FACE OF #6 ENTRY WAY 27RM WAS BEING MINED WHEN ROOF FALL FROM FACE OUTBY 10FT TO ROOF BOLTS AND1 ROW OF BOLTS FELL OUT.
INJURED WAS OPERATING SC IN THE 4 WEST SECTION. SC WAS LOADED AND OPERATOR WAS TURNING AROUND INSEAT WHEN HE SLIPPED CAUSING HIM TO FALL AND PULLING LT. SHOULDER.
ROOF FALL IN BREAK THRU AT 22 RM, 5 TO 6 ENTRY WORKING HAD JUST CUT THRU FROM #5 TO #6 ENTRY. 18'WIDE, 52'LONG, AND 5' FELL.
CLEANING BATH HOUSE, BENT OVER TO PICK UP HOSE, PULLED BACK.
INJURED WAS PULLING DOWN A LOOSE PIECE OF ROCK. ROCK MEASURED 3" X 8" X 4 FT. ROCK FELL AND HIT INJURED ON LT ARM, CAUSING A CUT TO RT ARM.
ROOF FALL ON 4 WEST SECTION #4 BELT ENTRY BETWEEN 9 & 10 RM. FALL WAS 30 FT LONG, 7 FT HIGH AND 18 FT WIDE.
ROOF FALL IN 4 WEST SECTION 10 RM BETWEEN 2 & 3 ENTRY INTERSECTION AT #2 ENTRY AND #3 ENTRY WERE CRIBBED OFF AND FALL CAME BETWEEN CRIBS. FALL 5 FT HIGH, 17 FT WIDE AND 20 FT LONG.
ROFF FALL IN THE 4 WEST SECTION #6 ENTRY AT 8 RM. FALL IS 18'WIDE AND 15'LONG. FALL WAS ABOVE THE ANCHORAGE. VENTILATION WAS NOT AFFECTED. TRAVEL WAS NOT AFFECTED.
ROOF FALL IN THE 4 WEST SECTION #6 ENTRY 10 RM THE FALL MEASURING 20FT LONG, 24FT WIDE, &6FT HIGH. THIS AREA HAD BEEN BOLTED WITH 3 FT RESIN BOLTS
A ROOF FALL OCCURRED AT 29 RM ON THE 4 WEST. THE INTERSECTION FELL 20 FT WIDE AND 25 FT IN LENGTH. THIS FALL WAS IN BY THE WORKING SECTION.
THE INJURED WAS IN THE PROCESS OF REMOVING THE IMPELLOR FROM A PUMP. WHEN THE IMPELLER CAME OFF,IT WAS HEAVY AND FELL ON HIS HAND, CRUSHING TWO FINGERS.
WHILE CHANGING OUT SEGMENT A PIECE OF METAL WENTAIRBORN & LODGED IN EMPLOYEE'S LEFT EYE. HE WAS WEARING SAFETY GLASSES.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY THE MINE EXAMINER, DURING HIS WEEKLY RUN, AT INTERSECTION OF #41 ROOM OF #5 INTAKE EXCAPEWAY ENTRY OF 4 WEST MAINS. ALL MINING WAS STOPPED UNTIL F ALL CLEANED AND SUPPORTED. NO INJURIES, NOR VENTILATION AFFECTED.
WHILE REMOVING SOME BOTTOM BELT ROLLERS THAT HADBEEN PLACED ON THE BELT, ONE OF THE ROLLERS INADVERTENTLY ROLLED OFF THE BELT AND STRUCK THE INJURED ON HIS LEFT ANKLE.
AN UNINTENTIONAL ROOF FALL OCCURRED ON THE NO. 2 RETURN ENTRY BETWEEN NO. 50 AND NO. 51 ROOMS HAULAGE; THE FALL OCCURRED ABOVE THE ANCHORAGE POINT OF THE BOLTS. THERE WERE NO INJURIES NOR WAS VENTILATION AFFECTED; FALL WAS ABOUT 4' HIGH X 18' WIDE AND OCCURRED ON THE ENTRY BETWEEN INTERSECTIONS.
AN UNINTENTIONAL ROOF FALL OCCURRED ON THE NO. 3 CABLE ENTRY @ 22 ROOM ALONG 4 WEST HAULAGE. THE FALL OCCURRED AT AN INTERSECTION. APPROX. 8' HIGH AND 18' WIDE. THE 4160AC HIGH VOLTAGE CABLE W AS CAUGHT UNDER THE FALL AND HAD TO BE REROUTED. THERE WERE NO INJURIES NOR WAS VENTILATION AFFECTED.
WHILE ATTEMPTING TO SCALE A LOSE PIECE OF ROCK FROM THE FACE, THE INJURED SLIPPED AND TWISTED HIS LOWER BACK.
INJURED HAS HAD PROBLEMS WITH HIS HAND FOR SOME TIME. THE INJURY WAS DIAGNOSED AS CARPAL TUNNEL SYNDROME. HAD OPERATION ON 5-24-00.
WHILE DRIVING HIS SHUTTLE CAR, THE INJURED HIT AHOLE IN THE TRAM ROAD. THE SUDDEN BOUNCE CAUSED HIM TO HIT HIS HEAD ON THE CANOPY AND HURT HIS BACK. DID NOT START MISSING WORK UNTIL 5/4/00.
THE INJURED WHO IS A ROOF BOLTER, WAS ON HIS KNEES HANGING LINE CURTAIN, OUT OF SIGHT OF THE OTHER PERSON WHO WAS BOLTING ROOF. AS THE OTHER ROOF BOLTER MOVED THE ROOF BOLTING AHEAD TO INSTALL A BOLT, HE INADVERTENTLY RAN THE WHEEL OF THE BOLTING MACHINE ON THE FOOT OF THE INJURED PERSON.
THE INJURED WAS IN THE PROCESS OF UNLOADING POSTS FROM A SUPPLY WAGON WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK AREA.
THE INJURED WAS IN THE PROCESS OF TIGHTENING BOLTS ON A BEARING FOR A BELT DRIVE. WHILE EXERTING FORCE ON THE WRENCH THE WRENCH SLIPPED AND THE INJURED STRUCK HIS LEFT HAND AGAINST A BULL GEAR . HIS RING FINGER WAS LACERATED, REQUIRING 9 SUTURES.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE NO.3 CROSSCUT AT THE NO.4 ENTRY WHICH IS THE INTAKE ESCAPEWAY OF THE 9 RIGHT 4 WEST SECTION. THE FALL IS 18'WIDE, 25'LONG, & ABOUT 7'HIGH. THERE WERE NO INJURIES NOR WAS VENTILATION AFFECTED. THE I.E. WAS MADE TRAVELABLE BY POSTING OVER THE FALL.
THE BARGE LOADOUT HOUSE THAT HOUSES THE BARGE MOVING CONTROLS AND OTHER ELECTRICAL TRANSFORMERS AND BREAKER BOXES CAUGHT ON FIRE ON AN IDLE SHI FT. A TRUCK DRIVER TURNED IT IN AND A LOCAL FIRE DEPT EXTINGUISHED IT. THE INSIDE OF THE BUILDING WAS DESTROYED. THE FIRE LASTED FOR ABOUT AN HOUR. IT MAY HAVE BEEN STARTED BY AN OVERHEATED TRANSFORMER. NO INJURIES.
