EE parked EE's machine to use the restroom. When EE returned to the loader, EE grabbed the handrails. When pulling self up, EE felt and heard a "pop" in left shoulder. EE proceeded up the ladder, sat down, and immediately notified the Foreman.

Employee came up to the Scalehouse. Stated that had chest pains. We asked a few questions. EE was responsive and coherent, but pale. We decided to call the paramedics. The paramedics showed up and put monitors on EE. The paramedics took EE via ambulance, to Platte Valley Hospital, in Brighton, CO.

EE was connecting a 4" discharge line. The discharge line required Bauer fittings. The last coupling gave EE problems. This connection gave EE problems and took extra effort to seat. this is when EE felt a strain in EE's abdomen.

Employee called me on the company radio around 9am. Employee stated that they were having the same chest pains and tightness that they had the previous week. Decided to take them to the hospital myself. We went to the same hospital, located in Brighton, CO. Platte valley Medical.

Employee finished using the restroom. EE then started complaining of tightness and pain in the chest. EE stated that EE thinks is having a heart attack. 911 was contacted, and we had medical teams dispatched to the site. The EMT's took EE to Platte Valley Hospital, located in Brighton, CO.

Operating a Cat skid steer, employee bottomed out the seat. After deciding the skid was too small. The employee decided to use a larger piece of equipment. EE had jump start it. Putting the cover back on the battery, the employee was twisting/turning and felt a numbing sensation in EE's back. EE also stated that this is a re-accruing injury.

The employee was preparing to grease the skid steer with an air grease as he was unrolling the grease line he started walking backwards not realizing how close the skid steer was parked he tripped backwards onto the bucket. The result of fall caused a left hip /lower back contusion

Employee was removing steel liners from hopper. A chipping hammer was used as wedge while cutting and got very hot. After completing removal of liner, he sat the hammer down in front of him and began grinding the old welds. He inadvertently knelt on hot hammer while grinding and jumped releasing hold on grinder. Grinding wheel came in contact with left thumb causing small cut.

Employee was diagnosed with Carpal Tunnel Syndrome on June 19th by the Company's Workers Comp Doctors. This process of determination of CTS had been going on over a period of 2-3 months while the employee continued to work with no Lost Time and or Restrictive Duty. The Plant Management Team was not made aware of this diagnosis by Company Personnel until July 10th 2014

Employee was welding, when welding the employee received flash burn to both eyes from another welder in the same area. The employee reported the flash burn 16 hours later, we are not exactly sure on the time of exposure.

On 10/16/12, EE was exiting the cat skid steer. Upon exiting, EE hit her shin on the cab which possibly cause here to slip. EE reported the accident to another EE on 10/23/12. On 10/24/12 EE showed me how she thought she may have hurt her thumb-wrist area-EE was taken to the doctor where she was given a brace-she was cleared to return to her full duty as an operator.

Employee was in a sited position (sitting Indian Style) as he rose he felt a large pop in his left knee.

On 2 Feb 12, employee was attempting to repair equipment. Employee felt something abnormal in his back while trying to remove a nut. Employee did not report the incident to his supervisor until 10 Feb 12. On 11 Feb 12, employee went a clinic and received medication. On 17 Feb 12, employee was taken to Concentra who placed the employee on restriction and medication.

The maintenance workers were splicing a conveyor belt. A worker slipped into an angle iron cross brace and bruised his chest.

Injured was stepping over the tailgate of his pickup truck, he placed his left foot on the bumper of the truck and his left hand on top at the tailgate. He swung his right leg over the tailgate. His left foot slipped off the bumper causing him to fall on his left side.

Employee was using a metal grinder after welding and grinder debris got into his eye. He was wearing safety glasses.

The employee was changing a flat tire on a water truck when he hit his hand with a sledge hammer while attaching the rim.

Miner was walking around crusher plant. Employee stated he was wearing safety glasses. A foreign object got into his eye at an unknown time.

