Employee was helping remove the cover from the shut that allows materials to flow from the 8x20 Telsmith Sizing Screen to the #15 conveyor. As he was lifting the left side of the cover he lost is balance and fell forward into the cover. He hid his mouth on the cover as he fell.

Welding burning ring an nut on cone crusher at the Miller Creek Quarry. After one weld employee move over to start another weld. Did not realize that he had put his left leg on the new weld and caught pants on fire.

Employee complained of chest pains and other pains. He was taken to local hospital to be checked out. (St Lukes - Tryon, NC) From there sent to Spartanburg, SC.

The employee was operating our hydraulic hammer in the pit at the Miller Creek Quarry. A piece of rock he was breaking entered the cab on the right side through the side glass. It hit him in the right eye.

Employee was changing an OTR tire in the parking lot of Thompson Cont.. when he got his finger caught between the clamp assembly and rim of the tire. Employee broke his right ring finger

Employee called me on July 31, 2008 and stated that on July 28, 2008 he was using the Port-a-John at our Mill Spring Quarry and noticed spiders in the facility. He said he killed them, then by Tuesday evening noticed some swelling on his left side of the trunk. This swelling had gotten worse by Wednesday evening. He said he thought he had been bitten by a spider.

EE was replacing side rails on the 6x16 Seco Shaker Screen.He was holding a bold head with his right hand and using an impact wrench to tighten a nut on the bolt.EE engaged the nut on the bolt turned catching his glove and wrapping around his hand. The bolt head cut through the glove and cut his right 3rd. finger.

Injured employee was helping with maintenance work on our primary jaw. They had disassembled the feeder rods and were removing the bolts from the washers. He was holding a bolt when another employee slipped and hit his thumb with a hammer.

Employee was dismounting Cat D6 dozer and stepped onto blade. He then jumped off the blade to the ground and sprained his left foot.

Employee was found slumped over in scale house by two customer truck drivers. He had been seen approximately 5 minutes before. 911 was called and Polk County NC EMS arrived by 9:00am. They treated the employee for approximately 20 minutes however he could not be revived.

Two employees were dislodging a rock that had become stuck on conveyor #1. One employee moved the rock and the injured employee's left thumb was pinched between the rock and conveyor frame. This caused a laceration which required stitches.

EMPLOYEE WAS REMOVING A WEDGE (20 TO 25 LBS) WHICH WAS ATTACHED TO A V-BELT FROM THE JAW AREA WHEN HE FELT A SHARP PAIN IN HIS LOWER STOMACH.

EE WAS ROTATING A PULLY BY HAND GOT HIS RT INDEXFINGER CAUGHT BETWEEN PULLY & CONVEYOR FRAME

EE WAS DISLODGEING ROCKS AND FINES FORM THE ROCK BOX BETWEEN THE #1 AND #2 A CONVEYORS. WHEN HE FINISHED THE SHOVEL HE WAS USING WAS CAUGHT AND PULLED HIS ARM AGAINST THE HEAD PULLEY OF #1 CON VEYOR. HIS ARM WAS PULLED AGAINST THE HEAD PULLEY AND A METAL FRAMFOR THE BELT SCRAPPER.

CHANGING BELTS ON FEEDER AT SAW. HAD USE A BAR AND HAMMER TO MOVE PULLEY. BAR SLIPPED AND HIT EE IN (LT) RIB CAGE.

EE RAISED BALL UNTIL IT HIT BOOM. HE CONTIUED TO RAISE BALL WHICH CAUSED BOOM TO RAISE CAUSING MACHINE TO FALL OVER TO RIGHT .

EMPLOYEE WAS HELPING REPLACE CABLE ON A NW80D. THE WEDGE SLIPPED CAUSING THE DRUM TO RECOIL AND THE CABLE CUT HIS FINGER.

EMPLOYEE WAS TRYING TO REMOVE SCREEN CLOTH FROM CHUTE IN HOPPER. SLIPPED AND FELL ON HIS BACK. 5-6' FALL.

EMPLOYEE SLIPPED AND CAUHT HIS SHOE ON A LADDER AS HE WAS CLIMBING. HSI TOE WAS WEDGED BETWEEN STEP & METAL CAUSING FOOT TO BE BROKEN.

EMPLOYEE PULLING 1" CABLE AROUND ROCK TO BE PULLED FROM JAW HOPPER.

EMPLOYEE WAS PUTTING A WEDGE IN CONE AREA OF JAW. HOLDING WEDGE WHEN IT SLIPPED CAUSING HAMMER TO HIT FINGER. FINGER REQUIRED CLEANING AND STITCHES.

EMPLOYEE WAS LIFTING SMALL DRUM OF GREASE & SPRAINED HIS BACK.

EMPLOYEE WAS GREASING PLANT RAISED UP & HIT HIS HEAD ON BRACE ON BIN CUT HEAD (10 STITCHES) & PULLED HIS NECK

EMPL TRYING TO PUSH BRIDGED -OVER ROCK OUT OF FEEDER BOX INTO JAW CRUSHER WHEN ROCK BROKE LOOSE AND HE FELL INTO JAW CRUSHER.

EMPLOYEE WAS PRYING A ROCK THAT WAS HUNG IN CRUSHER AND BAR SLIPPED AND HIT TOP OF RIGHT FOOT.

EMPLOYEE WAS HITTING A WRENCH WITH A 2LB HAMMER HE MISSED AND HIT HIS FINGER.

EMPLOYEE WAS HELPING WORK ON ENGINE. WHEN JOB WAS COMPLETE, HIS FOOT GOT CAUGHT OR WEDGED WHEN HE STARTED TO EXIT MACHINE. HE FELL WITH FOOT WEDGED AND TWISTED HIS KNEE.

EMPL WAS GREASING HEAD PULLEY ON #2 CONVEYOR WHEN THE JAW CRUSHER OPERATOR STARTED THE BEL JAW CRUSHER OPERTOR DID NOT NOW EMPL WAS ON BELT EMPL FELL APPROX 18-20' ONTO SURGE PILE

EMPL ENTERED SIDE DOOR OF SWITCH TRAILER & TURNED ANKLE & TWISTED KNEE ON UNLEVEL PORTION OF FLOOR AT ENTRANCE.
