Two welders were welding on the auger, the injured employee was helping the two welders hold the metal that they were welding. The flash from the welding burned the injured employee's eyes, resulting in his eye sight being limited.

Employee turned the A/C unit on and an unidentified object flew into the employees eye.

Employee was standing on the bumper of the truck checking his oil, as he started to descend down off the bumper of the truck he lost his balance and as he was reaching out to hold his self the employees index finger was lacerated on an unidentified object.

Employee was exiting the excavator, as he stepped down on the track he lost his balance due to the track being wet and twisted his left knee.

Employee was prying a bolt out of an A/C Compressor on a D10N employee back went out causing him to fall to the ground.

Employee placed his right hand in a pinch point area of the roller and boggie pins while trying to install a chain before performing work on the roller.

Employee was prying on a block of coal in the hopper, employees hand slipped off of the pry bar hitting the corner of the hopper resulting in lacerating his right finger, where he received four stitches.

Employee was changing the chip deflector and replacing a new bit. Employee was using a pipe to unstop the drill bit and the bit blew out rock fragments into the employee arms and chest.

Service Tech. was atempting to climb up on a dozer, as he grabbed the hand rail his hand slipped off the rail, resulting in the employee slipping backward and falling on the ground landing on his back.

Stepped out of truck and took a few steps and employees feet flew out from under him, resulting in hitting his head and knocking him out for a brief period.

Employee was walking up the haul road and slipped on the icy road resulting in a right arm fracture.

Employee was cutting on the bed of a 777C Rock Truck and the springy part of the metal broke and threw the employee approx. 2 feet in the air and as employee came down the metal landed on his left fore arm causing a small laceration on employee's forearm needing 8 stitches.

Employee was getting loaded and a rock fell from the highwall hitting the truck on the rear corner of the bed, causing the operator to have pain to his shoulder

Employee was decending down the ladder of his loader. When he approached the bottom step of the ladder his feet slipped off the step, resulting in him losing his balance and landing on his back.

Employee was decending down the side of the D11R Dozer, as he stepped down on the pusharm his right leg slipped causing him to fall to the ground, resulting in pain to his right knee.

Employee was ascending up the ladder of his loader, when he slipped due to wet conditions from the coal pit, causing employee to hurt his right arm.

Employee was grinding on a piece of metal. While doing so a small piece of metal shaving flew through the side of employee's safety glasses, resulting in the metal shaving embedding in his right eye.

Employee was lifting metal out of the bed of his pick - up truck, as he was shifting his weight to throw the metal from the truck he felt a sharp pain run up his left side.

Employee was mounting up the ladder of a 992C loader to service the transmission fluid. As he approched the top step the oil hose reel unlocked, causing the hose to jerk the employee, resulting in him loosing his balance and falling off the loader.

Employee was prying on bar it slipped hitting the employee in the left eye lid resulting in him getting four (4) sticthes in his left eye lid.

Employee was climbing down ladder of 988F loader and as he approached the ground he twisted his foot and slipped on a block of coal causing him to fall to the ground landing on a block of coal resulting in a laceration of 1-1/2 " to his left thumb.

Employee was unloading a water pump from the bed of the F350 truck. The truck bed was slick due to rainy weather, therefore as he was descending the truck bed with one foot out and one foot in, he slipped on the wet surface falling to the ground. Twisted his right ankle, resulting in it swelling.

Employee was welding on a 777C Rock Truck Bed, and a piece of Slag flew up and struck the employee below the left eye, causing a mild burn below his eye.

Employee was backing up the dump with a 777C Rock Truck and stated that he either blacked out or fell asleep and over traveled the dump resulting in the truck over turning.

Employee was climbing down the ladder of the 777C rock truck and as he approached the last step, it swung under the bottom of the truck resulting in the employee falling to the ground landing on his right side.

Employee was tighting roller bolt on D11R dozer, Employee was lying on his back while tighting bolt and socket spun off head of bolt causing metal shavings to fly in employee eye, Employee did have safety glasses on

D9R dozer operator was traveling in reverse and struck a 777C rock truck in the rear, the rock truck was also traveling in reverse. The D9R operator states he didn't see the rock truck as he began to go in reverse, the rock truck driver also states he did not see the D9R dozer through his mirrors as he was traveling in reverse.

Employee was taking out top pin of 992C Loader boom, (Boom Pin) while pushing up on the pin employee felt sharp pain in his lower left side below his rib cage, causing him to pull a muscle.

Employee was walking from auger and while doing so he twisted his right ankle on unlevel pit floor.

While climbing up ladder employees left foot slipped off first step of ladder

Employee was lifting 45lbs wet hole bag from truck and was placing it into the drill hole (blast hole) Doing so Employees back began to hurt.

Slipped while mounting truck. Employee grabbed railing and pulled.

Employee was climbing off of the bed of the hydro seeder truck; he slipped causing him to fall to the ground landing on his right elbow.

Employee was climbing down the ladder and slipped on the bottom step. Caught himself on the handrail resulting in hurting his back.

Employee was climbing down the ladder of the truck he had been driving and slipped catching his foot/ankle in the step causing him to fall to the ground.

EE was pulling a 6" pump hose.

EE was stepping up on the boarding ladder. As he began climbing up on the ladder, he felt a very sharp pain shoot up his lower back. The pain progressed when he started working, causing him to leave early.

Employee was hitting the side of powered bin with a sledge hammer. The hammer came back and struck the employee in the right rib cage causing him to black out hitting his head on the beam of the powered bin.

Employee was tossing bails of multch on seeder truck and felt a sharp pain shoot up his right side neck.

EE was pushing material down to 992D loader with a D9R dozer co. # M37 & the machine's blade slipped off a rock causing a sudden jerk, which resulted in EE having sudden pain in lower abdomen.

Outburst of fuel hose, blowing fuel in his eyes.

Employee was tamping dirt in drill hole of a rock approx. 4 ft in height, as He was tamping he twisted his back, causing him to pull a muscle in his back.

Employee stepped on loose material at the edge of a hole. The material moved causing employee to slip into hole.

Employee was standing on stabilizer bar, slipped and fell due to it being oil-covered.

Employee stepped in loose material on drill bench twisting his ankle.

Employee was moving from one dozer to the other when he slipped due to muddy and wet conditions.

Employee was assisting in removing the 4" discharge hose which was wrapped around pump during transport. As employee pulled on hose, which had become tangled, the coupling on end of hose came loose and struck employee.

EE was climbing down ladder; highwall fell and a rock flew from highwall hitting him in the left hand.

Employee steps down on push arm on 395 Dozer on a dirt clot & dirt clot rolled causing the EE to fall from push arm to the ground.
