Miner was hardfacing mill parts. After stopping miner raised the welding mask to look at the progress. Material popped off the surface as miner was looking at it and it struck miner's face and eye.

Miner Disconnected a 4" line to unplug it. The line had not fully depressurized. Cement was blown into the face of the Miner. The miner's eyes were injured, at this time indications are the injury will result in the loss of 2 days work.

A 50# bag started to slip from the pallet the employee was pulling bags from. The miner grabbed the slipping bag. EE believes that is when EE pulled a muscle in EE's forearm. Sprain of Left wrist.

Miner was walking on a surface that was wet and slippery. He slipped and fell, hitting his head on the concrete. This resulted in a laceration on the back of his head.

Miner was removing a cap to free the journal from the anchor box. The anchor box was hanging from a cherry picker attached to the forks of a lift truck. The forks suddenly and unexpectedly dropped and cherry picker fell on the miner's hand pinching it between the journal and the cherry picker.

A bag of barite broke open and barite got into the eye of the miner

Miner was packing and stacking 100 lb. bags of barite. Instead of dropping the bag in place on to the pallet as he was trained to do, he threw the bag and strained his back

Employee was removing a cover to a Derrick screen. As he was setting it down, it slipped out of his hand and fell on his right foot.

EE was to tighten the belts on the blower. He turned off and locked out the blower. The belt was still turning because of vacuum from the dust collector. EE grabbed the belt and it pulled his fingers into the pulley.

Employee was putting bags on 25 lb blocks of clay when his arm became sore. The next day it was swollen. He was diagnosed with tendonitis.

EMPLOYEE WAS CUTTING THE BOLTS HOLDING A METAL PLATE. WHEN HE CUT THROUGH THE LAST BOLT, THE PLATE FELL ONTO HIS FOOT.

EE ASSIGNED TO B-MILL. PACKING OFF 50 LB BAGS.

EE WAS CHANGING PLATES ON PUGMILL USING HAMMER
