THE FALL WAS IN THE #6 NORTH ENTRY (SUPPLY ROAD) AT +5321 TO +5241 IN PILLAR LINE. WAS DEVELOPED APPROX 8/2/04. THE ROOF WAS PINNED WITH 5' TENSION REBAR BOLTS. THE FALL WAS APPROX 50' LONG, 18' WIDE, 5' HIGH.
Employee said he had put Ram battery on charge & when he jerked on battery lid his back started hurting, he worked until 11/30/05, went to Dr.
HAD FALL IN INTERSECTION, BELT ENTRY, 2 X-CUTS INBY FEEDER ON #2 UNIT OF 650'WEST PANEL. FALL MEASURED APPROX. 49' X 42' X 6'HIGH. PULLED 4' DOUBLE LOCK ROOF BOLTS. FALL IS BEING CLEANED UP.
The injured said he was getting out of cab on 488 scoop when he put his foot on the ground he heard a pop in his right knee.
THE INJURED WAS BUILDING A BRATTICE ON #1 UNIT. HE DID NOT HAVE THE CURTAIN DOWN AND NO FLAG WAS HANGING TO CURTAIN CAR DRIVER. A RAM CAR RAN THRU CURTAIN, HIT THE TRAILER OF BLOCK, HITTING THE INJURED ON LEFT LEG. THE CAR DRIVER DID SOUND HORN AND FLASH HIS LIGHT BEFORE GOING THRU CURTAIN.
The injured was drilling hole in roof to install roof bolt. While drilling hole, a burr on the steel caught his glove and fractured his left ring finger.
The injured said he was picking up a 5' long 6X6' timber when he felt a sharp pain in his right groin area.
THE INJURED SAID HE WAS HELPING MOVE BELT UP. HE WAS LIFTING BOTTOM BELT ROLLERS UP AND HANGING IT ON ROPE, WHEN HIS BACK STARTED HURTING.
The fall was int he #2 (N. intake) entry, which blocked escapeway from the #1 unit no work will be done on #1 unit until escapeway is sate to travel. The fall was 70' long, 5' high, 18' wide. The fall will be cleaned up before work is rejumed on the affected #1 unit.

THE #6 NEUTRAL ENTRY WAS CLEANED & USED FOR A NEW SUPPLY ROAD. THE FALL WILL BE CLEANED UP. THE APPROACH TO THE FALL HAS BEEN REINFORCED, CRIBBED & TIMBERED.
The injured is a roof bolter, he said he had a bolt stuck in the hole in roof when he pulled the bolt out of roof he mashed his litte finger between the bolt head & the wrench on bolter.
Had roof fall on the 2nd M. North belt line, at +4681, intersection #4 entry. Belt line was timbered up to this area, approaches are timbered. Fall will be cleaned up. Belt, structure and water line, under fall. The fall is appx 70' long, 4 to 6' high, 18' wide, had been supported with 5' tension roof bolts.
THE 5 HP MOTOR ON PRESSURE TANK WENT TO GROUND. HAD TO CHANGE MOTOR. THIS HELPS OPERATE BRAKES. THE MEN UNDERGROUND WERE NOTIFIED & PRODUCTION WAS STOPPED. THE EMERGENCY HOIST IN SHAFT WAS SENT TO BOTTOM. THE HOIST WAS RUNNING AGAIN AT 11:00 AM. IT WAS DOWN 1 HR 45 MIN.
THE INJURED SAID HE WAS WALKING OVER TO GET THE SCOOP WHEN HE RAN INTO A ROOF BOLT, FELL DOWN TO HIS KNEES, AND HEARD SOMETHING POP IN RIGHT KNEE. HIS KNEE WAS NOT HURTING THAT BAD. HE FINISHED WORKING THE REST OF SHIFT.
THE INJURED WAS HOOKING UP BATTERY TRACTOR TO THE WATER WAGON & BRUISED HIS RIGHT HAND WHILE PUTTING THE PIN IN TONGUE.
The injured said he had drilled a hole in the mine roof, to install a roof bolt when the top section of drill steel hung in the roof - The drill steel then fell out of the roof, striking him on the left hand.
THE INJURED TOOK RAM CAR TO BATTERY BARN TO CHANGE BATTERIES. WHEN HE WAS GETTING OFF OF THE CAB OF RAM CAR, HE SLIPPED, FELL & HURT HIS LOWER BACK.

The injured was driving a ram car, from batt. barn to unit, when a 2x4x8' was bolted to roof and isolation curtain was nailed to it. The 2x4 had been knocked loose from roof bolt & was hanging down, as he drove the ram car thru curtain, the 2x4 caught under canopy of car, came in cab, striking him on right knee.
The rock fall was in xcut between belt entry and neutral entry at 520 east panel, fall was approx 5' high 19' wide and 40' long. This was supported with 5' tension rebar roof bolts. The 2 approaches have been cribbed off. The unit at 1170' working out intake rooms. The fall will not be cleaned up.
The fall was in the #2 main north entry, intake at +5721. The fall was in partial intersection & mostly inbetween the pillars. The fall will be cleaned up; in the process of setting timbers in ajoining entries.
Had partial rock fall in South return panel, #8 entry. A slip was running through this area. The fall was approximately 25' long, 16' wide & 6' to 8' high. This area was bolted with 5' tension rebar roof bolts and was reinforced with some 8' pins. Fall will not be cleaned up. It is cribbed & timbered at approaches.
Rock fall is in West Intake, #2 Entry, West Panel. Fall was bolted with 5' Tension Rebar Roof bolts. Fall is 5' high, appx 30' long, 18' wide. Approaches are timbered.
The injured strained his back while lifting a battery cell. He was replacing a bad cell in ram car battery.
