Going up in man-lift and vulcanizer started to slide out they grabbed the vulcanizer and caused laceration to left hand.
While overseeing a maintenance job, the manager was checking the crews progress of reinstalling the impeller. The manager placed a hand on the pump shell to look around the back side. The hoist tool shifted onto manager's hand causing crushing injury to left ring finger.
The top row of paddles was installed on dual log-washer shafts. While using a pipe wrench to turn shaft for additional paddle installation the unbalanced weight of the shafts caused the shafts to rotate quickly, the quick rotation caused the pipe wrench to turn with the shaft, pinning employee's wrists between the pipe wrench the adjacent metal enclosure.
The contractor could not line up the track bolts and used a 10-lb sledgehammer to relieve pressure on the links. When striking the track pin, a metal fragment from the pin sheared off penetrating contractor's left upper thigh requiring a tourniquet. Emergency medical services transported contractor to an ER for treatment. Contractor was released several hours later.
while installing the 122 steel paddles mount on a log shaft being prepared for placement in log washer, using a ratchet to tighten bolts, the ratchet slipped causing employee to lose balance. EE fell and twisted right leg/foot.
Repairing the log washer, walking to the job and fell into. Received laceration to head, and scrapes on both legs.

Dredge mining winter repair - replacing and realign #3 sand screw liner plate - weight 300-400 lbs. was 4' X 8" X 1/4" steel. Dropping liner plate into screw box landing on hand of employee.
Loading sand from high wall to hopper in pit. Employee states seatbelt buckle came unfasten , breaks pressed which caused employee's head to hit windshield
Employee picked up 5HP motor being moved from the towboat to the electrical shop. As he was carrying the motor to the pump flat he encountered un-level surface. He over extended and lost his balance, as he was going down slowly, his right hand was caught between the motor and barge surface. His fourth finger was cut (bursting laceration)
Installing clam bucket on excavator, trying to hook up hydraulic line on 375L excavator CLCN05 hydraulic line to bucket, popping in right elbow. It happened on 4-7-14 but still working full time, was not stop time loss until his surgery on 7-1-14.
Was putting wire rope on gravel barge and the wind took off, pushed the barge and the wire rope got tight, knocked him to the deck of the dredge and landed on right shoulder. Compensated till 4-5-14 then MRI (4-14-14) to rule out fracture or tear

Tire contractor came to site and was given specific hazard training. Loader was outside of shop on gravel area that was still damp from morning rain. Front of loader was jacked up and placed on jack block. Contractor went to back and placed jack on blocks. Jack fell off of block striking contractor on left hand on third finger.
MOVING CUT STONE, TURNED TO PUT IT ON A SKID AND HURT A POP IN LOWER BACK
Was tightening lugs in the starter box, box blew out.
Person was in shop cutting the edge of a tire when utility knife slipped and punctured right knee. Person received five stitches to close puncture wound. Person returned to regular duty.
Using a high pressure steamer, spraying material off a customers truck. When he pulled the trigger, he sprayed his middle finger on his right hand, cutting it.
Lifting chute on the front of the sweeco shaker with a come along. Slipped and caught finger on the Sweeco trough.
Sometime, the aftenoon of 5/12/10, the employee was helping load a 55 gallon drum into the bucket of a front end loader, when he felt a pain in his upper back. Employee states the pain is a direct result of loading the drum but did not report it until the next morning.
Was looking for a bad spot in the belt, when the wind gusted and blew sand into right eye.

EE was replacing conveyor belt on coarse sand conveyor, as belt was being pulled on. Employees involed were using a 2x6 board as a guide to keep belt from sliding off conveyor rollers, board suddenly broke striking EE in left shoulder knocking him to the ground.
EE was taking out troughing roller to repair conveyor belt: troughing roller slipped off conveyor frame unexpectedly and strained ee's wrist while attempting to catch it. He returned 10/8/07 for follow-up dr visit. Dr wants to try physical therapy starting 10/10/07 on wrist. Return to dr 10/22/07.
Employee was cutting conveyor belts while doing maintenance repair. While cutting belt with utility knife, knife slipped cutting upper right leg.
Employee was preparing to load sand into a railroad car. Before sand could be loaded, hopper door on railroad car had to be closed. As employee was pulling the door shut, he experienced a burning sensation in his left elbow. On 7/18/07 doctor informed employee of cortisone shot on 7/23/07.
EE was welding a brace on logwasher. EE placed 6' (2x8) board on 3rd rung of ladder and guardrail of logwasher, (unsecured)for platform. Scaffold board slid, causing him to lose his balance and fall into the logwasher, approx 44" from bottom of the board.
Employee was working on a conveyor from manlift basket and strained lower back while replacing a bearing on conveyor. 9/19/06 - Employee had six physical therapy treatments. Employee continues to perform regular duties.
Employee was putting the flipper door out on conveyor. As he was turning the handle, the pulley broke, handle spun out of employee's hand and handle struck employee in the forehead.
EE was attempting to cut a piece of conveyor belt using a box cutter. Cutter slipped, striking left forearm.
EEwas taking guard off conveyor. As he stepped down 12", he twisted left knee. EE refused treatment at that time. On 6/26/06 employer made employee go to DR. Determined possible meniscus tear. MRI was done. On 7/3/06 he went to orthopedic. He received by mail notice of surgery scheduled for 7/20/06. As of 7/12/06 ee has performed his regular duties without medication.
Approx. 2:15 AM EE was cleaning around gravel plant. EE attempted to walk outsde the guardrail on the plant and fell from the plant to ground.
Placing cable on end of barge to serve as safety devise. Strained chest wall cavity while pulling cable from one side of barge to the other side.
OTHER EMPLOYEE WAS MOVING A BARGE WITH WINCH. AS BARGE WAS MOVING THE WINCH CABLE UNEXPECTEDLY CAME OFF STRIKING EMPLOYEE IN THE LEGS

While employee was working on conveyor cutting skirt board rubber with razor knife, knife slipped and cut employee.
WHILE REPAIRING COMPUTER & CABLES, EE WAS KNEELING DOWN TO CONNECT CABLE & WHEN HIS KNEE CAME IN CONTACT WITH A CABLE ON FLOOR, EE JERKED & HIT HIS HEAD ON EDGE OF DESK.
EMPLOYEE WAS USING UTILITY KNIFE TO STRIP ELECTRICAL WIRES WHILE ATTEMPTING TO REPAIR LIGHTS ON A SERVICE TRUCK. KNIFE SLIPPED AND CUT FINGER.
INJURED EE STATES HE WAS CLEANING HIS SAFETY GLASSES WHILE A CO-WORKER ATTEMPTED TO CUT WINCH CABLE USING A SLEDGE HAMMER & CABLE CUTTER. WHEN CO-WORKER STRUCK THE CABLE CUTTER WITH THE HAMME R, A ROCK FLEW FROM UNDER THE CABLE CUTTER STRIKING INJURED EE IN RIGHT EYE.
SLIDING BATTERY CHARGER OFF FENDER, SLIPPED OUT OF RIGHT HAND, STRAINED BACK WHILE TRYING TO CATCH IT.
