INJURED WAS OPERATING 20 TON LOCOMOTIVE. HE APPLIED ELECTRIC BRAKE TO STOP. ELECTRIC BRAKE DIDN\T WORK. INJURED THEN APPLIED AIR BRAKE WHICH DI WORK BUT MOTORMAN WAS AFRAID HE COULDN\T STOP IN TIME AND JUMPED OFF THE LOCOMOTIVE. HE FELL TO THE MINE FLOOR INJURING HIS RT. SHOULDER AND BACK.
A METHANE IGNITION OCCURRED IN THE 6 WEST SECTION I.D.#027. WHILE MINING IN THE NO.3 ENTRY METHANE APPARENTLY LIBERATED FROM A CLAY VEIN AND WAS IGNITED BY SPARKS FROM THE BITS. FLAME DURATION WAS APPROX. 4 SECONDS. NO INJURIES OR PROPERTY DAMAGE.
INTERSECTION AT 7+40, INTAKE ESCAPEWAY, HAD SETTLED ON CRIBBS. IN THE PROCESS TO GAIN ACCESS OVER THIS AREA, OUTBY TO 6+60 FELL IN. HAD TO RE TOUGHT ESCAPEWAY.
THE INJURED WAS ROOF BOLTING WHEN A PIECE OF COAL & SLATE RIB FELL FROM ABOVE & STRUCK HIM ON HIS RIGHT SHOUDLER & BACK.

INJURED WAS HELPING TO INSTALL A NEW 10H.P. MOTOR ON A 2" GORMAN RUPP PUMP. INJURED AND COWORKER WAS IN THE PROCESS OF PLACING MOTOR INTO POSITION WHEN INJURED LEFT THUMB WAS CAUGHT BETWEEN CO UPLER.
INJURED WAS INTHE PROCESS OF BUILDING A CRIB WHEN HE REACHED FOR A CRIB BLOCK A FELLOW WORKER TOSSED A BLOCK TOWARD CATCHING HIS FINGER BETWEEN THE TWO CRIB BLOCKS.
INJURED WAS MARKING A BLOCK THAT WAS TO BE MINED WHEN THE #2 S/C CAME AROUND THE CORNER ON THE SIDE OF THE LEG JUST BELOW THE KNEES. INJURED FINISHED THAT SHIFT ON 8-6-84 & WORKED THE NEXT DAY 8-7-84 BEFORE GOING TO T HE DOCTOR.
INJURED WAS BUSTING&CUTTING DRAW RICK OFF THE MINER WHEN HE FELT A PAIN IN HIS LOWER BACK. NOTE: THIS WAS A RIOCCURIANCE OF AN OLD INJURY. ACCIDENT HAPPENED ON 8-2 BUT INJURED DIDNT REPORT IT TILL 8-6-84
INJURED WAS CROSSING BETWEEN TWO COAL CARS AT THE LICK CK. LOAD OUT POINT. WHENIN CROSSING, INJURED CAUGHT HIS PANTS LEGON THE CAR COUPLING PIN, CAUSING HIM TO TWIST HIS LEFT LEG. THIS WAS A R EOCCURANCE OF AN OLD INJURY.

DRILLING HOLE TO INSTALL ROOF BOLT INJURED REACHED FOR DRILL STEEL & A PIECE OF SLATE 2\X6"X6" FELL FROM ROOF STRIKING INJURED ON LT SHOULDER ARM & HAND.
INJURED WAS REPLACING A BROKEN ROPE IN A RED DEVIL. HE WAS USING CHISEL TO REMOVE THE RIVITS WHEN A PIECE OF THE CHISEL BROKE OFF HITTING HIM IN THE LOWER ABDOMEN GOING UNDER THE SKIN.
THE INJURED ALONG WITH OTHERS WAS LOADING BY HAND AC CABLE ONTO A SUPPLY TRUCK WHILE THE SUPPLY TRUCK WAS NEXT TO HIM HE TURNED HIS HEAD AND STRUCK SAME ON PIPE STANDARDS LOCATED ON THE TRUCK.
THE INJURED WAS ROOF BOLTING WHEN A PIECE OF SLATE FELL FROM THE ROOF & STRUCK THE OPERATOR ON THE BACK.
INJURED WAS IN PROCESS OF DROPPING RAILROAD CAR. HE WAS UNDOING THE LATCH ON THE HOIST ROPE WHEN THE CAR LOADER BEGAN TO PULL THE CARS BACK, CATCHING THE INJUREDS ARM IN THE HOIST ROPE.
STEPPED IN HOLE AT CAR HAUL FOR OTERY DUMP. SPRAINED LEFT ANKLE.

