Maintenance employee was replacing a skirt rubber on a chute above the main feed belt. The employee did not Lock Out the feed belt and while working on the chute a piece of metal fell onto the moving belt, the employee tried to grab the metal and caught his right arm between the belt and belt cover. The employee sustained a fracture to his right wrist and forearm.
A maintenance employee was reworking rubber belting in the finish screen tower when a spark caused the belting to catch fire - The employee utilized a Fire Extinguisher and was unable to put out the Fire - The local Fire Department was called and put out the fire. There were no injuries due to the Fire.
