Employee was driving a tractor and trailer off a haul road and the truck came out of gear causing the employee to hit a berm that was set up for a run away truck ramp.

Employee was driving a water truck and stepped out and twisted his right knee on un-level ground.

Employee was running a 992G front end loader and was prying rock from the coal and the bucket slipped from under the rock and loader bucket came up and went forward into solid wall.

Employee was turning water valves inside of dozer and caught his right thumb on a pulley causing a laceraion which required stitches.

Employee stepped off the bottom step of 777D rock truck and stepped on a rock and twisted his ankle.

Took hose off that was under pressure

After parking the equipment for the evening, (no night shift at this mine) the security guard called and said one of the dozers was on fire. This was approximately 1 1/2 hours after the shift was completed.

Employee was dragging a fuel hose and the hose got caught on a rock pulling him around and hurting his left shoulder.

Tech was opening box with box cutter when it slipped and hit him in the leg causing a cut requiring 2 stitches.

Employee was putting seals in a main pump on a drill. He was using a pry bar to pry the pump out of the gear box. The pry bar slipped.

Driver error. Driver lost control of his truck.

EE called another EE and said he had gottin the truck out of gear and could not get it back in gear and he had put the truck into a ditch. (Driver error shifting on hill)

Employee was hitting a sprocket with a sledge hammer, when a small piece of metal hit his right upper arm, resulting in one stich.

Going down steep bank, slipped on slick rock and loose dirt. This did not become a reportable until 9/29/10. The employee had to have surgery on his knee.

He was replacing main pumps back in the boxes to take back to the shop, when he slipped and fell from his truck bed and struck the bumper in his back. He was in pain, so had to quit working.

Employee was standing about ten ft. away from the loader bucket, where they were replacing a tooth on the bucket. When they hit the keeper with a hammer, a piece of the keeper shattered hitting the employee below the left knee, required stiches.

Mechanic was working on a customer-owned 785 Rigid Dump Truck steering cylinder. He was using the drag link as a pry point, the bar slipped and came down across his right shoulder. His right shoulder socket was broken. Approximate time off from work - three weeks.

EE was removing wear strips on a loader bucket. Wrench slipped cutting left hand, he received 5 stiches.

Mechanic was changing the fuel filters on a motor grader. He reached back for the grab handle, missed and fell backwards off machine.

Mechanic helping to install transmission on D9R Dozer. Mechanic caught his middle finger on his right hand underneath when the transmission was being slid into the tracks.

He was walking around the Ford truck. There was a door open on another truck parked beside it. He didn't see it and walked into the door.

Operator stated that the 773-B rock truck skided into the berm, then comming to a rest on the left side of haul road into a mud hole. He was taken to the Hazard ARH Hospital. They did a CSAT Scan on him, the doctor said that he had a strained lower back and should be able to return to work in a couple days.

Employee was installing a fan and pump on 992 G wheel loader. Fan and motor was sitting on machine while chain was bang repositioned. Fan and motor began to fall knocking him to the ground. the fan fell on the right foot. A blade from the fan cut throught the steel toe on his boot severing his big toe.

Dozer was pushing material (rock/dirt) to a 992-G loader, the highwall fell with-out any warning. The operator had a cut/laceration to his forehead. Extensive damage was done to the dozer.

Drove up on top of the berm for approximately 80 feet, then the truck rolled over two times coming to a resting point 140 feet over embankment.

operator removed a piece of paper that was caught in blower fan of the heat and a.c. unit. When he removed the paper, the fan started turning, therefore cutting his index finger into two places.

The dozer operator backed into the highwall, resulting in injury to the neck.

The mechanic was holding a segment bolt with a scerw driver, the screw driver sliped, the bolt caught the glove pulling his finger in with the scew driver.He was taken to the ARH Hospital, he received 9 stiches in his left fore-finger.

The mechanic was lifting a front axel with his boom truck into the bed of a parts-truck when the chain slipped causing the axel to drop hitting the mechanic on the hand.

EE was getting out of truck. He turned toward the cab, got hold of the door, but not the handle. He started to step down with his right leg and missed the lower step. He tried to catch himself with his right arm and went down on his elbow. He felt and heard his right shoulder pop.

While changing a PTM cradle location, a pry bar was used to keep the cradle from moving. When they leveled the highwall miner, the bar dislodged and struck EE in the back - As a result, he was diagnosed as having a cracked rib.

As employee was climbing down off his rock truck at quit time, he placed his right foot on the ground. When he put his full weight on it, he broke a bone in his right foot.

Driving a pin into a cutter chain when a piece of the pin shattered and lodged in his abdomen.
