The employee was fabricating a pump stand. EE moved a 4"x57" piece of channel. It slid off the support at one end & the other end pinched EE's finger on another channel. EE was wearing gloves and inspected their right index finger. It required 3 stitches.

Miner was assisting in wash plant repairs & assembly. While installing parts on the plant, right middle finger was pinched, creating a blood blister under the nail.
The miner was performing a tail pulley adjustment but not using the company-provided gloves or tools. The adjustable-jaw wrench being used, slipped & the miner's left hand contacted the metal guard, cutting the outside edge. EE was taken to the local ER where the injury was cleaned & ee received 11 stitches. EE was released for immediate return to work.
A EE & Co-worker were cleaning rock dust form under a motor base of a inclined finish screen. The 2 men were on a catwalk that collapsed and dropped them 10' to a conveyor which they bounced off of and fell another 7' to plant grade. EE was released.

Employee violated policy and attempted to clear a feed opening from the top when he was engulfed by sand and pulled through the feeder. Damage was loss of consciousness and bruises.
While placing a large wear plate in the jaw hopper, EE was using a pry bar to position the plate for attachment. The plate slipped on the bar, slapping the bar against the hopper. EE's hands were on the bar and his middle finger on his left hand got pinched between the bar and the hopper
While performing maintenance, employee stepped down off of the tail frame of the secondary cone supply conveyor. Employee lost his grip on the handrail and fell approximatly 30" on to the edge of a concrete block, fracturing his left hip.
EMPLOYEE WERE REMOVING A FLYWHEEL FROM THE SIDE OD A WASH SCREEN. A "T" BAR WEIGHING APPROX 10 LBS WAS USED A SPACER TO JACK THE FLYWHEEL OUT. WHEN THE FLYWHEEL CAME LOOSE THE BAR FELL APPROX (3) THREE FEET STRIKING EMPLOYEE'S RIGHT INSTEP OF HIS RIGHT FOOT.
