Subcontractor employee was administering maintenance, took a break, went to employee's pickup truck. Was found in the vehicle unresponsive. Cardiac arrest. Employees notified 911 and administered CPR until paramedics arrived. Employee was then transported to local hospital, where employee was pronounced deceased.

Employee was using a hose to clean under a plant. EE squatted down with the hose and felt a pain in EE's back.
Miner was shoveling material off of a belt and felt pain in EE's back.
Mechanic slipped off the damp track of an excavator. EE fell and hit EE's head.
Employee was cleaning the chute on the wash plant, trying to remove the angle iron on the bottom of the chute. As employee drove a steel pry bar in between the angle iron and the chute, the bar pinched employee's left middle finger against the chute.
Employee was changing the screen out on a screener deck, which was locked out. The adjacent conveyor was still running, however. As the employee lifted the screen, the screen was pulled into the running conveyor, pinching left index and middle fingers against the running conveyor belt. Employee suffered a laceration and tendon damage to left middle finger.
