Belt was lowered so chains could be adjusted. While lifting the operator also went into reverse & one unhooked, catching the frame throwing a miner to the ground. This miner received stitches for a laceration to right side of face, two broken bones in left wrist, and three broken ribs on the right side of rib cage.

Guard was removed from front head pulley to manually attempt to free conveyor. Employee grabbed exposed v-belt. At this time another employee started conveyor belt grabbing the right hand & lodging it into the exposed pulley. Employee was taken to Hospital where EE underwent surgery to close off torn pinkie & add pins to two broken fingers.

Employee was using a small tractor to clean under a stationary screening unit. This particular instance the tractor operator went in too far under the screen striking left wrist, that was positioned on the steering wheel with the steel frame of the screening unit. EE then notified Crusher Superintendent who took EE to a hospital for treatment.

Employee was helping another Employee roll up a piece of conveyor belting approximately 124 feet long, by way of pushing the roll of belting, Employee stated he felt a sharp pain in his lower right groin.

Employee was stepping down from a front-end loader and when his foot touched the ground, he stepped and slipped on a rock, causing him to fall to his knee and injure his right ankle. Medical diagnosis: right-ankle sprain.

Employee, had climbed up onto an old shaker that was out of service and for sale in an area of the pit, known as the 'bone yard'. While atop the piece of equipment, a handrail broke and gave way and the employee fell to the ground, injuring his right shoulder.

EMPLOYEE WAS SERVICING THE LOADER. WHILE GREASING IT THE GREASE HOSE BLEW OFF FITTING, STRIKING HIS RIGHT PALM.

JUMPED FROM A PIECE OF MACHINERY TO THE GROUND. IMPACTING THE GROUND PRIMARILY WITH ALL HIS WEIGHT ON ONE FOOT, FRACTURING BONE IN HIS HEEL.

DR'S FIRST EVAULATION STATED THIS WAS NOT A WORKRELATED INJURY. EE'S MOTHER COMPLAINED TO DR & INSURANCE
