FALL WILL BE TIMBERED OFF AND ESCAPEWAY REROUTED. THIS AREA WAS MINED SEPTEMBER 2004 AND BOLTED WITH 4 FT RESIN BOLTS. THIS AREA WAS LAST FIREBOSSED ON 9/22/05.
A fall occurred in the #7 entry at spad 200 measuring 19' wide x 20' long x 5' high. The fall will be timbered off and dangered off and will not be cleaned up. This fall is 1800' from working face.
EMPLOYEE WAS INSTALLING A STOP ON END OF LOW STRUCTURE FOR A BRIDGE SYSTEM. THE STOP TURNED OVER AND CAUGHT THE EMPLOYEE'S FINGER BETWEEN IT AND THE ROLLER.
Mine Foreman was making fireboss run. At apptox 6"22 am, discovered a roof fall in #3 entry. BRK 41, 1 bk. inby spad 214. Fall measures 20'lx18'wx4'thick. Type of roof bolts used in this area was #5x48". No, person or equipment involved. The wall will be timbered and dangered off.

(NOTE THERE IS 2 INTAKE AIRWAYS #6 IS USED PRIMARY ESCAPEWAY, REROUTED ESCAPEWAY FROM #6 TO #7 IN THIS AIRWAY. BOTH FALLS MEASURED 20'X18'X5'
EMPLOYEE GOT A PIECE OF METAL IN HIS EYE
EMPLOYEE WAS TRAMMING A SCOOP WHEN A HOSE GAVE WAY AND SQUIRTED HYDRAULIC OIL INTO EE'S EYE.
EMPLOYEE WAS WORKING ON SHUTTLE CAR WHEN A PIECE OF DRAW ROCK FELL & STRUCK THE EE'S LEFT KNEE
EMPLOYEE ALLEGES THAT HE PUT HIS LEFT HAND ON THE MINER & GOT SHOCKED. THE COMPANY QUESTIONS AND IS PROTESTING THIS INCIDENT.
EMPLOYEE PUT THE LAST BOLT UP IN ROW & WAS GETTING READY TO MOVE THE BOLTER WHEN A PIECE OF DRAW SLATE FELL BETWEEN BOLTS STRIKING EMPLOYEE.
#2 FACE EMPLOYEE WAS BOLTING, PUT OUTSIDE BOLT UP, SWUNG BOOM IN FOR INSIDE BOLT, ROCK FELL OUT AND STRUCK EMPLOYEE ON RIGHT THIGH.
Employee was walking behind roof bolter when roof bolter operator started reeling cable up on cable reel. The cable was in a mud hole and the employee didn't see the cable being taken up. The cable wrapped around left ankle, throwing employee up into roof of mine and employee landed in mud and water.
Employee was helping put guard on feeder. The other end of guard slipped out of co-worker's hand, shifting all the weight of guard on injured employee. The weight from the guard jammed right hand of injured employee against metal feeder frame.
A SCOOP WAS PULLING A PIECE OF BELT 240 FT LONG TO USE ON THE NEXT MOVE. EE WAS STANDING NEXT TO THE BELT WHEN HE SLIPPED AND GOT CAUGHT IN THE BELT CAUSING THE INJURY.
EE HAD JUST MADE S-CAR SPLICE. WHEN THE CABLE WAS TAKEN UP, A LOOP OF CABLE CAUGHT UNDER HIS LIGHT POUCH CAUSING HIM TO FALL WHICH CAUSED THE INJURY. (THIS REPORT MAY BE LATE BECAUSE I WASN'T AWARE THAT HE HAD TO MISS WORK MAKING IT REPORTABLE.)
EE WAS HAULING COAL FROM THE MINER TO THE FEEDERAND BACK. WHILE DOING SO HE RAN OVER SOMETHING CAUSING HIM TO BOUNCE UP AGAINST THE CANOPY OF THE S-CAR CAUSING THE INJURY
EE WAS HAULING SUPPLIES INSIDE WITH A SCOOP. THE BUCKET OF THE SCOOP CAME IN CONTACT WITH THE RIB JARRING HIM AROUND INSIDE THE CANOPY CAUSING THE INJURY.
