DURING THE NORMAL MINING CYCLE A ROOF FALL OCCURRED SUDDENLY AND WITHOUT WARNING IN SLIP FORM STARTING APPROXIMATELY 39 TO 40 FT INBY SPAD NO 48 IN THE NO 1 ENTRY WHICH RESULTED IN FATAL INJUR IES TO THE MINER OPERATOR.THE FALL MEASURED APPROXIMATELY 2 TO 10 FEET IN WIDTH AND 0 TO 6 FT IN THICKNESS.THE MINER OPERATOR WAS LOCATED IN THE OPERATOR'S COMPARTMENT UNDER PERMANENTLY SUPPO
THE VICTIM AND CO WORKER EE HAD BEEN SENT TO THE POWDER MAGAZINE TO DESTROY DETERIRATING BLASTING POWDER THAT HAD BEEN DISCOVERED EARLIER THAT DAY. FOR SOME REASON, UNKNOWN AT THIS TIME, A PRE MATURE DETONATION OF EXPLISVES OCCURRED RESULTING IN FATAL INJURIES TO EE AND CRITICAL INJURIES TO OTHER. TRUCK OPERATED BY I.NAME TRAVELING DOWN HAULAGE ROAD,DRIVER LEFT TRUCK WHILE COMING DOWN HILL,SUFFERED FATAL INJURIES.TRUCK CONTINUED ON INTO DITCH AND STOPPED.

AT THIS POINT, THE MEN COULD NOT GET THE CAR COUPLED TO THE LOCOMOTIVE SO TWO OF THE EMPLOYEES HOOKED TO IT TO PULL IT TO WHERE IT COULD BE. AS THE CAR WAS BEING PULLED, THE VICTIM CAME FROM APPROX. 20' INBY THE CAR
I NAME WAS REMOVING A PIECE OF DRAW ROCK THAT WAS CAUGHT ON A STRAP (32"X 27"X 6" THICK) WE HAVE COME TO THE CONCLUSION THAT HE FELL ADN THE ROCK FELL ON HIM THE ROCK STRUCK HIM ON THE RIGHT S IDE OF THE FACE AND HEAD ALSO ON THE CHEST AREA

EMPLOYEE WAS DRIVING A 769 CATERPILLAR ROCK TRUCK HAULING REFUSE TO THE SLURRY POND WHEN HE BACKED UP TO DUMP HIS LOAD THE TRUCK WENT OVER THE EMBANKMENT FLIPPED OVER UPSIDE DOWN OUT IN THE SL URRY POND THE CAB SANK IN THE SLURRY TRAPPING THE DRIVER
EMPLOYEE WAS AT3THE LEFT SIDE OF ROOF BOLTING MACHINE WHEN HE APPARENTLY WENT IN BY THE ATRS SYSTEM WHERE A ROCK FALL OCCURRED CAUSING HIS DEATH.
A PIECE OF ROCK ABOUT 5FT WIDE 12FT LONG 0-5 1/2IN THICKNESS FELL STRIKING THE VICTIM CAUSING FATAL CRUSHING INJURIES.
VICTIM WAS AREPAIRING A VENTILATION CHECK CURTAIN IN THE 2ND LINE OF OPEN BREAKS BETWEEN THE (3NOS. 5 & 6 HEADINGS ON THE NO. 6 HDG. SIDE. THE SHUTTLE CAR CAME THROUGH THE CURTAIN FROM THE NO . 5 HDG. SIDE, STRUCK AND RAN UPON THE VICTIM FATALLY INJURING HIM. THE VICTIM APPARENTLY THOUGHT THE SHUTTLE CAR WOULD TRAVEL UP THE NO. 5 SENTRY WHERE THE CONTINUOUS MINER WAS.
