EE was attempting to investigate a problem with a shuttle car. EE slipped and started to fall. Trying to stop EE's fall, EE reached out EE's hand-cutting two fingers.
RIDING MANTRIP WHEN MANTRIP HIT THE RIB ON THE RIGHT SIDE AND COAL AND ROCK FELL ON EMPLOYEE, WHO WAS RIDING MANTRIP.
A NON-INJURY ROOF FALL ACCIDENT OCCURRED AT APPROXIMATELY 7:30 AM ON 04/05/2021. THE ROOF FALL OCCURRED IN THE #3 PILLAR BLOCK OF THE 001 MMU SS #14957.
A PIECE OF DRAW ROCK FELL AND HIT EE ON LEFT EYEBROW.
A roof fall was found on 8/19/2020 at 7:15 AM in the No. 1 Entry inby Survey Station #2250. The approximate size of the fall was 4-5 feet high, 20 feet wide and 70 feet long.
A roof fall was found during weekly examinations on 11/8/18 at 7:30AM at three breaks inby survey stations #1603. The approximate size of the fall was 6-8 feet high 19 feet wide and 60 feet long.
EE was signaled to tram s/c forward by operator twice & didn't respond. Operator approached ee & observed ee slumped in seat. Not able to arouse ee operator called for boss/met boss checked for a pulse & none found removed from car & started CPR transported to surface while performing CPR. Continued until ambulance personnel took over corner pronounced dead at mine.

A PIECE OF WATER LINE FELL TWO FEET AND STRUCK EMPLOYEE ON THE SHOULDER CAUSING BRUISING.
An accident on the date of 2/9/12 resulted in a continuous miner operator/helper being crushed between the miner's tail boom and the coal rib. This inflicted a fractured let femur to the victim.
While using a hammer, a piece of metal hit him in his right eye.
Alleged black lung claim.
EE stated that he put up three rows of bolts, shortly after he felt a numbness in his hand. He does not recall anything specific to have caused this alleged injury.

A roof fall was found at no. 2 heading at spad no. 463 measuring 90' long, 18 - 20 feet wide and 12' in height.
A surface slide 14-15 feet high and 12-14 feet wide was found on 06/05/06 at 4:45 A.M.
A piece of coal fell from the top and hit the continuous miner and bounced off, striking employee.

The shuttle car was tramming away from miner up an incline of app. 6 degrees. The shuttle car came off point while tramming and drifted backward hitting the rib and miner before stopping. As the shuttle car hit the miner and rib, the employee was also hit.
A roof fall 20 feet long, 5 feet in height, and 5 feet wide was found on 08-01-05 at 5:30 a.m. at No. 1 entry at spad No. 315.
A roof fall 170 ft long, 12 ft thick, and 50 ft wide was found on 7-25-05 at 5:30 a.m. at break no. 19 on no.3 main, 5 breaks out by face area.
EE was found lying on his back. We assume that he slipped and fell on the ice while he was cleaning up at the mine site.

A ROOF FALL 150 FEET LONG, 6 FEET IN HEIGHT, 20 FEET WIDE WAS FOUND ON 04-28-04 AT 10:00 A.M. FROM #4 THRU #6 HEADING ONE BREAK INBY SPAD #445.
IN THE PROCESS OF LOADING A BELT MOTOR TO BRING OUTSIDE, WHEN EE'S ARM WAS CAUGHT BETWEEN THE MANTRIP AND BELT MOTOR.
A ROOF FALL APP 40 FEET LONG, APP 6 FEET IN HEIGHT AND APP 20 FEET WIDE OCCURRED ON 8/4/03 AT SURVEY SPAD NO. 28, APP 5TH INTERSECTION UNDERGROUND FROM PORTAL AND APP 1700 FT OUTBY FACE.
A ROOF FALL 110 FEET LONG, 6 FEET IN HEIGHT AND 20 FEET WIDE OCCURRED ON 7/31/2003 AT SURVEY SPAD NO. 47 TO WITHIN 40 FEET OF SURVEY SPAD NO. 58, TWO BREAKS TO THE LEFT OF THE BELT HEADING AN D APP 1500 FT OUTBY THE FACE.
