Employee was using a hose to clean under a plant. EE squatted down with the hose and felt a pain in EE's back.

Miner was shoveling material off of a belt and felt pain in EE's back.

Employee was cleaning the chute on the wash plant, trying to remove the angle iron on the bottom of the chute. As employee drove a steel pry bar in between the angle iron and the chute, the bar pinched employee's left middle finger against the chute.

Employee reported pain and swelling right foot and ankle from repetitious throttle motion in dozer. History of gout and tendonitis. Employee operated dozer for 1 day.

Climbed out of chute and jumped down about three feet and slipped on frozen ground, twisting his left ankle.

EE WAS STEPPING OFF LOADER AND FELL-FF DECK OF LOADER FALLING APPROX. FIVE FEET DOWN TO GROUND CAUSING A POSITIVE FRACTURE TO THE RIGHT WRIST AND HAIR LINE FRACTURE OF THE LEFT WRIST.

EMPLOYEE WAS CLIMBING A LADDER WITH TOOLS IN HAND. HE SLIPPED AND FELL DOWN THREE STEPS AND LANDED ON A ROCK, SUBSEQUENTLY TWISTING HIS LEFT ANKLE.

EE WAS USING A PRY BAR TO REMOVE STONE JAMMED INCONVEYOR, HAND SLIPPED/LACERATED HIS LEFT PINKY FINGER.

WAS WALKING DOWN THE CONVEYER AFTER GREASING THE HEAD PULLEY IT WAS REALLY COLD OUTSIDE SO THEIRWAS FROST ON EVERYTHING AND HE SLIPPED AND FELL OFF THE CONVEYER WHERE THE LADDER WAS SET UP ON THE BOTTOM OF THE CATWALK, FALL WAS ABOUT 5-6 FEET FROM GROUND, BRUISED AND SWOLLEN MUSCLES IN HIS BACK.