EE WAS HELPING PULL A PIECE OF BELT BY USING PLIARS TO GRIP THE BELT. WHILE PULLING THE BELT HE FELT HIS SHOULDER POP. HE STOPPED WORK AT THAT TIME, HE TRANSPORTED HIMSELF TO THE HOSPITAL.
EMPLOYEE WAS USING A KNIFE TO REMOVE THE PLASTIC POUR SPOUT IN ORDER TO USE A ROSE GUN. THE KNIFE SLIPPED CAUSING THE INJURY.
EE WAS PULLING BELT TOGETHER WITH A PAIR OF CHANNEL LOCKS, FELT SHARP PAIN IN RT SIDE AND GROIN AREA.
IN BETWEEN THE DAYS OF 8-1-02 AND 8-4-02 A ROOF FALL OCCURRED IN INTAKE ESCAPEWAY 2 BREAKS OUTBY SPAD #400. THE FALL MEASURED 6'HIGH, 18'WIDE, &18'LONG. THIS FALL WAS IN AN INTERSECTION WHICH KNOCKED A STOPPING OUT BETWEEN INTAKE AND BELT MAKING IT INTERFEAR WITH MINE VENTILATION. THIS AREA WAS BOLTED WITH 4'RESIN BOLTS.
EMPLOYEE WAS LOADING ROOF BOLTING MATERIALS ON A SCOOP, LIFTING TWO BUNDLES OF ROOF BOLT PLATES (8" X 10", 10 PER BUNDLE), WHEN HE EXPERIENCED A SHARP PAIN IN CHEST AND BACK.
EE WAS ATTEMPTING TO JUMP OVER A SHUTTLE CAR CABLE #2-3 FLAT. MINE FLOOR WET AND SLIPPERY SURFACE FEET CAME OUT FROM UNDER HIM THEN HIT MINE FLOOR ON BACK. SELF RESCVER WAS UNDER HIP AND LOWER BACK WHEN HE LANDED.
EMPLOYEE WAS GETTING IN THE SCOOP DECK WHEN HIS UTILITY KNIFE CAME OPEN AND GOT UNDERNEATH HIM CAUSING LACERATION TO RIGHT HIP.
EE WAS OPERATING SHUTTLE CAR & JAMNMED NECK & LOWER BACK WHEN HE HIT HOLE IN ROADWAY. (BOUNCED UP AGAINST CANOPY)
INJURED WAS SHOVELING #3 TAILPIECE 002 SECTION. TWISTED TO PUT COAL ON BELT, FELT SHARP PAIN IN LOWER BACK.
EE WAS OPERATING S/CAR WORE BLISTERS ON LEFT FOOT AND LOWER BACK FROM PULLING BRAKE AND TRAM PEDALS BACK WITH FOOT AND FROM BOUNCING IN SEAT TO RIB LOWER BACK. NOTE: EE IS A SUGAR DIEBETIC SOR ES IS NOT HEELING. ACCIDENT IS QUESTIONABLE DUE TO NATURAL OF ILLNESS.
EE IS A DIABETIC. HE ALLEGES THAT BECAUSE HE WAS OPERATING SHUTTLE CAR IT GAVE HIM BLISTERS FILED FOR COMPLIANCE ONLY IFL IS PROTESTING CLAIM.
EE WAS PULLING UP SLACK ON MINER CABLE. HANDS SLIPPED OFF OF CABLE, FELL TO GROUND HIT RIB WITH LT. SHOULDER FELT SHARP PAIN IN LOWER BACK AND LT. SHOULDER.
EE WAS CUTTING BELT TO MAKE BELT SPLICE WHEN KNIFE SLIPPED CUTTING PALM OF LT. HAND.
EE HAD BATTERY LID RAISED ON SCOOP WHILE WATERING BATTERIES, WHEN THE LID FELL AND MASHED MIDDLE FINGER ON HIS RIGHT HAND.
