EE contacted a low-hanging salt water formation with hard hat while driving into the UG part of mine. Flesh wound was cleaned. No medical treatment & released to full duty. On 4/15, EE experienced a seizure at home. WC determined it was related to the 2/24 incident resulting in a reportable injury. Diagnosed head trauma resulting in post-traumatic epilepsy. EE remains hospitalized.

Ee was walking down stairs and missed last 2 steps. Ee did not fall, caught self with Left leg. Ee did not immediately know that there was any injury.

Employee was stepping down while steadying themselves with Left Hand on a vertical beam. As the hand slid down beam, a burr lacerated the tip of the Left Middle Finger. Requiring sutures to close wound.

EE was working in the auto shop, underneath a truck, when they felt something fall into Right Eye. EE rinsed the eye, but still felt symptoms. EE was seen at clinic, and Dr prescribed antibiotic for preventative measures and released to full duty.

Nov 2019, Ee was walking out of warehouse bay and stepped on uneven ground. Ee felt Left Knee pop, but did not fall. Feb 14, 2020...Ee had surgery on Left Knee as a result of injury.

Nov 5, 2019 Ee was using a bar to clean build-up off of a conveyor frame, the Ee's pinky was pinched between the bar and the handrail. Laceration was cleaned and employee returned to work. Recently, Ee noticed a growth under the skin. Dr recommended that it be removed. Ee had surgery on Feb 2, 2021. Ee lost time due to surgery.

Employee was exiting a Genco jeep and failed to clear leg before body out of vehicle. This caused a strain to employee's Left Knee.

Ee was driving a mantrip, and as traveling through an air curtain the Left front tire climbed the rib. Ee's Right knee was impacted and caused a fracture.

Employee was descending stairs and foot slipped on stair tread. Employee caught weight on handrail and strained shoulder. Employee was checked by a physician and returned to full duty and has been working without restrictions until today. Today the employee went out for shoulder surgery.

Ee was assisting in changing of hoist guide brackets. Some loose salt contacted the Ee's upper left back, just below the shoulder causing a fracture in 3 ribs. The fractures was not found by x-ray, but were discovered during an MRI.

Employee was mopping tile floor in an office area, using a mop and mop bucket

Ee was exchanging oxygen bottles and crushed the tip of Left Pinky Finger, causing damage to fingernail and slight fracture to very tip of pinky.

EE was clearing a dryer feed chute. Fine material from grating above made its way into their Right Eye and caused irritation

While installing a 20'cable bolt, Ee's Left hand was guiding the cable bolt. Ee's glove was caught on cable and caused laceration and fractures to two fingers.

While closing a railcar bottom door, bar slipped and pinched Right ring Finger between bar and rail. Laceration required 3 stitches to close.

Ee was riding service hoist when it came to an abrupt stop. Employee flexed knees and ankles to take shock. Resulting in fracture to metatarsal bone in left foot.

Lining up a pipe flange when HDPE pipe laid over and struck shoulder and pushed employee into side of excavation. Pain in back showed itself several days later.

EE was cutting wire with a hack saw. After repositioning wire, EE misjudged hand placement and drew blade over Left Thumb causing a laceration that required stitches.

On a wet/rainy day, EE slipped while dismounting a dump truck. EE felt a pull in shoulders as they attempted to catch themselves using the grab irons. EE had not had any missed days or restricted duty until now. EE required surgery on Rt Shoulder as a result of incident.

Employee stepped onto cage, and updraft blew debris up past employee's face. Something went behind safety glasses and went into Right Eye, causing minor irritation. Employee flushed eye several times. Later in the day, eye was still irritated, so we took employee to town. Dr. flushed the eye. Dr. gave prescription for antibiotic ointment for precautionary measure.

Employee was returning to lab after collecting a sample. As ee stepped off of concrete stair landing to ground, ee felt a pain in back of Right Knee. After several weeks of discomfort, employee had MRI that showed a need for surgery.

Employee was using a pry bar to unseat and slide out a tram motor. Motor moved, pry bar slipped, and employee's movement carried hands into the frame of the continuous miner frame. Left Middle Finger was pinched between pry bar and miner frame. 2mm was trimmed off the finger tip, 8 sutures were used to close the wound.

Trolley had slipped off of an I-beam. Employee reached across to lift trolley back onto I-beam and felt a pain in stomach. It was determined that the employee will need surgery to repair a minor hernia.

Ee inspecting pump parts. As part was set back, Ee pinched finger against another part. Resulted in a laceration, requiring stitches.

EE and coworker were preparing to roof bolt in area and stopped to bar down loose ribs. As rib fell, a hidden section of loose rib higher on rib dropped striking EE's L forearm. The wound required stitches.

