Employee was climbing down off a D11 bulldozer and slipped on the muddy track, when EE slipped EE pulled something in the shoulder. EE was released to full duty that day but after a CScan in June they found a slight tear in EE's shoulder.

Operator running a machine noticed that there was oil coming out of a Trac-Hoe running in the area and before being able to warn the operator the machine caught fire. The fire burnt the wire to the shut off so the pumps kept running and feeding the fire. Operators walked away and called for the fire department. Operators done as they were trained to do. No one hurt!
EE was shoveling around mesh belt guard of crusher. EE hit his right hand on the mesh guard, which caused a laceration to his hand. Approx. 10 stiches were needed to close wound, he was able to RTW immediately.
EE was moving backhoe down a ramp into the coal pit. The door on the machine was in the open position, the operator thought the door was latched in the open position. The door was not latched so while he was moving down the ramp he was holding onto the door jamb of the machine. The machine hit a bump causing the door to swing closed on his hand.
Employee was attempting to hook boom truck to shaker screen. He was holding hook to get aligned with screen (the hook is on a swivel) & when they picked up the screen, the swivel turned & pinched his right pinky finger against the shaker screen. His finger was lacerated on top & bottom. Stitches were needed to close the wound.
The auger was found in the early morning covered with rock. The auger had been left up against the highwall because there were two holes left to be augered.
Rain had occurred a couple of days prior to accident; also heavy rain was present at time of accident. While contractor was loading blasting holes next to highwall, a rock fell from the highwall, hitting the man in his left leg. Man was taken to hospital. He was diagnosed with a left lower shin bruise.
Employee was trying to see if the D-11 dozer he was operating had an antifreeze leak. he climbed down from the track of the dozer (the track was muddy) he slipped and fell on his right forearm.
EMPLOYEE WAS CLEANING A MOTOR ON A COAL CRUSHER WHEN A PIECE OF METAL (RUST) FLEW INTO HIS LEFT EYE.
EMPLOYEE WAS STANDING ON A ROCK WHILE HE WAS CONNECTING WATER HOSES TOGETHER (FOR A WATER PUMP). HE SLIPPED OFF OF THE ROCK AND TWISTED HIS RIGHT KNEE.
EE WAS PICKING UP A SUD SAVER DRILL BIT COUPLING 80 LBX APPROX. TO PLACE IT ON DRILL. AS HE WAS LIFTING SUB SAVER UP, WHEN HE GOT ABOUT WAIST HIGH APPROX. 3 FT OFF OF GROUND HE FELT SOMETHING POP IN HIS MID BACK.
EE WAS CARRYING A BAG OF BLASTING MATERIAL & HE STEPPED INTO A DRILL HOLE. STEPPING INTO THE DRILL HOLE CAUSED HIM TO STRAIN HIS GROIN.
EE WENT INTO STORAGE TRAILER TO GET WIRE CLIPS TO FIX BROKEN HEADLIGHT ON ROCK TRUCK THAT HE WAS DRIVING. WHEN HE REACHED INTO THE METAL BOX THAT WAS UP ON A SHELF TO GET THE CLIP, HE DROPPED THE BOX THAT ALSO CONTAINED A TEST LIGHT. THE BOX WAS HEAVY, BENDING HIS WRIST & THUMB BACK & THE TEST LIGHT PUNCTURED HIS RIGHT HAND BETWEEN THE THUMB & INDEX FINGER.
EE WAS CLIMBING DOWN FROM DRILLTECH D40K DRILL, AS HE WENT FROM ONE STEP TO THE NEXT HE SLIPPED, WHEN HE CAUGHT HIMSELF, HE HIT HIS LEFT ELBOW ON THE MACHINE CAUSING A CONTUSION OF THE ELBOW.
