. Miner 2 was driving the frame in place with a 3 lb. hammer. Miner 2 struck the index finger of Miner 1, causing a small fracture on the tip of the index finger of Miner 1.
The injured miner was using a Caterpillar D-10T2 bulldozer to push material parallel to a bermed bench. While traveling in reverse the dozer drove off the downhill side of the bench and rolled over, ending up on its tracks. The total damage to the D-10 is unknown, but the machine appears to be structurally sound and repairable.
The Miner was using a handheld grinder on steel without safety glasses and a face shield. A steel shaving contacted EE's right eye.
The miner claimed that the air ride in the seat of the 824 John Deere loader was not functioning and the ride control was also not functioning causing lower back to ache.
Miner slipped while descending a piece of crushing equipment and caught self before falling. Employee felt a pain in employee's stomach two days later and reported the incident. Employee has been diagnosed with an abdominal hernia.
EE was on a screen catwalk, shoveling spilled material off the catwalk from a kneeling position. EE set EE's left hand on the material pile to re position self. Another Miner, who was helping loosen the material pile with a 6' digging bar, struck EE in the hand with the digging bar.
The Miner was standing over a 3" discharge waterline from a diesel powered pump. The discharge waterline had a small leak in it. When the line became pressurized, it tore striking the Miner in the left knee.
