A timber cutter for Virco Inc. was cutting a tree on a mine site located in Mercer County WV. As EE was cutting the tree there was another tree lodged up against the tree that EE was cutting. When the tree fell it pulled the other tree down hitting the timber cutter in the back. (broken arm)(broken shoulder)(broken ribs)(cut on hand).
Replacing control valve on D11. Used swivel socket to take out bottom bolts. Had socket on impact. Safety ring came off the socket and the pin came out cutting his left hand through his safety glove.
Loose ground and employee fell injuring his left hand when trying to catch himself. Employee sought medical treatment on 5/2/13 and x-rays were negative and return to work 5/3/13. Employee on modified duty until 5/9/13 no lost time.
FAILURE OF ROOF STRATA- 10' OF UNSTABLE ROCK BETWEEN THE NO 4 & 5 SEAM. BELTED WITH 7' POINT ANCHOR BOLTS AND SUPERBOLTS IN INTERSECTION ANCHORED IN SANDSTONE ABOVE THE NO 5 SEAM WITH A CRIB I N INTERSECTION.

WE ARE STILL LOOKING INTO WHAT MADE TOP FALL. WE BELEIVE AT THIS TIME IT WAS A NODETECTABLE SLIP. WE ARE USING A 7' TENSION BOLT AT THE MINE AND THEY SEEM TO BE DOING A GOOD JOB EXCEPT IN THIS INSTANCE.

PUTTING FUEL IN THE FUEL TRUCK.HAD MUD ON HIS FEET ONE FOOT SLIPPED AND LOST HISBALANCE,AND FELL OFF THE TRUCK ON A 1/2DRUM,HIT THE LEFT SIDE OF HIS RIBE AND CHEST.