ROCK TRKS WERE BACK STACKING OLD RAMP. EE DECIDED TO PULL DOWN RAMP INSTEAD BCKING DOWN RAMP HE THOUGHT HE COULD STOP HALFWAY DOWN RAMP WHEN HE WENT OVER THE CREST OF THE RAMP, HE DECIDEDA HE WOULD NOT BE ABLE TO STOP THE LOADED TRUCK HE TREID TO STOP THE TRUCK BY PUTTING IT IN REVERSE THIS CAUSED THE TRUCK TO STALL WITH THE TRUCK STALLED THE DRIVER LOST CONTROL OF THE TRUCK AS IT
EE WAS WALKING TO HIS CAR, WHEN THE WORKDAY WAS COMPLETED. THE GROUND WAS VERY ICEY, WHILE WALKING TO HIS CAR, HE SLIPPED AND FELL ON THE ICE. DURING THE FALL HE TWISTED HIS RIGHT KNEE. THIS A CCIDENT WAS NOT REPORTED TO THE OFFICE UNTIL 1/20/99. HE DID NOT BE THE DR UNTIL 1-21-99.
THE EE WAS OPERATING A DRILL,M HE OPENED THE DOOR OF THE DRILL & REACHED OUT TO UNCOCK THE BOOM. THE WIND BLEW THE DOOR SHUT, THE EE WAS UNABLE TO MOVE HIS HAND OUT OF THE WAY OF THE DOOR, HIS LEFT INDEX FINGER WAS CAUGHT BETWEEN THE DOOR &THE DOOR JAMB.
EE WAS HELPING TO START UP A TEREX 3309 ROCK TRUCK. HE WAS SPRAYING STARTING FLUID INTO THE AIRCLEANER, WHEN THE ENGINE TURNED OVER, THEN EXPLODED. THE EXPLOSION CAUSED NUT FROM THE AIR CLEANE R TO PROJECT INTO THE CAN OF STARTING FLUID THATEE WAS HOLDING HIS HAND. THE STARTING FLUID CAN WAS PUNCTURED. WHEN THE NUT HIT IT, THE FLUID WENT ON TO HIM AND THE FLAME IGNITED IT. WHILE PU
A FELLOW EE STARTED UP A DOZER, WHICH HAD BEEN SITTING IDLE FOR A FEW MONTHS. RUST PARTICLES CAME FROM THE EXHAUST & ENTERED INTO HIS LEFT EYE.
EE WAS STRAIGHTENING BELLY PAN ON A ROCK TRUCK. HE WAS USING A HYDRAULIC JACK TO ASSIST HIM IN HIS WORK. THE JACK SLIPPED, SOMETHING HIT HIM IN THE FACE. HE IS NOT SURE WHAT IT WAS THAT STRUCK HIM IN THE FACE. HE TWISTED HIS KNEE TRYING TO MOVE OUT OF HARM'S WAY.
EE WAS DROPPING 22# BAGS OF BLASTING AGENT INTO HOLES. HE THEN BENT DOWN TO PICK UP A 5 POUND BOOSTER, AS HE DID THIS HE GOT AN EXTREME PAIN IN LOWER BACK. THE PAIN LASTED FOR ABOUT 5 MINUTES.
WHILE STEPPING DOWN ON PLATFORM TO CHANG EFILTERS ON D-11 EE LOST HIS BALANCE, HE FELL INTO DOZER HITTING HIS RIGHT SID EON A PANEL DOOR THAT HE HAD OPENED.
MAN WAS PUTTING PAD BOLTS IN THE TRACK OF D-9 DOZER HE STEPPED DOWN FROM THE DOZER TO SIDE ARM & HE SLIPPED ON MUD & FELL TO THE TRACK (APPROX 32FT) LANDING ON HIS RT ARM & RT HIP
MAN WAS GETTING DOWN FROM D-9 DOZER. H E STEPPED ON TRACK AND HIS FOOT SLIPPED ON THE TRACK (TRACK WAS MUD COVERED). HE FELL ABOUT 5' TO THE GROUND.
WHILE CHANGING EXTENSIONS ON DRILL TEHC, EMPLOYEE REACHE DUP OVER HIS HEAD AND PUSHED EXTENSION BACK INTO PLACE. WHILE PUSHING EXTENSION HE FELT A SHARP PAIN IN HIS RIGHT SHOULDER.
