Employee was assisting moving a 50ft. section of poly pipe when it got hung-up on something. Once it was free it "popped", and the pipe struck employee on right knee and right hand.

Mostrando los 23Employee slipped in a muddy area however did not fall but sometime later felt some back pain. On 7/1/2024 employee reported this to the manager, and EE was seen at the Med Clinic for a lower back strain.
Employee was changing cutting edges on 988 loader and placed left hand on bolt head which cut palm of hand when tightened.
Employee was using nylon rope when laying poly pipe. The nylon rope was weathered and frayed. The rope broke and the poly pipe hit employee on the shoulder upper arm. Employee was transported to hospital, evaluated, no injuries found only possible minor bruise. Employee was released with full duty release on 10/5/2023.
Employee was working on a VSI top plate lid and was lowering it when it lost pressure pinching employee's hand between the outer housing of the hydraulic cylinder and lower bushing. A loader was used to raise it back up and free employee's hand. The employee was taken by vehicle to medical center.
Individual (non-miner) was working on Loader (980m) when EE turned around and headed back to the truck. Individual lost balance and collapsed. EE was transported to the Medical Center and was treated for factors resulting from stress, anxiety, severe dehydration and low iron. No other factors were contributing for this Illness.
Employee was spotting a skidsteer bucket to collect some loose material. The operator of the skidsteer postioned bucket on employee's left foot causing a laceration which required approximately 4 stitches.
Employee was shoveling and reported that they injured shoulder while shoveling material at plant.
Employee felt sick from dehydration. Employee stated had been drinking water however was not sweating. Employee was taken from jobsite to emergency room and was treated for dehydration. This incident happened on 5-31-2022 employee returned to work on 6-2-2022 with a full duty release to work.
Employee was dismounting excavator and lost footing and twisted right ankle. EE has been continuing to work regularly and has had only observation type medical appointments, however on 4/11/2022 EE was given an injection and has been taken off work for 2 weeks as prescribed.
Injured employee was removing cutting edges on a loader when EE's elbow made a popping sound. Employee was treated at doctors office.
Injured employee was cutting conveyor belting with a box cutter towards EE's body and punctured just under right knee.
Injured worker was using a hand held shovel and as they stepped on it to dig they strained their left calf muscle.
Injured worker was cutting a belt for a conveyor system when the utility knife slipped and cut left wrist/lower arm.
Employee was working on a conveyor belt using air impact gun. While using the impact gun, the clamp that holds the quick connect hose came undone and the hose whipped around and struck employee on lower lip, causing small laceration in inner and outer lower lip.
Employee was climbing down off of loader at the end of the day on 1/24/2020 and felt an elbow pop. Employee informed the supervisor of the incident on 1/27/2020.
Injured worker was working on 1/14/2020 when we reported that EE had injured self the previous day while changing crusher shoes. EE stated that EE pain in EE's chest when EE moved or went to pick something up and EE would have to stop and take a minute until the pain subsided. As soon as this was reported EE was taken to the clinic to be evaluated.
Injured worker was eating lunch and felt a poking EE's leg and when EE looked at the bite area it was red and itchy. The next day the area was red and swollen and painful, so EE was taken to Dr. to be checked out. Worker was treated for insect bite and prescribed medication and returned to work.
Injured employee was utilizing a sledge hammer to remove teeth from the excavator bucket when a piece of metal broke off striking and puncturing employee right above the left knee.
Injured employee was cutting leggings when EE cut left hand below the thumb. Employee was wearing gloves when this accident happened.
Injured worker was loosening a hydraulic line letting some pressure off when the line came loose and hit EE with the pressure of the line
Injured Worker was unloading anvils did not complain of any injury, the next morning ee showed up to work with bruising and a small knot located on left bicep. Injured worker was sent to the clinic for evaluation which diagnosis is pending an MRI to be certain.
Injured worker was changing a grease line hose and employee was hooking up harness to cable system and cut finger on a sharp piece of metal