A ROOF FALL OCCURRED IN 6 WEST AMINS SECTION ID #027. THE FALL WAS IN #6 ENTYR AT SURVEYORS STATION #13+62. THE FALL WAS APPROX 8\ HIGH.
INJURED WAS RECONNECTING A BURT OFF LEAD ON A AMMETEER ON THE TIE BREAKER OF A SUBSTATION. THE REASON THE LEAD WAS SHOT OFF TO BEGIN EITH WAS THAT THE AMMETER WQAS GROUNDED. THE INJURED DID T HIS WITH THE POWER ON THE UNIT. WHEN RECONNECTING A FLASH INSUED RESULTING IN T HE INJURIES SUSTAINED.
FALL OCCURRED AT 7430 TO 7460 1 FACE SOUTH WEST HAULAGE. FALL IS 8\H X 15\W 30\L, AREA WAS SUPPORTED WITH 8\ CONVENTIONAL BOLTS SPOT BOLTED WITH 6\ RESIN AND TIMBERS 14\ X 15\ X 17" INSTALLED ON 4\ CENTERS
THE INJURED WAS PLACING CAP BLOCKS UNDER THE CATS OF A FEEDER FOR BLOCKING WHILE THE FEEDER WAS BEING TRAMMED WHILE IN THE ACT OF PLACING THE BLOCKING UNDER THE CATS THE FEEDER SAT DOWN ON THE SAT DOWN ON THE BLOCKS CATCHING THE TIP OF HIS RING FINGER OF HIS LEFT HAND BETWEEN TWO BLOCKS CUTTING THE TIP END OFF.

AN UNINTENTIONAL ROOF FALL OCCURRED ON 007-0 SECTION IN THE OLD BELT ENTRY INBY & BEHIND THE NO 1 BELT TAILPIECE. THE FALL AVERAGED 15-20\ IN LENGTH & 18\ IN WIDTH AND 4 TO 4 1/2\ IN HEIGHT TH E FALL WAS NOT AFFECTING ANY AIRWAY OR ENTRY OF ANY IMPORTANCE
WHILE STEPPING OUT OF MOTOR INJURED LEFT FOOT WENT BETWEEN A TIE AND THE BRIDLE BAR OF SWITCH THROW AND BRUISED HIS LEFT HEEL.
THE INJURED WAS OPERATING CM WHEN A PIECE OF SLATE FELL FROM THE ROOF AND STRUCK THE CANOPY THEN FELL INTO THE OPERATORS COMPARTMENT STRIKING THE INJURED ON THE LT. INSTEP.
AN ENTRAPMENT OCCURRED WHEN A STEEL RAIL SET AS ROOF SUPPORT WAS DISLODGED BY A ON COMING LOCOMOTIVE THEREBY CREATING A ROOF FALL. THE LOCOMOTIVE OPERATOR WAS TRAPPED IN THE OPERATORS COMPARTM ENT FOR APPROX 5 HOURS DUE TO THE FALL.
ROOF FALL OCCURED IN GWM SURVEYOR STATION .
THE INJURED WAS ROOF BOLTING IN THE PROCESS HE WAS USING UP PRESSURE WITH THE DRILLING HEAD WHEN THE DRILL STEEL BEING USED BROKE AND THE SAME STRUCK HIM ON THE RIGHT SIDE OF THE FOREHEAD.

THE INJURED WAS BARRING DOWN SOME LOOSE RIB WHEN UNDETECTED LOOSE ROCK FELL FROM ABOVE STRIKING THE INURED ON HIS LEFT FOREARM.
