While attempting to remove a hydraulic hose, the employee pulled to hard and gave self a suspected hernia.
A rock rolling around on the crusher return belt came off and struck the employee in the foot. Causing a break to the 2nd metatarsal of the right foot.
While cutting a piece of rubber flashing, the utility knife slipped. The knife entered EE's right hand beside the bottom knuckle closest to the palm of the hand.
While changing screens on the screen deck, the employee lifted the screen to move it into position. While doing so, they felt a pop in their left arm.
Employee was closing out for the day. After parking equipment, employee was walking around it. Employee was stepping over the bucket and their leg twisted. They fell and their knee hit the corner of the bucket.
The employee was riding in the jump seat of a Komatsu HM400-3 during training. The trainer hit a rut in the road causing the employee to strike EE's head on the handle beside EE. The employee suffered a concussion.
While cleaning off cat walk, individual walked beneath area and was struck on the hard hat, knocking the individual to the ground. Entire crew was warned by supervisor that cleaning of cat walk was in process. Individual ignored warnings.
Employee was getting up to Track hoe, that was sitting on shot rock, and had a hand on a small rock that gave way, releasing a larger rock to strike their hand. Employee said they were unable to react in time.
On Monday, January 7, 2019 a miner was injured while operating an articulating truck on surface mine property. The miner was driving the articulating truck down a hill when it struck a berm. The associate was treated at a local hospital for injuries incurred in the accident. Minor damage was sustained to the articulating truck.
Loading a magazine onto a trailer, went to release the chains on the magazine, magazine shifted, laceration to right lower leg.
EE cut EE's hand white trying to remove a bolt with an impact gun on the Kleemann crusher. The bolt was spinning and EE put hand on it to try and hold it. It spun in EE's hand and a burr on the bolt cut the palm of EE's hand. EE required 9 stitches from the injury.
Sweeping bed of truck, broom handle broke at a weak spot and cut right hand
Employee was climbing a ladder and somehow missed gripping the ladder with EE's right hand about 5 feet up. EE fell from ladder but landed on feet at ground level. EE's right foot actually landed on another employee's foot causing EE's to roll and break at the ankle area.
Employee was hanging gaurds etc. on the plant. He noticed his elbow getting sore but did not say anything because he has arthritis. He worked all day. (the 28th) Came into work the next day (29th) with his elbow red and swollen. The doctor said he had an infection in the elbow and should be fine with antibiotics.
THIS IS A REVISED VERSION OF AN ALREADY REPORTED WORK RELATED INJURY. PLEASE NOTE: PER BRIAN IN SAFETY AT MSHA. EE was breaking rock loose using a sledgehammer. He swung the hammer and it ricocheted, striking another rock, jarring and smashing one of his fingers.
Injured was changing a tire on a Bobcat skidsteer. He proceded to remove tire and wheel assembly. The tire and wheel assembly then fell, smashing his middle finger between the inner wheel and axle hub. His finger began to slightly bleed. Employee then notified his supervisor. He was then driven to the local hospital where he recieved ten stitches.
Employee was trying to break up large rock that was stuck in jaw.
Employee was using a box cutter to cut conveyor belt material for use as flashing. He had the knife in his right hand and was cutting towards his body when the blade left the rubber belt and ran across the thumb on his left hand. Employee required 10 stitches but did not have any lost time.
SLICK CONDITIONS (ICE ON GROUND). HAD PREVIOUS PAIN IN SHOULDER PRIOR TO SLIPPING. ALSO HAD BUILDUP OF BONE SPURS PRIOR TO INJURY.
EE WAS CLIMBING DOWN THE LADDER ON THE POWDER TRUCK. WHEN HE SLIPPED, HIS ARM WAS CUT. THE CUT WAS SO CLEAN THAT HE HADN'T REALIZED HE WAS CUT UNTIL SOMEONE POINTED IT OUT. IT STOPPED BLEEDING SHORTLY AFTERWARD.
AS EE WAS MOVING THIS CHAIN, THE SAFETY CATCH CAUGHT HIS THUMB AND THE MOMENTUM OF THE CHAIN BEING DROPPED CAUSED THE SAFETY CATCH TO CUT HIS THUMB.
EE WAS STANDING WITH THE "PORTA-POWER" TOOL ATTACHED TO THE PIECE HE WAS ATTEMPTING TO MOTION INTO PLACE. THE PORTA-POWER SLIPPED, FELL AND STRUCK HIM ON THE NOSE. THIS CAUSED A CUT REQUIRING CLEARING, SUTURES AND A TETNUS SHOT JUST IN CASE
EE WAS LOOSENING THE BOLT & WHILE TRYING TO BREAK IT LOOSE THE WRENCH GAVE WHEN THE BOLT LOOSENED ABRUPTLY. HIS HAND SLIPPED & STRUCK THE NEARBYHAND RAIL & SMASHED HIS RIGHT THUMB A LITTLE BUT PUT A CUT ON IT AS WELL
OPERATOR WAS STARTLED DUE TO NOT KNOWING SOMEONEHAD ENTERED BEHIND HIM. AS HE TURNED ABRUPTLY HE STRUCK A GLASS PAYNE AND CUT HIS HAND ON THE TOP RIGHT.
EMPLOYEE WAS ATTEMPTING TO PULL A HUNG DRILL STEEL OUT OF A FACE BOTTOM HOLE; IT SLIPPED, SMASHING HIS RIGHT THIRD FINGERTIP AGAINST SHOT ROCK. THIS RESULTED IN AN AVULSION/LACERATION TO THE FINGERTIP. X-RAYS SHOWED A SLIGHT HAIRLINE FRACTURE, AND 9 STITCHES WERE REQUIRED TO SUTURE LACERATION.
