Employee filled the water truck up and started back across the street to the hopper to blow it out, EE heard a noise sounding like a tire blew out and the truck started to tilt over to the right and then flipped 3 times, EE passed out for a minute and when EE came to, EE took EE's seat belt off and looked around to see if EE could tell anything leaking EE climb out
3rd party truck driver got stuck picking up a load. Employee used loader to help get stuck truck out. While backing up - the loader made contact with the front of the truck. This caused the truck door to close and make contact with driver's knee. The driver does not have an MSHA ID.
Cleaning build up at conveyor, wet muddy conditions. Stepped in soft material twisting, employee sought treatment for knee sprain.
Employee was dusting blinds in the office. While trying to reach the highest point of the blinds employee jumped up, and twisted right ankle.
Employee was at work for 4hrs and 20mins when they started feeling a sensation on their left hand and side. They tried to gather themselves and figure out what was happening. Coworker called ambulance to be safe. Paramedics evaluated employee and didn't find any cause. The emergency room didn't find the cause of the pain. Employee was sent home and given a few days to rest.

Employee was carrying out routine maintenance on the dryer drum drive belts. While changing the 2nd belt, the belt backlashed & pulled right hand causing a cut on pinky & ring finger
