Two maintenance employees were working to replace broken bolts on the longwall stage loader chain flights. One employee was swinging a hammer to free the flight and the other employee was pulling the chain and was struck by the hammer requiring 13 stitches.

Had a roof fall above the anchor points in the return of the tailgate of the longwall.

Employee was evaluating materials and found a roof fall above the anchor points at 299 15HG #1 crosscut.

Employee was evaluating areas for material storage and found a roof fall above the anchor points at 458N in #1 room.

Supervisor was walking the 14hg area of the mine to ensure that there was no equipment, or tools that needed to be removed before the stoppings were installed. During EE's inspection EE noticed that the top was looking like it was deteriorating. EE contacted engineering and when they arrived, they walked the area and discovered the roof fall.

Had a power trip on the primary man hoist (8 shaft) and one of our secondary shafts was locked out for maintenance. Power was restored to the primary man hoist at approximately 8:15PM.

HVAC Technician was working on HVAC unit on above ground concrete pad on 12/10/24 that was covered with ice. The technician slipped and jammed shoulder when EE went to catch self. The technician tried taking care of it for a few weeks, but it was still hurting on 1/3/2025 so EE went to seek medical care and was issued work restrictions and prescribed medication.

Employee was changing out a leaf spring & when it broke free it pinched employee's hand requiring 13 stitches.

Employee was removing a bolt from a junction box when EE received a shock to EE's left hand.

Employee tripped landed on EE's hand sustaining a fracture.

Employee rolled EE's ankle when walking into a building causing a sprained ankle.

8 Shaft hoist went down while taking the night shift crew into the mine. A ground fault issue on the surface caused an undervoltage event. There were 2 faults that occurred on the hoist motor. The first was found relatively quickly, but the second one took some time to find. Therefore, the crew was stuck on the cage for over 30 minutes. We notified MSHA at 11:25PM.

While working in the bucket elevator the employee stepped down on to a beam, causing the employee to lose balance. Employee reached with left arm which dislocated employee's shoulder.

Employee went to grind on a piece of pipe, when the cord got entangled, went across employee's hand causing a laceration requiring 9 sutures.

The 8 shaft hoist went down due to an electrical issue after troubleshooting, the electrical issue was resolved. There was one miner on the man hoist when it went down and was hoisted out at approximately 10:20PM.

Employee bumped a valve handle on an airline and came into contact with the debris in the line.

We experienced a power trip on the PPL's mainline that caused the underground mine to lose power. There were approximately 27 miners underground until secondary power was established at approximately 2:15AM.

Employee was ascending stairs, tripped on a step, and stuck hand out and it went through glass light. Employee sustained a laceration to the right index finger requiring 6 stitches.

Employee was putting away tools, climbed up on the tool truck, felt a shock as EE's right knee touched the tool trip. EE reactively stood up and hit head on the mine roof. Employee was taken to the ER as a precautionary measure and fully released on 6/19/24. Due to persistent shoulder pain, EE received an MRI and was put off work for further medical evaluations on 7/29/24.

While prepping a pipe penetration another employee was grinding on a piece of stainless pipe in the immediate area. Employee immediately felt something in eye.

7 Shaft hoist went down due to electrical issues with two miners on the cage the electrical issue was resolved, and the miners were hoisted out at 10:15am.

Miners were receiving elevated levels of CO and as a precautionary measure, evacuated the mine. Approximately 10:44PM miners found that there was a belt fire located at 2nd 54 drive. The area was smoldering when found the miners applied water to cool the affected equipment and flagged the area.

Double tie line trip on Rocky Mountain Power caused a power outage at 8 shaft rendering the man hoists inoperable. Power was restored at 10:53AM all miners were hoisted out at that time.

Employee smashed right hand loading structure onto a trailer requiring 3 sutures.

Employee was blowing out drag conveyor and got dirt in their eye that caused irritation. EE was given a prescription.

A roof fall occurred above the anchor points at 592 South XC 168, area was flagged off and MSHA was immediately notified.

Employee was turning valve, felt pain in shoulder, after MRI was put on light duty.

Employee was dismantling a cable handler, when the cable handler slipped pinching employee's finger causing a laceration and partial amputation of the pinky finger.

Employee was opening a valve when the handle slipped off, hit their tooth requiring dental repair.

Elevator tripped at Mono Utilities contractor was stuck in the elevator and released at 6:52AM. Safety was notified at 7:25AM and contacted MSHA at 7:35AM.

Employee was steadying box when the box fell, pinching EE's hand causing a fracture. Employee was returned to work with no restrictions later referred to a specialist and prescribed work restrictions on 11/1/23.

Employee was removing protective covering from blade and cut a finger which required stitches.

Employee was training the belt and strained shoulder, was prescribed restricted duty.

EE was hydroblasting a heat exchanger in the bicarbonate unit. At approximately 15:08, the barrel of the shotgun failed and broke at the mfg weld. This caused EE's hands to rise and hit EE's faceshield with the shotgun trigger assembly. This action caused EE's faceshield and hard hat to strike EE's glasses causing a cut to EE's face that required stitches.

Employee was working on a pump, when opened a drain valve and was sprayed with hot liquor causing burns on their arms and stomach.

Employee rolled ankle on mine floor was given a full medical release on 09/04/2023. On 10/10/2023 went to see an orthopedic and was put off work for two weeks.

Employee was walking to the cage and rolled their ankle causing it to dislocate.

Employee was working on removing guarding and received a laceration on the expanded metal requiring sutures.

Employee was walking on grating when the grating shifted causing the employee to fall on left knee. Employee was given work restrictions and medication.

Roof fall at xc 144 592 west roadway above the anchor points L-15' x W-20' x H-7. Access nor ventilation is blocked and, there is no active work inby the fall. The roof fall is barricaded off.

Employee was assisting another employee hold packing around shaft and received a laceration requiring 5 stitches.

Employee was hammering on a crossover and injured EE's shoulder. Employee was fully released back to work that day. Employee saw another medical provider that put EE off work on 5/9/23.

Three employees were stuck in the 8 shaft cage for approximately 3 hours.

Employee was moving belt; belt slid and impacted the employee causing a cervical sprain.

Employee was working on a crusher insert, when employee cut a piece of metal, it folded in on employee's hand, causing a 3rd degree burn to employee's wrist.

Employee was loading a portable A/C unit into an elevator when employee's hand was smashed between A/C and elevator doorway, causing a laceration requiring sutures.

Employee was using a die grinder and a foreign object got in EE's eye. Employee didn't report the incident until 9/21/22.
