He was using a cutting torch to cut scrap metal when oil ignited causing a flash fire.

EE was carrying 5 gallon cans of oil when he twisted his back.

Employee was pulling cable to move continuous miner, afterwords his wrists were experiencing pain. He went to doctor and has been treated by Integrated Medical Group, they determined he has carpal tunnel syndrome and surgery is required, his surgery was scheduled for Fri 1/4/13, therefore: he is unable to work.

Employee was lifting block to build walls when he twisted his back.

Employee was lifting jackhammer out of a low spot to change leg when he slipped on a rock & injured back.

Employee slipped in mud & twisted his right knee. He had pain in his knee but continued working. Doctor determined he needed surgery, had surgery on 3/21/12, he has been off of work since.

Was walking belt line checking roller when he tripped over large flat rock laying behind the scraper board striking left hip

Employee was drilling a hole with jackleg drill when the steel became stuck. Employee pulled drill down, steel fell out of hole striking the back of his right hand.

EE was pulling miner cable when he felt a burning pain in his back. Had pain all weekend, sought medical attention on Monday 7/4/11.

EE operated shuttle car entire shift near the end of shift he was experiencing pain in neck and shoulder sought medical attention next day.

Employee was operating shuttle car when he hit a bump causing him to strike his head on shuttle car canopy.

Foot was stuck in mud, attempted to pull foot out of mud when he pulled muscle in left leg. Kept working on it for 9 shifts. When he still had pain he sought medical attention.

Employee was fixing back board at belt drive, when he slipped, striking his left side on corner of belt drive.

EE was in an area where timber was being installed when the timber was struck with an axe it caused a piece of coal to lodge in EE's right eye.

Employee was drilling a hole in the bottom to pin down a bolt tail. He attempted to pull jackleg drill out of hole when he experienced pain in his lower left back.

Employee was helping carry a steel beam when he pulled his groin muscle.

Employee was helping to carry a 6" steel airline on rough terrain, when he twisted his left knee.

Employee was in an area where a hammer was being used to hit a piece of steel. A sliver of steel flew off an imbedded itself in employee's right hand.

Employee was hand loading timber from timber pile to scoop bucket. While loading the timber he experienced pain in his lower back.

Employee was operating continuous miner all day. Near the end of shift he experienced pain in shoulder, limited movement in shoulder.

Employee was pulling cable at miner, when he twisted his right knee.

Bolters were moving hoses and hose caught employee in left ear. Employee finished shift and continued working for several days. He experienced swelling and pain in left ear, went to doctor several times but swelling and pain would not go away. Last day worked was 11/4/09.

Employee experienced pain in left hip, thigh & knee area on 9/28/09. He sought treatment on 9/29/09. He worked with the pain until 10/6/09. He sought treatment at emergency room on 10/10/09 and has been off work since 10/6/09 on doctor's advisement. Employee feels condition is a result of previous accident dated on 4/12/2004.

Employee was carrying 2 cans of hydraulic oil, when he felt a sharp pain in his groin area. The pan subsides but employee defected lump on groin the night of 9-14-09. He sought medical attention the next day 9/15/09

Employee was walking in entry when he slipped in low spot causing him to twist his back.

Employee was pulling cable when he slipped on wet coal causing him to fall down and strike left knee on bottom rock.

EE was pulling buggy cable away from feeder, when it threw him off balance and knocked him down. His left knee, right knee and lower back were injured.

EE was riding in travelway on mantrip when the spare tire, which was on top front of mantrip, rolled out and slid. This trapped EE's left foot between the tire and the roof.

Employee was standing on the back of a mantrip, closing the outside shop doors. He slipped and fell injuring his left knee.

Employee left work at 7:00 am on 7/21/08 complaining of a sore back. On 8/8/08 employee claimed injury was caused by under inflated air cushion seat bottoming out when travelling over uneven terrain.

Employee strained his lower back while changing a flat tire on his mantrip.

Employee was walking in a low area of the mine and hit his head against the roof.

Employee was on his knees near the feeder. As he stood, a piece of copper telephone wire which was hanging from a roof bolt plate, entered his left ear.

EE was operating the continuous miner. As he was backing up, he noticed line brattice behind him. He reached through the opening of the canopy, directly behind the operators seat, to push the brattice to the side. While doing this, his left hand was above the canopy and his left index finger was squeezed between the roof and the canopy.

EE was operating a Joy 21 SC Shuttle Car behind a continuous miner. A lump of coal approx 2" X 2" X 2" was thrown from the boom of the miner and struck him on the left eye.

Employee was opening an air valve and the line blew apart. He fell back and landed on his left shoulder. Employee has been on light duty since 2/12/08. He is now not working.

The roof bolters were installing bolts in a low area of the mining section. The EE was on his knees beginning to install a bolt, when the jack hammer and leg struck him on the left side of his forehead.

Employee was crossing over the section conveyor belt. He stepped off the belt and sprained his right ankle.

Employee had his right hand grasping the top of the canopy of his shuttle car, while tramming toward the south miner his hand became squeezed between the canopy and the roof.

Employee was cutting stainless steel and incurred flash burn to eyes from errant plasma cutter arc.

EE was backing a lee norse 265 cont miner out of a cut the miner climbed onto a large stone. When the machine came off the stone, the operators compartment hit the bottom throwing his left shoulder against the roof canopy.

EMPLOYEE JUMPED OFF FEED CONVEYOR, LANDED IN SILT, LOST BALANCE AND TWISTED HIS BACK

Employee was tramming his shuttle car toward the continuous miner. The shuttle car slid against the coal rib, which caused the employee's left knee to strike the rib and rebound up into his chest.

EE WAS WALKING DOWN AN INCLINED ENTRY, SLIPPED AND SPRAINED LEFT FOOT.

EE SLIPPED AND FELL ON CONCRETE FLOOR OF COAL PREP PLANT DURING A SCUFFLE WITH EE SUPERVISOR

EE WAS CHANGING BITS ON THE CONT MINER, SLIPPED AND FELL AGAINST THE SIDE OF THE GATHERING PAN ON THE MINER.

EE WAS OPERATIING A SHUTTLE CAR AND HAD JUST RECEIVED A LOAD OF COAL FROM CONTINUOUS MINER. HE TRAMMED FROM FACE & MADE THE TURN TO GO WEST TO THE FEEDER. OPERATOR OF 2ND SHUTTLE CAR WAITED FOR EE TO CLEAR THE INTERSECTION. 1ST CAR HAD TO BACK UP A SHORT DISTANCE TO CLEAR THE LEFT RIB. OPERATOR OF 2ND CAR THOUGHT HE HAD CLEARED THE INTERSECTION & PULLED FORWARD STRIKING 1ST CAR.

Employee had been standing on the #5 main belt tailpiece. when he stepped off, he slipped and fell injuring his left hip/thigh area.

EE SLIPPED & FELL BACKWARD, FALLING ON HIS RIGHT LEG.
