EE said that EE went to inspect the Drag Drum and Cable after EE positioned the machine to correct the Backlash. EE re positioned the guard, EE slipped and fell into the clutches which ripped EE's boots and cut EE's ankle and leg.
Welding h-beams and channel inside hoppers when employee picked up a beam and felt pain in left shoulder.
EE was changing broken rear step on a 980G loader, When EE was taking off the bolts that hold the step on with a impact wrench, the bracket spun while EE was holding the rubber mount and the Rusty Bracket cut EE's left hand.
While painting 992 loader bucket, the step ladder that was leaning against bucket slipped back and fell flat to the floor. This caused the employee to fall and while trying to catch self EE jammed EE's left shoulder.
While using torch to remove and replace shaker screen, a piece of debris hit the employee's hard hat and fell behind cutting PPE. Employee was taken for evaluation at medical facility and returned to work with no restrictions.

EMPLOYEE SET A STICK OF 20' ANGLE IRON ON 2 FLOATS WHICH MADE IT 3' OFF THE GROUND TO CUT IT INTO PIECES TO MAKE FLOAT PIPES. WHEN CUTTING OFF A 39" PIECE (THE 20' PIECE) FELL AND LANDED ON THE EMPLOYEE'S FOOT.