ROOF FALL OCCURRED IN THE FACE OF THE #2 ENTRY #2 SECTION. FALL MEASURED APPROX 4 FT WIDE 2' RIDER THICK BY 20 FT WIDE BY 30' LONG. THIS AREA WAS MINED 11-26-01. BOLTED WITH 4' RESIN BOLTS. NO EMPLOYEES OR EQUIPMENT WAS INVOLVED. NO AIRWAY OR TRAVELWAY WAS EFFECTED. THIS AREA IS CRIBBED AND DANGERED OFF.
A ROOF FALL OCCURRED IN THE INTERSECTION IN THE #3 ENTRY, 600' OUTBY THE NO. 2 SECTION AT SPAD #391. FALL WAS ~5'THICK, 19' WIDE & 19' LONG. NO PERSONS OR EQUIPMENT WERE INVOLVED NOR WERE TRAV ELWAYS OR AIRWAYS AFFECTED. THE AREA WAS CRIBBEDOFF.
A ROOF FALL OCCURRED AT THE NO. 1 SECTION IN THE#4 BELT ENTRY. THE FALL WAS ~30' LONG X 15' WIDEX 5' HIGH. IT FELL ON THE SECTION FEEDER AND NOEMPLOYEES WERE INJURED OR INVOLVED. THIS PANEL W AS FINISHED; WE CRIBBED OFF & PULLED BACK 1000'.THE FALL WAS 5 BREAKS FROM THE FACE.
INJURED WAS RIDING RUBBER TIRED MANTRIP FROM END OF TRACK TO 001 SECTION. PIECE OF DRAW ROCK MEASURING 3'X4'X4" FELL FROM IN BETWEEN BOLTS STRIKING VICTER IN BACK OF NECK AND BACK. WILL BE OFF APPROX. 4 WORKING DAYS. T
INJURED WAS RIDING RUBBER TIRED MANTRIP FROM END OF TRACK TO 001 SECTION, PIECE OF DRAW ROCK MEASURING 3' BY 4' BY 4" FELL FROM IN BETWEEN BOLTS STRIKING VICTIM IN BACK OF NECK AND BACK. WILL BE OFF FROM WORK IN APPROX 4 DAYS.
INJURED WAS CHANGING A FOAM FILLED TIRE ON A S/ CAR. HAD SLATE BAR UP UNDER TIRE TO LIFT TIRE SLATE BAR SLIPPED OFF OF TIRE THROWING WEIGHT OFTIRE ON EMPLOYEE.
INJURED WAS HOOKING 3/8 CHAIN TO BELT TAKE-UP AND CONVEYOR BELT TO TIE BELT OFF TO MAKE A MECHANICAL BELT SPLICE, CHAIN CAME LOOSE FROM BELT STRUCK INJURED IN MOUTH.
INJURED WAS HOLDING A REEL COVER SHIELD UP WITH A SCREWDRIVER TO GET BOLT HOLES LINED UP FROM WEIGHT OF SHIELD, SCREWDRIVER SLIPPED OUT OF HOLE ON SHIELD LETTING ARMS GO IN UPWARD POSITION CAU SING INJURED TO PUNCH SCREWDRIVER IN THE LEFT CORNER OF RIGHT EYE.
EMPLOYEE WAS ATTEMPTING TO WATER THE BATTERIES ON A SCOOP WHEN LIFTING BATTERY LIDS EXPERIENCED SHARP PAIN IN LOWER BACK.
HELPING CHANGE PAN JACK ON MINER, WENT TO LIFT OLD JACK AND LET IT DROP DOWN, DROPPED DOWN AND CAUGHT LT INDEX AND MIDDLE FINGER.
EE WAS COMING FROM MINER TO FEEDER. COMING INTO XCUT RAN INTO RIB CAUSING EE TO SLIDE FORWARD IN SEAT COMING IN CONTACT WITH DECK AND RT. KNEE.
EMPLOYEE AND SUPERVISOR WAS MOVING 14 CM MINER CABLE WAS PULLED FROM HANGER PINNING INJURED BETWEEN CABLE AND ROOF BOLTING MACHINE.
INJURED WAS HANGING 2NF MINER CABLE, CO-WORKER DROPPED HIS END OF CABLE CAUSING ALL THE WEIGHT FROM CABLE TO BE SUPPORTED BY INJURED.
