Employee was exiting Skid Steer and hit EE's head on plant staircase. Closed head injury. Contusion and laceration.

Employee was weed-eating near the scrap metal storage area. EE tripped and cut EE's right palm on an aluminum truck fender. The laceration was sutured closed.

Employee was attaching a hose to a portable water pump used for drainage. They pinched right pinky with the hoses attachment cam-lock while locking in the hose. Bruising only.

Employee was installing a new track retainer pin on a CAT 349 Excavator. When hitting the pin with a sledgehammer a piece of the pin broke off and impacted the employees leg. This left a puncture wound on the employee right thigh.

Employee was performing maintenance on a front wheel loader engine bay. EE lost balance and fell ~4ft to the shop floor. EE's right ring finger was dislocated and the Sacrum bone was cracked.

Employee lost balance in the mud stepping off the ladder of a Articulated Haul Truck and strained Right Knee.

A Customer truck driver pulled over onsite complaining of extreme fatigue, trouble breathing, and feeling sick. 911 was called and EMS arrived on scene to take the driver to the hospital.

Employee was working on CAT Excavator tracks attempting to reinsert a roller pin. While striking the pin with a hammer a sliver a metal broke off and enter EE's right calf. Metal was removed by emergency clinic and 10 stitches were given.

Employee was using pry bars to turn water pump sheaves in order to fit new belts between the fines recovery pumps and its motor. Employee's hand slipped off bar smashing EE's finger.

The silo bucket elevator broke down with loaded buckets. The maintenance crew began working on the elevator and when the backstop and tensioner were removed the buckets rolled slightly and a come-a-long smashed the injured employees hand against the structure. The employee received a deep cut to the left index finger and knuckle.

Worker was attempting to weld on a piece of c-channel iron placed on a welding table. When leaning over they knocked the iron off the table and onto their right foot. The impact broke 2 bones in the arch of their foot.

Employee was greasing excavator boom and slipped on the tracks. Employee hit employee's head on the tracks/ground and received a concussion.

rough waters/wind caused a work boat to slam against the dredge pipe + Pontoon while employees were working on extending the dredgeline. Employee's finger was slammed between the boat edge & pipe. This acutely fractured employee's second distal phalanx of the index finger & caused some small tissue damage.

Employees were cleaning under the main feed belt. Foreman failed to properly lockout plant. Pulley became energized & employee that was using a rod to turn the pulley was struck on hand/side of face(small cut on head). Broke two fingers/lacerated both tendons on left hand(pinky/ring fingers. Everyone involved was suspended & entire plant was re-trained on LO-TO-TO procedures.

Four employees were carrying a steel pipe. Two of them dropped one end of the pipe and the pipe landed on one of their feet. The impact broke the miner's right pinky toe. The miner was wearing steel toe boots.

Employee was conducting maintenance on a dozer's tension idler. The idler was unknowingly under pressure and when the mechanic began removing the bolts holding the idler in place the idler broke loose and struck the mechanic. The idler fractured the knuckle on his right pinky finger and the area of the skull above his right eye.

EE was conducting maintenance on the truck scale. A trucker drove onto the scale despite EE telling the trucker to stop and wait for him to finish. The scale shifted slightly and smashed EE's finger, causing the right ring finger to break.

The Loader Operator was loading on-road commercial trucks with concrete sand. The operator strained his neck while getting a bucket full of material out of the stockpile. A doctor visit revealed the injury to be a minor strain. The employee was on light duty until 3-9-15. The suspension on the loaders chair was replaced to insure a similar injury does not occur again.

The mechanic was under the bulldozer looking for an oil leak. He then tried to get out from between the blade and the machine. His left foot slipped off the track. He landed on uneven ground breaking his left leg.

WE DID NOT KNOW ANYTHING ABOUT THIS UNTIL JAYS BROTHER AREPORTED TO WORK ON FRIDAY THE 26 AND JAY WAS IN THE HOSPITAL WITH POSSIBLE MONOXIDE POISONING FROM DREDGE MOTOR.

STRAIGHTENED UP FROM A SQUATTING POSITION AND HEARD A SERIES OF POPE IN KNEE. WOKE UP THE NEXT DAY AND COULD NOT STRAIGHTEN OR STAND ON KNEE.

CLEANING TRUCK RAISED UP AND HIT HEAD ON BRAKE CHAMBER OF TRUCK
