Employee was releasing a chain tie down from a secured load. The handle sprung back, striking the employee in the face breaking/displacing the bones of the nose.
Individual was roof bolting in the #3 entry, right slab @ xc 18 on unit #2. When a piece of rock fell from roof striking canopy on DBT roof bolter, flipping over and striking individual on the left side of head and left ear resulting in stitches.

A rock measuring 20 feet in length by 15 feet in width and 10 feet thick fell from the upper #7 bench onto the canopy entering the mine.
Employee was exiting a DBT battery scoop when right foot slipped and left shin struck the deck of operators compartment causing a 2.5" x .25" laceration. Received stitches and staples.

Employee was getting ready to clean the scrubber screen when a piece of shale 24" long x 24" wide x 4" to 6" thick fell from between the bolts striking his left hand which was resting on the miner. The avulsion that occurred required 15 stitches.
Hammering bit into bit lug, piece of metal broke off, striking him in the upper lip, piece of metal lodged in lip. Resulting in 5 stitches in upper lip.
Taking trailer tongue out of bed of trailer, smashed pinky between tongue and rib, breaking left pinky and having two stitches.
There has been an unintentional roof fall in the #2 entry, in the Main West Return, @xc-26. Approximately 25' long, 17' wide, 3' high.
At approx 5:20 pm a 40 ton articulating truck was traveling along a haulage road impacting a 63" high berm. The truck traveled across the berm and fell 72 feet to the mine floor below. The victim was extracted from the truck by EMS and airlifted to the Hospital.

Lost power @ 3:18 a.m. and fan. All employees were evacuated out of mine. Ventilation was restored @ 4:00 a.m. Employees did not go back in the mine due to rising water in the pit. At 5:08 a.m. the water breached the return portal and went to xc-7 MW return and over to xc-6 MW travelway. Reported at 5:19 a.m. There was no k order issued.

An unintentional roof fall has occurred at xc 7 entry 7 of the 2 west panel intake. The fall is approximately 19 feet wide, 20 feet long, and 8 feet high.
Cribs were being built, victim was struck by a hammer on his right ring finger, resulting in the tip of his finger being broken.

An unintentional roof fall has occurred at XC 13 of the Main West Return. The fall was discovered during the weekly examination of the return. The fall is approximately 20 feet in length and 19.5 feet in width and 7.5 feet high.
At 11:55pm the mine lost power and had we received over 2 1/2 inches of rain in about 30 min. The diesel pump was pumping, but could not keep up, at around 12:58am water started running into the return portal down to XC 6 into travel belt, and Intake all at xc 6. MSHA was called at 1:06am and a Verbal K-order was issued.
An unintentional roof fall has occurred between the #3 & #2 entries at 27+60 on the 2 mw in the return. The fall was discovered on the weekly exam. The fall was approximately 20' long and 7.5' high and approximately 16.5' wide. The intersection #3 had been cribbed prior to fall, from the intersection of #3 to the #4 entry.
A cleaning person was performing his duties at the mine office when a guard from the detective service drove past and saw him caughing & turning blue. Guard stated he came in mine office & victim sat down in a chair & passed out. Guard then called 911 and notified mine officials. Victim passed away enroute to hospital. Coroner's report said he died from heart attack.
At 2:25 pm Severe thunderstorms passed thru dumping heavy amounts of rain. At approximately 2:35 pm the Mine manager was notified of rising water in the sumps and the section was notified to deenergize power and head out of the mine. At approximately 2:45 pm water breached the return portal and started running into the Mine. Three MSHA inspectors were on site.
Injured was riding on a mantrip going out of the mine; mantrip pulled out of travel way into crosscut to get out of oncoming traffic's way. When pulling out of crosscut injured's foot became wedged between the lip of the foot rest of the mantrip and an overhang on the rib, severely bruising his left foot.
An unintentional roof fall has occurred in the #2 entry of the 2nd Main West Return at 81 XC (48+60) in the intersection. The fall was discovered during an E01 inspection with an MSHA inspector. The fall was approximately 18' wide x 18' long x 6' high. The area was dangered & timbered off.
Bolting vc2 at 3+00 in 1st west panet, installing 2 bolts on corner of xc. When finished, started backing up, offside operator was loading supplies, when right foot got caught under tire. Inj was transported to hosp where it was discovered a broken bone 6" above right ankle in tibia bone.
An unintentional roof fall has occurred in the #10 Entry of the Main South @ 19+20 - 20+40. This is an extension of a previous fall which has made it a reportable roof fall. The fall was approx. 120' feet long x 17 1/2' wide and 9' high. This area had already been cribbed and dangered off prior to fall.
An unintentional roof fall occurred in the 1st west rooms off of 1st main south in the #1,2,3,4 rooms at 1+80 & connecting at the #4 rooms with 2 + 40 running in 2+40 in the #4 & #5 rooms. The fall extended for approx 300' and was 18' wide and approx 9' high.
An unintentional roof fall occurred in the 1st West Rooms off of the 1st Main South at 3+60 of the #5 room, connecting with 4+20 at #5 room extending through #11 room. The fall was approx 360' long, 18' wide and 9' high.
An unintentional roof fall occurred at 7+20 2nd set of rooms off of main south intake at evaluation point B. The fall was approximately 180' long x 17.5' wide and 8' high. This area had been timbered & dangered off due to suspect roof conditions.
A mechanic lit diesel fuel in 5 gallon bucket to thaw air tanks on truck. EE was trying to stay warm by the fire and his clothes caught fire. He had on clothes from previous day that had diesel fuel on them. Contrary to company policy, the mechanic lit the fire. Air brake alcohol was provided by the company to thaw the lines out. The mechanic had instructed EE to watch the fire.
An unintentional roof fall occurred on the 1st south main xc 34 - xc 37 (20 + 40 - 21 + 60) in the #4 entry. The fall was approximately 130' long, 15' wide and 9' high. This area of the mine had been dangered off on 9/19/06 due to questionable roof.

Injured was working on a coal hauler. Piece of metal struck victim in left shoulder causing a laceration about an inch and a half long. Injured received 4 stitches in left shoulder.
An unintentional roof fall occurred on the 1st Main South at the 31 XC (18+60) in the #8 entry. The fall was approximately 40' long, 17' wide & approx 8' high. The fall was found on an idle shift, during a fire check at the mine. They were called in on Sunday 8-13-06 at 1:00.