Employee walked through woods to examine work area and came into contact with poison ivy. Employee developed contact dermatitis as a result.

The miner was assisting in the clean-up of a diesel fuel leak when he came into contact with poison ivy. The spill had run into a wooded area where poison ivy was present. The employee developed a rash the next day (6/29). On 6/30, the employee received a steroid shot to help dissipate the rash.

Employee was cleaing silt when a block of wood slipped, then flipped and hit the employee in the right ankle.

The employee was perfoming maintenance on hydraulic hoses on a classifying tank when hydraulic fluid got in his eyes. Employee suffered temporary conjunctivitis.

EE was steering the loader, he turned the wheel counter clock wise and felt a sharp pain in his left shoulder.

Employee slipped on wet ground (bank) while filling I-302 (a water truck).

Employee was cleaning up scrap steel and putting the steel in back of a pick-up truck. The piece in his hand slipped, cutting his wrist.

EE was adjusting the CB antenna, as he walked back across the walkway of the loader, he tripped over a shovel that was laying on the walkway, lost his balance and hit his knee on the hood handle.

EE STEPPED DOWN FROM A CONVEYOR FRAME TO THE TRUCK BODY. AS HE TRANSFERRED HIS WEIGHT FROM ONE LEG TO THE OTHER LEG HE FELT A SHARP PAIN IN HIS RIGHT LOWER BACK.

Mechanic was taking hydraulic pump off a backhoe and it slipped and fell on his foot. Diagnosed at first with contusion of right great toe. Followup visit on 11/3/05 showed fracture. Referred to orthopedic on 11/4/05.

DRIVER WAS OPERATING TRUCK IN HEAVY RAIN. HAUL ROAD WAS VERY SLICK AND TRUCK SLID SLOWLY INTO BERM AT STEEP ANGLE, THEN TURNED OVER. DRIVER CUT LEG WHILE CLIMBING OUT OF TURNED-OVER TRUCK. ALSO SUFFERED LUMBAR STRAIN. TAKEN TO E.R. AT DOCTOR'S HOSPITAL FOR TREATMENT WHERE HE RECEIVED SUTURES.

DRIVING DUMP TRUCK. TAILGATE DID NOT CLOSE, EE GOT OUT TO CLOSE IT AND IT WAS RAINING & MUDDY. HE PUSHED THE TAILGATE CLOSED & HIS FEET SLID OUT FROM UNDER HIM. HE CUT HIS HAND ON PIN.

MINOR EE TOOKBOARAL BRICKS FRONT END LOADER FOR A JOY RIDE AND OVERTURNED IT.

MVA-HEAD ON COLLISION. INJURED EMPLOYEE WAS A PASSENGER RIDING IN A CHEVROLET P/U THAT WAS STRUCK HEAD ON BY A MACK TRUCK. FULL INVESTIGATION PENDING.

MVA-HEAD ON COLLISION. INJURED EMPLOYEE WAS DRIVING A CHEVROLET P/U ON HAUL ROAD AND WAS STRUCK HEAD ON BY A MACK TRUCK. FULL INVESTIGATION PENDING.

DUMP TRUCK OVERTURNED WHILE DUMPING LOAD. NO INJURY (DRIVER WAS WEARING SEATBELT). COSMETIC DAMAGE TO TRUCK (LEFT MIRROR, FENDER, STEPS, CRACK IN HOOD).

ONCOMING GARBAGE TRUCK IN THE MIDDLE OF THE ROADAT A NARROW INTERSECTION--THE TRUCK DRIVER TRIEDTO AVOID A COLLISION AND PULLED OFF TO THE EDGE OF THE ROAD WHERE THERE WAS SOFT SAND. AS IT PUL LED FURTHER INTO THE DITCH IT HIT AN EMBANKMENT WHERE PIPE CROSSES THE ROAD, TURNING THE TRUCK OVER AND UPSIDE DOWN.
