Mostrando los 21Employee was starting to climb the ladder up on to the end loader when with left leg on the ladder and right foot still on the ground they felt a pop, (pain) in right knee while pushing their weight upon the ladder.
Employee was in a sitting position welding on the front side of a D11 dozer blade when employee noticed movement of the blade. While attempting to move out of the way employee turned right ankle causing a displaced fracture of the talus bone in the ankle.
Employee was walking at the west spoon fuel island and tripped on uneven ground, stiff armed the ground with left hand. At the time of the report EE stated EE was fine and that EE just wanted to report it.
HT loaded by shovel, 1st bucket the door opened unexpectedly, material fell on the dovetail causing the bed to raise up, slam back down. HT driver experienced leg pain upon impact, was seen at the ER for general body aches, given a work as tolerated release. On 11/30/20 employee informed the mine operator that EE had been receiving physical therapy which started on 10/09/20.
Employee was changing a bottom roller bearing on a 475 Komatsu 475 bulldozer. While positioning the roller the roller fell off of the blocking and caught the employees right pinky finger between the shaft and the ground.
When replacing the control arms on the front of 21.4567 service truck the mechanic was using a pry bar to get the control arm into place when the bar slipped off the control arm causing the control arm to fall on EE's left thumb causing a fracture to the tip of the thumb bone.
Mechanic leaned an 8 ft ladder up against the outside right front haul truck tire to perform an inspection. EE was descending the ladder, the ladder slipped and fell to the ground, the ladder landed on the mechanics right hand and they landed on top of the ladder. This caused a dislocation injury to the right hand. Employee had surgery on 6/2/20 to repair the injured hand.
Lightning initiated a grass fire on the south property line at Belle Ayr today. The Campbell County Fire Department was dispatched and put the fire out. The grass fire consumed approximately 11 acres of grass mostly Cordero/Rojo's property and only about 1 acre on Belle Ayr's property along the fence line.
Employee was removing the water truck tank drain hose, the hose popped loose from the fitting striking the employee in the mouth. The impact resulted in a contusion to the lower lip and chipped the front left tooth. The tooth will need to be capped and the employee did not miss any scheduled shifts.
Mechanic cut out a driveline bracket with a torch. The bracket started to fall, EE grabbed it to help guide it to the ground. The bracket fell, pinched EE's index finger between the driveline and bracket causing a crushing injury that split the index finger open requiring 6 sutures to close the wound. Employee wore welding gloves at the time of the incident.
Employee was walking to a pickup when EE stepped into a hole twisting EE's knee. Employee was evaluated at Black Hills Urgent Care and received a knee brace. Black Hills Orthopedic suspects a meniscus tear and ordered an MRI. The Doctor has taken the employee off of work at this time until further information is available after the MRI.
Employee waiting to be loaded was found unresponsive. A mayday was called, rescue team arrived and 911 was phoned in. Rescue team started CPR until ambulance team arrived took over and worked on patient for 15 to 20 minutes called employee deceased 13:56. Employee was then recovered from truck for coroner.
Employee was removing lift cylinder for drill 10108 mast. Mast was secured hydraulic lines bled off, but when cylinder was cut the mast dropped 4-6 inches striking employee on hard hat. Employee went to coalition and saw Dr where EE was diagnosed with concussion and not allowed to return to work. Employee will return to work when concussion symptoms are gone.
While moving the Cat bulldozer off of the spoil to have maintenance work preformed, a fire was discovered under the operators cab floor board. The operator immediately shut down the equipment and fire extinguishers were used to try and extinguish the fire. fire department was called to finish putting out the fire.
A fire was discovered at the North Pratt Bathhouse area in a location of old offices that were locked off to the miners.
Employee arm-fingers swelled 1 hour after using torque wrench, taken to ER received work as tolerated release. 4/01 ESM notified, employee would need shoulder surgery 05/06. ESM disputed claim, 06/05 WC review surgery payment denied. 9/29 ESM notified by State of Wyoming Medical Commission honored in favor of employee WC claim, shoulder injury linked to incident on 01/15/20
Employee was walking down the ramp to a low boy trailer when EE's feet slipped causing EE to fall onto the ramp
The Employee was hooking up a fuel line to a bull dozer when EE twisted EE's body to turn around and felt pain in EE's lower back and leg.
Operator tramming track dozer 26641, encountered a rock caused operator to inadvertently set park brake. Machine suddenly stopped, operators left shoulder jammed causing a shoulder strain. Employee was given a work as tolerated release 1/2/2020, then on 1/9/2020 notified the mine that the MRI showed rotator cuff tear, taken off work 1/9/2020. Begin physical therapy 1/13/2020.
Employee was in the office, experienced chest pain and slumped over. Rescue team administered CPR and used an AED. EMS was called to the mine and took over the first aid, employee was transported to the hospital, diagnosis was cardiac arrest and admitted to the hospital.
