Employee was getting out of a Euclid R25 that was parked next to walkway/platform, lost his footing causing him to slip and fall. When he landed he felt pain in his left ankle.

Employee was operating the scraper. While discharging material from scraper, a large rock caused the machine to come to a complete stop violently shaking the cab and the operator.
Employee was cutting steel plate 1/2"x40"X15' from a larger sheet, when the cut plate fell to the ground, then fell on employee.
Employee was changing Anvils on Impact Crusher, lifted and dropped anvil on left middle finger
Employee was cutting a rusty overhead pipe with an acetylene torch when slag fell onto suspended acetylene hose causing a second degree burn to injured right lower abdomen.
EE opened left side access door on Bulldozer to change oil filters. While doing task the EE squatted on the top of the tracks and the door closed and struck the EE's right knee. EE suffered a bruise to his right knee.
Employee reached down to move a broken piece of concrete. The piece of concrete had a rusty nail sticking out and stuck into employee's hand under thumb.
EMPLOYEE WAS ON TOP OF AN R25 MACHINE TRYING TO POSITION A HOIST CYLINDER PIN, THE CYLINDER MOVED & PUSHED HIM OFF THE MACHINE.
EE was lifting a metal barrel lid and grease pump (approx 34'40 lbs) to cover a 55 gallon grease barrel, when he felt a pain in his back. Doctor diagnosed upper lumbar strain.
Injured was walking down smooth gradual slope & twisted his left ankle. Initially he thought it was a mild sprain, however, within a couple of days it became inflamed & he was taken to the doctor. X-rays indicated a small fracture in his ankle. Because the injured is diabetic & less likely to heal naturally, the orthopedic dr. recommends surgery to minimize complications.
EE was realigning elev chain on 623G by placing log between chain & sprkt while engaed to make chain skip tooth. The log rolled across outer housing pulling his hand across surface smashing/severing tip of right ring finger. He rtw w/in 2 days w/restrict due to pain meds. He chose not to take meds and was allowed to operate 623G as long as no meds were taken. EE retrained.
The injured employee was either lifting screens or shoveling under a tailpulley when he strained his lower back. Cannot quantify damage or impairment at this time. Previous injury from prior employment could have contributed to injury.
EMPLOYEE WAS LOWERING HINGED CHUTE WHEN HIS STOMACH WAS SCRAPED BY HINGE POINT OF CHUTE
Small metal fragment broke off of excavator tip and lodged in injured's bicep. ER Dr chose not to remove item b/c no danger as did surgeon. Injured to return in one week for removal of 2 stitches. No lost time/No impair
EE WAS CUTTING FRAME OFF OF OVERTURNED TRAILER WHEN METAL FRAME CAME LOOSE AND STRUCK EE'S HAND.
EMPLOYEE WAS OBSERVING OPERATION OF EUCLID END-DUMP FROM PASSENGER SEAT. THE END-DUMP OPERATOR BACKED INTO RETAINING WALL OF LOAD OUT BIN CAUSING EMPLOYEE TO STRAIN HIS NECK.
EE WAS HELPING LIFT A PIPE ONTO A TABLE. HE GRABBED HIS BACK AND SAID IT HURT. HE HAD AGGREVATED AN OLDER INJURY.