EMPLOYEE WAS CLIMBING DOWN FROM 2400 DRAGLINE HIS FOOT SLIPPED ON THE TRACK HE FELL TWO(2) FEET WHEN HE HIT GROUND HE ALSO HIT HIS BACK ON THE TRACKS THE EMPLOYEE ALSO HIT HIS RING FINGER OF T HE RT HAND IN THIS FALL
EMPLOYEE WAS DESCENDING LADDER OF LOADER. HIS PANTS LEG CAUGHT ON LOSE BOLT FROM LADDER, LOST BALANCE AND FELL TO GROUND.
MAN WAS PULLING ON A RATCHET TO BREAK A BOLT LOOSE ON HD-41 DOZER.THE RATCHET SLIPPED OFF THE BOLT CAUSING THE MAN TO TWIST WHEN HE TWISTED HE PULLED MUSCLES IN HIS LOWER BACK.
MAN WAS WELDING PLATE ON CRUSHER(THE PLATE HAD HINGES ON ONE END AND THE BOOM CRANE HELD OTHER END).MAN RAISED UP HITTING HIS HEAD (HARD HAT) ON PLATE WHICH CAUSED THE WELD TO BREAK.THE PLATE STRUCK THE MAN IN THE BACK AND PUSHED HIM (FACE FIRST INTO THE CRUSHER.THIS CAUSED HIS INJURY.
DURING THE COURSE OF THE DAY WHILE PICKING SULFUR BALLS FROM THE COAL CRUSHER, THE CONSTANT MOTION OF BENDING & TWISTING CAUSED THE EMPLOYEE TO STRAIN HIMSELF. HE THINKS THAT AT 11:00AM HE PIC KED A HEAVY SULFUR BALL WHICH CAUSED THE INJURY.
MAN WAS PICKING RICK OFF OF CRUSHED BELT HE FELT SOMETHING IN HIS NECK POP, CAUSING HIM PAIN. Y

MAN WAS WORKING ON HYDRAULIC TANK IN WELDING SHOP WHEN HE FELT SHARP PAIN IN LOWER BACK.
THE DRIVER WAS RUNNING A PAN, HE HIT A LARGE ROCK WHICH CAUSED SOMETHING IN HIS BACK TO GO OUT OF PLACE.
WHILE HE WAS BURNING WITH CARBON ARC ON BLADE OF 41 DOZER THE ARC SLIPPED OUT OF HIS HANDS. THE GUN FELL AND HIT HIS LEFT THIGH. BURNING HIS LEG.

THEY WERE CHANGING THE BUCKET ON THE 2400.WHILE PUSHING A CABLE THROUGH A DUMP SHIEVE THE CABLE KINKED. IT FLIPPED AND HIT HIM IN THE RIGHT KNEE.
HE WAS TAKING BOLTS OUT OF THE RWG BEAR IN THE HD41. HE WAS USING AN IMPACT WRENCH. THE NUT STRUCK IN THE IMPACT WRENCH, WHEN IT CAME DISLODGE AND HIT HIM IN THE RIGHT UPPER JAW.
I,NAME WAS GETTING DOWN OFF OF HIS HI-LIFT, HE WAS CLIMBING DOWN THE STEPS AS HE APPROACHED THE LAST STEP HIS RING FINGER CAUGHT ON THE ROUGH GRADING OF THE STEP AS HE JUMPED TO THE GROUND THE RING REMAINED LODGED-THIS CAUSED THE SKIN OF HIS LEFT HAND RING FINGER TO BE TORN OFF.

I NAME TOOK HIS LUNCH BREAK AT 12:00PM HIS MACHINE WAS PARKED WITH HIS CAB SIDE CLOSEST TO THE HIGH WALL THE HIGH WALL COLLAPSED CRUSHING HIM INSTANTLY
THE 2400 DRAGLINE SHUT DOWN FOR LUNCH, INJURED OILED THE BOOM, HE CAME OFF THE BOOM AND WAS READY TO CLIMB THE GANTRY, WHEN HE HEAR A RUMBLE, THEN THE HIGH WALL COLLAPSED. HE JUMPED ON THE CA TWALK AND WAS STRUCK BY A CABLE.
