Performing maintenance on the 1A conveyor. Propped the step ladder against the tower holding up the tail of the conveyor. Ladder was on uneven ground. Climbed to the 3rd step (36") placed left foot on the tower, step ladder kicked out and she fell to ground.

Mostrando los 12Climbing down ladder from loader. When he stepped to the ground he felt a pop in his knee. Went to the doctor on 6-26-13. X-ray negative. Went back to doctor on 7/1/13 - taken out of work for another week - Rtw on 7-8-13.
Working on a dry screen plant in the garage. Knelt down on the floor and got something in right knee. Throughout the day knee swelled.
Cleaning up dirt around plant. There was a dirt ridge under his feet slipped and fell on knee and injured shoulder.
Covering grizzly bars w/ corrugated metal stepped in between bars and hurt calf. EE originally refused medical treatment. He was laid off on 11/18/11. He went for treatment on 11/23/11 but did not notify us. We just received medical bills.
Employee claims that he hurt his shoulder while tightening bolts on shaker. He did not report this injury until he went to urgent care on 7-30-11. Returned to work on 8-1-11. Went back to doctor, out of work until 8-5-11
Struck by piece of metal (metal bar)
Employee died of natural causes while walking from his dozer to his car to have his lunch. He fell down, driver saw this. CPR was attempted. EMT's took him to Syracuse to St. Joseph's.
Plant shut down for the season. At plant checking on equipment due to recent vandalism and theft. Called in that he injured his leg. Went by ambulance to hospital. No broken bones reported. Waiting to hear from EE.
Conveyor was not locked out. Employee was on the conveyor working. Plant operator started conveyor knocking employee off, falling 12 feet.
Putting the window down in the crane. Bees flew toward employee. He is allergic to bees so he jumped off the crane and hurt his left knee.
Employee was performing preventive maintenance on a twin screw sand tank. He stepped inside the guarded area while the machine was running. His grease gun got caught in the open gear drive resulting in his hand and grease gun being pulled through the open gears.
