THE EMPLOYEE WAS CHANGING A BELT, AND ANOTHER EMPLOYEE ACCIDENTLY HIT WRONG BUTTON AND TURNED THE ENGINE ON. THE BELT STARTED TURNING AND IT BROKE THE SKIN AND THE TIP OF THE MIDDLE FINGER ON THE LEFT HAND.
EE WAS HOLDING STEADY A GEAR THAT WAS SETTING ON A LEDGE PRIOR TO INSTALLATION. THE GEAR SLIPPED OFF OF LEDGE TURNING OVER AND LANDING ON EE'S FOOT. NO BONES WERE BROKEN BUT UNSURE ABOUT OTHER INJURY TO HIS FOOT.
EMPLOYEE WAS USING A TRACOTR TO CLEAN SPILLAGE AROUND THE CRUSHER. HE BACKED UP TO THE PLANT TO LOWERED THE BOX BLADE ON THE BACK TO DRAG OFF THE MATERIAL. INSTEAD OF CHANGING GEARS ON THETR ACTOR TO GO FORWARD, HE PUT IT IN ANOTHER REVERSE GEAR. UPON LETTING OU THE CLUTCH, THETRACOTR BACKED UNDER THE CRUSHER PINNING HIM BETWEEN THE CRUSHER FRAME AND THE STEERING WHEEL ON THE TRA
THE EMPLOYEE WA8 WORKING ON A CHUTE ON A SIZING SCREEN WHEN THE CONVEYOR HE WAS SITTING ON WAS TURNED ON. THE CONVEYOR CARRIED HIM BACKWARD HITTING HIS BACK ON THE REVERSE SIDE OF THE CHUTE.