THE INJURED WAS OPERATING THE SCOOP, HE HAD HIS LEFT HAND ON CANOPY EDGE. WHILE TRAMMING SCOOP, THE CANOPY CAME IN CONTACT WITH THE MINE ROOF, MASHING OFF THE END OF (2) FINGERS ON HIS LT. HAND. INDEX & MIDDLE FINGERS, JUST BEHIND FINGER-NAIL.
The rock fall was in a x cut between the supply road and belt entry. the support in this area is 5' tension rebai roof bolts, both approaches have been cribbed. this was in the best panel at 7425 the face on unit is 2240.
THE DYNAMIC BRAKES WAS NOT WORKING ON HOIST. ELECTRIC PANEL WAS CHECKED AND THE CONTROL TRANS AND 2 DTODE WERE BAD. PARTS WERE CHANGED OUT AND WOIST WAS OPERATING CORRECTLY AT 925 AM.
THE INJURED SAID HE WAS LOADING CRIB TIES (4 X 6 X 30" WOOD) INTO SCOOP BUCKET WHEN A PIECE OF ROCK FELL FROM ROOF & STRUCK HIM ON RT. ANKLE.
EE said he was walking in #7 entry (in return) when he stepped in a hole and twisted his left knee.
EE SAID A PIECE OF ROCK FELL FROM MINE ROOF & STRUCK HIM ON HEAD, HURTING HIS NECK. HE WAS ON M.NORTH BELT LINE CHANGING B.O. ROLLERS. HE HAS BEEN OFF WITH NECK INJURY AT EARLY DATES.
THE INJURED WAS SHOVELING ON N. BELT LINE, WHEN HIS SHOULDER STARTED HURTING.
The injured was operating s&s scoop, he was driving scoop on haulage read just inby feeder, when the ram car operator ran into scoop. the injured stepped in haulage road & car driver did not see him.
THE HOIST BROKE DOWN APPROX 40 MIN, 4:00 PM TO 4:40 PM. THE SOUPLING BETWEEN MOTOR & REDUCER CAME APART. A NEW COUPLER WAS INSTALLED. WORK AT FACE WAS STOPPED DURING BREAK DOWN OF HOIST.
HAD ROCK FALL IN THE MAIN NORTH INTAKE, #3 ENTRY, AT 4 WAY INTERSECTION, AT 4344. FALL IS 80' LONG, 5' HIGH, 17' WIDE. WAS SUPPORTED WITH 5' FULLY GROUTED ROOF BOLTS. APPROACHES ARE BEING TIMBERED AND CRIBBED INBY FALL.
EE WAS CHANGING BELTS ON THE #4 BELT DRIVE JUST INSIDE THE WASHER. THE CHUTE AT #4 TAIL WAS PLUGGED. 2 OTHER MEN WERE IN THE AREA, TRYING TO CLEAR TAIL ROLLER & CHUTE. THE THOUGHT EE HAD GONE TO RESTROOM. WHEN THEY JOGGED THE BELT, THE EE GOT HIS 4 LEFT FINGERS IN BETWEEN PULLEYS AND BELT AT DRIVE MOTOR.
EE IS UNIT REPAIRMAN. HE WAS TRYING TO TAKE FITTING OFF OF Y STRAINER ON FEEDER WATER SYSTEM WITH A PIPE WRENCH. WHILE PULLING ON PIPE WRENCH, HE STRAINED LOWER BACK MUSCLE.
Employee was working on the back side of unit belt tail. The belt went off for unknown reason. as he was crawling across top of belt it started up carrying him down the belt for approx. 70'. The area is very low and he hit the mine roof several times.
The injured was dragging with S&S tractor and drag. He ran over a piece of rock in the road, this bounced the tractor and threw him up against the canopy on the tractor, straining his neck muscles.
THE JOY MINER CABLE HAD BEEN DAMAGED WHICH KNOCKED(TRIPPED) BREAKER TO MINER. INJURED CHECKED CABLE, REPAIRED IT AND CHECKED CABLE AGAIN. AFTER REPAIR, CABLE CHECKED OK. WHEN EE THREW POWER UP TO THIS MINER, THE BREAKER BLEW-UP BURNING HIS RIGHT HAND.
The rock fall is in #2 entry +980, west intake. Fall was 5' high, 90' long, 18' wide, at intersection. #1, 2, 3 entries are all intake, #1, #3 entries are ok. We have cribbed & timbered approaches to fall & cribbed #1, #3 entries. Notified MSHA insp. on 6-12-04 of fall.
The injured was roof bolting, after putting bolt in roof, he let his foot off before letting the canopy down, when he did this the boom and canopy came down 3-4", hit him in the head, he had too much pressure on foot jack and canopy.
THE INJURED SAID HE WAS PUTTING HYD. OIL IN ROOF BOLTER WHEN HE STRAINED HIS LOWER BACK. 5 GALLON CANS.
EE said he was riding in d-mantrip, when driver ran over a rock, this caused ee to fall over against other seat and hurt his rib on left side. The driver of mantrip did not remember this happening.
EE SAID HE WAS HAND LOADING THE ROCK DUSTER (BAG DUST). HE HAD 2 STACKS OF BAG DUST, 6 BAGS EACH. WHILE CUTTING OPEN A BAG, THE OTHER BAG FELL OVER AND HIT HIM ON NECK.
ROCK FALLS IN RETURN AIR COURSE, IN 2ND N. INTAKE ROOM. THE FALLS WERE IN INTERSECTION, NEXT TO BARRIER PILLAR. LOACATION OF FALLS +390 - EAST ROOM TO 2ND NORTH FROM +1020 TO 1380.
