Employee was helping to flip the blow Bars at the Hazemag crusher area; pounding out wedges. A piece of metal flew out of the area where they were hammering, hit employee in the left arm, and became embedded in the arm.

EE was using a bar to turn a ratchet wrench, pulling toward himself. The bar slipped and hit the EE on the right side of his head.
Employee strained his chest muscles while performing maintenance on the underground drill.
EE was working on a loader that had its cab & transmission removed. He was where the transmission had been, cleaning the area. He raised an access panel on the hood to give himself more room. His hard hat fell off. He stood to replace it, the wind blew the access panel shut striking him in the head before he could replace his hard hat. He required 4 stitches, 1 day off work.
Bumped head on roof of mine while filling drill holes with explosives
EMPLOYEE WAS DRIVING A MINE HAUL TRUCK. HE MADE A WRONG TURN IN THE MINE, AND WHEN HE DISCOVERED THE WRONG TURN HE BRAKED WHICH THEN CAUSED THE TRUCK TO SLIDE IN THE MUD SLAMMING INTO THE MINE WALL.
EE WAS LIFTING A FOOT VALVE USING A COME-ALONG WHEN HE STRAINED HIS BACK.
STEEL CABLES WERE BEING TIED THRU CONCRETE BARRIERS TO FORM A FOUNDATION FOR A RAMP. EE WAS PUSHING AND PULLING THE CABLES THROUGH THE BARRAIERS WHEN A CABLE GOT STUCK. EE PULLED HARD ON THE C ABLE. IT BROKE AND FLEW BACK INTO HIS FACE CAUSING DAMAGE TO HIS RIGHT EYE.
WAS USING TORCHES TO CUT OLD MOTORS OFF. HE STEPPED BACK & TRIPPED OVER A PIECE OF STEEL.
