Employee was helping remove the cover from the shut that allows materials to flow from the 8x20 Telsmith Sizing Screen to the #15 conveyor. As he was lifting the left side of the cover he lost is balance and fell forward into the cover. He hid his mouth on the cover as he fell.

Employee called me on July 31, 2008 and stated that on July 28, 2008 he was using the Port-a-John at our Mill Spring Quarry and noticed spiders in the facility. He said he killed them, then by Tuesday evening noticed some swelling on his left side of the trunk. This swelling had gotten worse by Wednesday evening. He said he thought he had been bitten by a spider.
EE was replacing side rails on the 6x16 Seco Shaker Screen.He was holding a bold head with his right hand and using an impact wrench to tighten a nut on the bolt.EE engaged the nut on the bolt turned catching his glove and wrapping around his hand. The bolt head cut through the glove and cut his right 3rd. finger.
Injured employee was helping with maintenance work on our primary jaw. They had disassembled the feeder rods and were removing the bolts from the washers. He was holding a bolt when another employee slipped and hit his thumb with a hammer.
Employee was dismounting Cat D6 dozer and stepped onto blade. He then jumped off the blade to the ground and sprained his left foot.
Employee was found slumped over in scale house by two customer truck drivers. He had been seen approximately 5 minutes before. 911 was called and Polk County NC EMS arrived by 9:00am. They treated the employee for approximately 20 minutes however he could not be revived.
Two employees were dislodging a rock that had become stuck on conveyor #1. One employee moved the rock and the injured employee's left thumb was pinched between the rock and conveyor frame. This caused a laceration which required stitches.
EMPLOYEE WAS REMOVING A WEDGE (20 TO 25 LBS) WHICH WAS ATTACHED TO A V-BELT FROM THE JAW AREA WHEN HE FELT A SHARP PAIN IN HIS LOWER STOMACH.
