TWO WELDERS WER9 ATTEMTPING TO DRIVE TOOTH FROM DRAGLINE BUCKET USING A BAKCINGOFF PUNCH AND SLEDGE HAMMER INJURIED EMPLOYEE WAS HOLDING THE 16 INCH HANDLE OF PUNCH WHILE FELLOW EMPLOYEE STRUC IT. WHEN IT WA8 STRUCK SMALL FRAGMENT OF METAL SEPARATED FROM THE NOSE OF THE PINCH AND IMBEDDED IN FINGER. FRAGMENT WAS REMOVED AT HOSPITAL AND 6 STITCHES WERE REQUIRED PUNCH WAS IN GOOD CON
EMPLOYEE HAD FI3ISHED WELDING A CLEVIS AND WITH HELP OF ANOTHER WORKER MOVED IT OUT OF THE REPAIR SHOP BY SUSPENDING IT FROM A LONG STEEL BAR. EMP WAS SEARCHING FOR 5 GALLON OPEN PLASTIC BUCKET TO AID IN PUMPING OPERATION IN PIN. HE COULDNT LOCATE ONE AND THEREFORE DECIDED TO MAKE A SUITABLE CONTAINER FINDING A 55 GALLON DRUM HE FIRS T CHECKED FOR RESIDUAL FLUIDS. FINDING NONE HE CHECKED FOR POSSIBLE ODORS COMING FROM THE BAREL. SATISFIED THAT THE BARREL WAS CLEAN HE LAY THE BARREL ON ITS SIDE AND STOOD AT THE BOTTOM END.
WIRE ROPE SPOOL WAS IN FRONT-END LOADER BUCKET WHEN VICTIM CUT RESTRAINING WIRE FROM ROPE END. AS ROPE UNWOUND, SPOOL TURNED SLIGHTLY AND CONTINUED TO UNWIND.