EE clearing chunk of material from feed end of screw conveyor. Pry bar being used pushed through chunk and pinched EE's R hand against the chute enclosure. Fractured R wrist & #5 finger. First LWD 08/02

EE lifting the travel-side return idler bracket after splicing conveyor belt. Idler shaft had not fully set in bracket & slipped out. Sudden release carried bracket to EE's face chipping a tooth--required dental repair.

Operating Genco truck pulling trailer with 2 MT totes. Genco struck rock and EE lost control of steering. Genco rode up on muck and overturned. EE extricated self immediately. Abrasions and bruises--non-displaced fracture R ulnar styloid.

Contract Millwright was cutting zip-ties with a knife. Knife slipped while cutting and cut the left hand between the thumb and index finger. EE received sutures at local emergency clinic. Doctor placed EE on light duty to help keep injury clean for 7-10 days.

EE reported feeling a pop in R shoulder while loading tools onto vehicle. Injury treated conservatively but subsequently determined to be a torn rotator cuff requiring surgical repair on 03-18-2016--first LWD.

EE was walking from the change room to the electric shop before shift start and slipped, straining his L knee. The injury was treated conservatively but required out-patient surgery on 2/5/16.

EE working on a shuttle car driveline when some material fell into L eye. No discomfort observed until after shift. Reported to supervisor on 12/7. Medical treated with prescription antibiotic eye-drops. RTW.

EE stepped on a rock and fell catching fall with R arm. Shoulder pain treated conservatively and EE continued to perform job functions. Subsequent medical evaluation identified shoulder damage requiring surgery. First LWD 11/4/2015.

One of the B-skip guides had broken from undetermined causes. This occurred at a location below the skip feed location underground. As the skip passed the break, it caught the guide bending some support structure. MSHA Hotline notified at 11:49.

EE attempting to close ball valve at 3-A preg cyclone. Significant force was required to close the valve resulting in contact with an adjacent (2-A) valve. A laceration to the R thumb required stitches to close.

EE pinched R #3 fingertip between conveyor take-up hyd. cylinder and frame. Stitches were required to close the wound.

EE was using a torque multiplier to install bolts on miner. Tool locked up. EE attempted to reverse setting when tool slipped pinching R index finger against motor. Fracture

A fire in a construction debris pile was reported at 2:45 p.m. Fire service was called and responded. The pile was located in a remote area of the property. The fire was contained and finally extinguished by 7:45 p.m. A fire watch was maintained overnight. Ignition source undetermined--believed to be spontaneous.

EE was in the process of removing baffles from 2A Hot Leach tank. The baffle slipped and lacerated EE's left index finger. The EE was wearing leather gloves. The wound required stitches.

EE operating transloader to load truck from rail car. EE stepped too close to conveyor while attempting to close car gate. R leg was caught between the belt and guide tire. E-Stop bar activated to stop belt. EE extricated in < 30-minutes.

EE assisting operator drilling air relief hole. In effort to retrieve drill steel, EE placed L foot under RB boom. Boom lowered onto foot causing bruise.

EE was assisting in removal of pin mixer bottom liner. The drum was blocked but allowed slight movement. EE reached into the enclosure to grip the liner when the drum bumped slightly pinching R index finger. Laceration required stitches.

Dent was found top of N conveyance, #2 shaft. Though operable, hoist was removed from service (precaution) pending further inspection. Shaft inspection found loose shaft lagging. 8:55 a.m. hoist was removed from service for repairs. Personnel in the mine were evacuated through the E Mine. MSHA & NM BMS hotlines were then called. Service restored to hoist at 11:45 a.m.

On 6/15, EE reported recurring back pain from an off-job back strain injury. By 7/8, short-term disability was denied and EE subsequently claimed work injury from operating SC on rough haulage in S-812. After investigation, claim was accepted. LWD start 6/16.

EE fell backward on a chair straining back. Conservative treatment applied. Medical follow-up resulted in medical deferment from work. First LWD 06/14

EE was moving a section of water hose for transport. He felt a sharp pain in lower back. The injury was treated and resulted in LWD on 6/4

EE dragging feeder cable back to work on splice, stepped on rock, turned R ankle, fractured fibula.

EE was helping guide the removal of a large equip part and was focused on clearance in one direction and not on the position of his R hand. The equipment shifted unexpectedly, pinching the R palm. Stitches required to close wound.

EE stepped back while co-worker struck link for conveyor chain to remove link. Steel chip from the link struck EE in face. Debriding attempted at ER. Stitches to close

EE was loading 12" pipe flanges onto a flatbed truck and cut his L #5 finger on flange. The wound required stitches.
