Employee was riding out of mine on mantrip and in a low spot he raised his head and a roof bolt knocked off his hard hat and the next roof bolt cut his head. Laceration to left side of top of head requiring 6 staples.
EMPLOYEE WAS WORKING ON MINER & WAS JACKING THE PORTABLE POWER PACK WHEN THE JACK HANDLES PRESSURE WAS RELEASED, THE JACK HANDLE HIT EE IN THE RIGHT SIDE OF HIS HEAD (TEMPLE AREA). REQUIRED STITCHES, NO LOST TIME.
Employee was removing lid from miner and a piece of draw rock and foot square x 1' thick, fell hit ee's three index fingers on left hand. Laceration of three fingers, requiring stitches.
EE CAUGHT LEFT WRIST BETWEEN DRILL HEAD AND T BAR
Employee was bolting top and rib rolled and rock hit employee in back.
EE stated he was pulling bolter cable slack up to power center and strained his back. last day worked was 11-2-04.
EE WAS OPERATING SHUTTLE CAR & WHEN HE WAS GOING TO TURN AROUND HE PUT HIS HAND BETWEEN TOP OF ROOF & CANOPY. DID NOT USE HAND HOLDS INSIDE CANOPY. FRACTURED RIGHT MIDDLE FINGER & FINGER ON EACH SIDE IS BRUISED.
FALL IS ONE BREAK INBY SPAD #955 IN #6 HEADING OF OLD INACTIVE PANEL. FOUND DURRING WEEKLY CHECK. APPROX. SIZE: 125'LONG X 20'WIDE X 5-7'HIGH. CALLED STATE & FEDERAL ON 10/27/04.
Employee was bolting top and canopy swung over and hit ee in face. Lacerations to upper lip and nose, broken nose and sinus cavity under left eye.
EMPLOYEE HIT CONVEYOR FLIGHT WITH HAMMER & A PIECE OF METAL HIT EE IN THE FACE, HIS RIGHT CHEEK.
Ee was trying to stop the shuttle car and he caught his right foot between the shuttle car frame and rib.
