employee was at the 500 scalping screen conveyor - it had gotten clogged and EE was removing rock from the conveyor when a rock fell from the platform above EE (EE thinks, but is not sure where the rock came from) and hit EE below EE's right shoulder blade; treated, diagnosed with internal bruising, cracked rib

EE was in the screen replacing panels, using a pry bar and hammer, hammer slipped and hit EE in the left knee

Employee worked almost all of the shift, they reported during the day they were experiencing minimal urination stating heat seemed to be affecting them. Employee was taken to the office, given cool water, rest in air condition until they felt ready to return to the plant.

Employee was pulling hose up on loader and felt pain in left shoulder

loosening a bolt on the WA600, sharp edge on bolt caused laceration on left hand palm. Was wearing gloves.

after changing screens on the 7x pit side screendeck top deck screens, the employee stood up and said their back was hurting. Diagnosed with lumbar muscle strain, working modified duty, return to dr 12/19

Employee was changing return rollers on C11. Employee lifted the belt to install the return roller and felt a sharp pain in neck and back.

employee was standing on bumper of pickup truck getting tool out of bed of truck. When EE started to get off the bumper EE's boot got hooked between the truck and bumper and EE sprained EE's right ankle

Employee was using a small hammer to hammer a pin in place. It appears a piece of rock broke off and struck the left upper arm of the employee, causing a laceration.

Employee was stepping off barge onto uneven ground, sprained right ankle. Ground has been graded to correct the hazard.

Complicated seizure disorder (not work related)

EE was rolling out a torch line from EE's service truck and turned to pull some more line and twisted EE's right knee while turning back toward the truck.

Alleged pebble in boot after working in water (rubber knee high boots on) - unsure of date it occurred. EE is not certain when it happened but it got infected due to diabetic condition and put EE in the hospital for infection, surgery.

used hammer to break metal band on pallet of rollers. hammer hit right index finger. Laceration to finger/sutures required

Changing screen on mid wash screen deck, belly crawling, felt pull in upper left side of back

Employee was checking display lights at scale house, slipped, fell and hit shoulder on rock. Fracture, top, left shoulder

slipped/fell on wet conveyor, pulled muscle in thigh

Employee was using cutting torch, cutting holes in conveyor frame for new bolts. Slag got in the external canal of left ear causing a minor burn.

The miner was operating the excavator. The boom came into the cab contacting the operator.

Employee was using water hose to wash underneath crusher, while bending over he felt a pain in his back.

Operator was backing atop ramp to the feed hopper, just before reaching the bumper stop he felt the ground give way causing his truck to go into the collapsing material.

Driller was tubing drill hole when wind blew dust into his eye. He was wearing safety glasses. He went to quarry office and flushed eye out. Still felt irritated so he went to doctor the next day. Nothing was found in eye but he was prescribed eye drops. No work time was missed.

employee was cutting at incline conveyor with a torch. Slag fell off onto torch hoses causing them to catch on fire. Employee was burned on stomach and palm/thumb of right hand.

Two employees were removing return roller on C1. When the roller cleared the hangar brackets it slipped out of their hands striking the right middle finger of one employee's hands against the concrete pad. Contusion to tendons/ligaments.

Employee was changing drill bit on power drill while it was plugged in. Employee accidentally hit power button and drill came on twisting right index finger, small cut to right thumb. Not wearing gloves. Right index finger fractured

Employee was lifting side of conveyor belt to push support wood over top of roller to install belt buckles and pulled his back muscle

Excessive rainfall caused the Tallapoosa river to overflow its banks and enter our pit area. No equipment or persons were involved.

Heavy rainfall from tropical storm Ida caused the Tallapoosa river to flood our pit located approx. 100 yards from the river bank.

Chging out stationary jaw dye trying to remove wedge from crusher. Metal blocks welded over bolt holes of wedge to prevent bolts from coming out of wedge holes. Porto power placed on back end of bolts to jack wedge out of mainframe of crusher. Pressure applied to bolts to remove wedge, metal block came off wedge, struck employee in L hand. Fracture to hand

Slid over a piece of metal while attempting to remove a rock stuck in the hopper. Metal was hidden by the sand.

Hydraulic line on CAT loader came loose and hit windshield, breaking the windshield. Glass and hydraulic oil hit employee's face/eyes. Employee had safety glasses on. CAT dealer advised they had a recall notice to fix the hydraulic oil line but they were behind and had not sent the notice out to customers.

Employee alleges that he was welding or cutting and sparks went into the air and wind blew them into his ear. Employee states he was wearing appropriate PPE.

Employee was pulling a cable out of the back of a pickup truck (strained lower back).

Employee was pulling on a prybar while working on conveyor belt and pulled something in his back

Employee was working on Radial Sand Stacker, using a screwdriver to try and pry box open. Screwdriver made contact and the box blew up, causing electrical flash burns to eye, hands and face.

Employee slipped and fell on catwalk.

Employee was helping to put V belt on rock shaker. His screwdriver slipped and hit him in the lip, causing a laceration to the lip.

WHILE HITTING WITH A HAMMER, THE TOOTH OF BACK-HOE BUCKET, A PIECE OF THE TOOTH CHIPPED OFF AND STRUCK/CUT UPPER ABDOMINAL AREA OF EE. RESULT WAS A CUT SHIRT AND CUT SKIN CAUSING BLEEDING, BUT NO EVIDENCE OF TOOTH PARTICLE REMAINING IN WOUND. THE CUT WAS APPROX 1 1/4" IN LENGTH. SUPERVISOR TRANSPORTED HIM TO RECEIVE MEDICAL ATTENTION.

STRUCK CORNER OF PIECE OF ANGLE IRON AT FOREHEAD. 1/4 INCH LACERATION REQUIRING ONE STITCHES.

DAYTIME OPERATION 2, OPEN AREA AROUND AGGREGATE STICKNILE. WEATHER AND VISIBILITY NOT A FACTOR. INJURED WORKER DROVE TRUCK PERSONAL THROUGH STOCKPILE AREA AND FAILED TO YIELD TO LOADER IN OPER ATION.

BACK STRAIN WHILE WORKING AROUND PLANT. HAD BEENINVOLVED IN SOME LIFTING DURING COURSE OF DUTY.

OPERATOR WAS WAHING OFF EQUIPMENT (BULLDOZER) AND SLIPPED ON MUD AND FELL BACKWARD ONTO SIDE AND VCRACED A (R) RIB.

EQUIPMENT OPERATOR TWISTED HIS ANKLE DISMOUNTING HIS EQUIPMENT.

EE WAS CLEANING AN AIR SPRAY GUN USED FOR PAINTING AND GOT SOME PAINT THINNER IN HIS EYE.

WIND BLEW SAND INTO HIS EYES WHILE WORKING

HOT METAL TOUCH HIS FOOT.

SLIPPED ON PIPE AND FELL HURTING UPPER BACK.

PICKING UP PC OF STEEL
