Mostrando los 13An unplanned fire not extinguished within 30 minutes of discovery occurred on a Caterpillar 325c excavator on the course refuse disposal area of the preparation plant. The machine was owned and operated by Doss Enterprises, LLC. No injuries resulted from the incident.
Injured employee was attempting to install a roof bolt when the bolt bent, separated from the drill chuck, and struck EE's right forearm resulting in a fracture and laceration.

Injured employee was attempting to push a rock off the cable tray of the longwall panline when EE caught EE's left index finger between the rock and shield canopy resulting in a fracture to the tip of EE's finger and a laceration requiring sutures to close. No time was missed as a result of this incident.
Employee was working in soft ground conditions when EE's right foot became stuck. EE lost balance, fell, and fractured EE's ankle. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.
A dozer entered void on the #2 Met Stockpile at the #3 Feeder resulting in an entrapment. No injuries resulted.
Employee set up a 5' step ladder to assist with hanging a bag. When they stepped up onto the first rung the left leg shifted causing them to lose balance and fall. They used their left arm to brace for the fall. Their hand struck the ground spraining their thumb. Injury was not the result of a training or compliance issue.
