Highwall failure measuring 40'(H) x 5'(T) x 30'(W).

Injured was using a sledge hammer to help take a cutter chain apart. As he struck the pin, the pin went sideways, thus allowing his hand to strike chain.

Employee injured his heel(R) when he jumped from the control room of the highwall miner to the ground.

A methane ignition occurred.

Employee was attempting to hit a punch with a 3# sledge hammer, in order to repair the handle on another sledge hammer by driving out the wedge. Employee missed the punch & struck his left hand.

Emplyee was replacing shield over cutter motor when the shield slipped and smashed his middle(R) finger.

Employee was drilling an anchor hole with hand-held auger. The auger was bent and caused the employee's hand to be caught between drill and anchor pin, injuring his left middle finger.

Employee was putting pedestal into a pickup bed. While placing the pedestal on the tailgate, he knocked another pedestal onto his left foot.

Employee decided to start equipment in an area of the job where no one else was present. He came in on a road on the backsided of the mine. After trying to start a CAT 777 he fell while descending the truck hitting the ground.

While walking away from the highwall miner and putting up tools, He stepped into a hole and heard something pop in his foot(R).

While tightening the cutter chain on the highwall miner, EE was struck by a piece of metal that broke off a hammer that was being used.

Employee was driving a pin on the chain conveyor when a piece of metal broke off of the hammer that he was using and struck him in his neck- thus requiring sutures.

EE was driving a pin on the chain conveyor when a piece of metal flew off and struck employee in the left hand, between the 3rd and 4th finger. Employee did not miss work or recieve medical treatment until 8-7-2008, when the metal was removed.

While guiding a pin into the hole with his knee, the employee felt his knee pop and pain shot down his leg.

Employee's shirt got caught on a bolt on the kelly bar of the auger- thus injuring his back.

Employee alleges that he strained his back while working as a groundsman. He is unsure as to when this injury occurred, and did not report this alleged injury until 5/22/2007. His first scheduled day missed was 5/29/2007.

Employee's foot got caught unger the auger while putting a block under the Kelly Bar.

EE was struck in the jaw by an auger flight while hooking a wrench to a bit.

A piece of frayed wire from a hoist cable became embedded in EE's hand. He didn't start missing work until 1/9/2007.

Employee was striking latch pins when a piece of metal came off of pins & struck his leg (R). THIS WAS NOT A LOST TIME ACCIDENT!!!

Employee was injured while attempting to lift an auger flight with a hoist. The flight hung up and when the employee freed it, one end of the flight fell onto the employees left hand.

While working on the highwall miner, employee was struck in the back by a rock that fell from the highwall. He then struck his his knee on the cutter head.

Employee injured his finger when a miner head that he was working on slipped & caught his finger between the plate & miner head.

Ee was striking tow ring with hammer when a piece of metal flew off and struck ee on left calf. Ee did not miss work or receive medical treatment until 6-28-04.

EE DISMOUNTED TRUCK & STEPPED IN A HOLE. EE TWISTED HIS BACK.

THE CREW WAS IN THE PROCESS OF BRINGING THE FLIGHTS OUT OF THE AUGER HOLE. EE PULLED A PIN & ALLEGES TO HAVE TORN A MUSCLE IN HIS LEFT FOREARM.

EMPLOYEE WAS STACKING AN AUGER FLIGHT IN RACK WHEN HE LOWERED THE FLIGHT ONTO HIS LEFT THUMB.

EMPLOYEE WAS REMOVING A PIECE OF SLATE FROM THE CONVEYOR. THE CONVEYOR WAS DE-ENERGIZED, BUT WHEN EMPLOYEE GRABBED THE SLATE, SOME COAL ROLLED DOWN THE CONVEYOR, STIKING HIS HAND.

EMPLOYEE WAS MOVING AUGER FLIGHTS OFF OF SLED, WHEN SOME FLIGHTS BROKE LOOSE AND STRUCK HIM, INJURING HIS LEFT KNEE AND LOW BACK.

WHILE MOVING IN EQUIPMENT, EE'S RIGHT LEG GOT PINNED AGAINST CONVEYOR.

EE WAS PULLING CONVEYOR IN WHEN IT GOT CAUGHT IN A RUT, JERKING OUT & ROLLING OVER EE'S LEFT FOOT & ANKLE

EE WAS LIFTING AUGER FLIGHT WHEN THE KELLY BAR PINCHED L THUMB AT 1ST JOINT.

EE WAS STANDING BESIDE AUGER CONVEYOR OBSERVING THE LEVELING OF THE AUGER MACHINE. SMALL ROCKS STARTED FALLING FROM HIGHWALL. EE TURNED TO RUN, BUT WAS TRAPPED BY AUGER MACHINE. ROCKS STRUCK E E ON LEFT LEG AND RIGHT ANKLE.

EMPLOYEE WAS LEVELING THE HEAD ON THE AUGER MACHINE. HE TURNED TO SPEAK TO THE AUGER OPERATOR AND A ROCK FELL FROM THE WALL AND STRUCK HIM IN THE BACK. HE THEN FELL UNDER THE AUGER MACHINE A ND WAS STRUCK ON LEFT LEG BY ANOTHER ROCK.

HE WAS HELPING TO MOVE AUGER FLIGHTS SO THAT THE FLIGHTS COULD BE PULLED UP BY AN ENDLOADER. HE ALLEGES THAT HE LOST HIS FOOTING AND FELL DOWN. HIS HAND WAS THEN CAUGHT BETWEEN THE FLIGHTS. HE DID NOT START MISSING WORK UNTIL 5-19-97.

THE CREW WAS PULLING THE CONVEYOR WITH A WINCH WHEN THE CONVEYOR CAUGHT ON A ROCK AND KICKED THUS CATCHING EE'S LEG BETWEEN THE SKID AND CONVEYOR.

A SMALL PIECE OF ROCK FELL FROM HIGHWALL & STRUCK EE'S KNEE WHILE HE WAS PULLING STEEL.

ROCK CAME OFF THE HIGHWALL AND STRUCK EMPLOYEE.

EE FELL OFF OF COAL CONVEYOR WHILE WORKING ON IT.

WHILE BACKING UP TRUCK, EE STRAINED HIS NECK.

AUGER FELL OFF RACK AND STRUCK HIM IN HIS FOOT. THIS ACCIDENT WAS NOT REPORTED TO MINE MANAGEMENT UNTIL 4-15-95.

ROCK CAME OFF HIGHWALL AND STRUCK EMPLOYEE'S FOOT.

PULLING PIN - PULLED TOO HARD RESULTING IN LUMBAR STRAIN.

EE WAS BINDING DOWN AUGERS ON A SLED WHEN CHAIN SLIPPED & STRUCK EE ON HIS CHIN

STEEL SLIVERS BECAME LODGED IN EE KNOCKERS

EE CLIMBED ONTO DUMP T RUCK TO KICKOUT DIRT HE SLIPPED & FELL TO THE GROUND

AUGER ROLLED ON HIS FINGER.

PUTTING PIN IN KELLY BAR GOT HAND PINCHED.

EE GOT HIS THUMB CAUGHT IN A PINCH POINT BETWEEN THE KELLY BAR AND AUGER.
