EE was carrying a crib along belt drive & tripped over pressure plate causing EE to fall forward landing on belt structure, causing EE's finger to be smashed between crib & Belt structure resulting a laceration.
EE was stepping over screen pile at back of bolter, foot caught the screen causing EE to trip landing on left shoulder. Pulled ligaments between the Clavicle & Collar bone.

EE was cutting metal bands off mesh bundle. While doing so a piece of wire cut EE's forearm.
EE was checking the #2 belt drive installation area. EE notice belt stop running, notice smoke and immediately went to belt drive pc to assess the situation in the process of doing so inhaled smoke.
Employee was operating a scoop, when the employee stopped to exit the scoop, they got out of the cab and stood up and a piece of mesh contacted the employee right above the right eye.
Two employees were filling the Fletcher roof bolter with hydraulic oil utilizing the power fill function, they activated the cable reel by mistake causing the cable to strike EE as EE was walking past the bolter.
Employee was tasked with handing the welders grizzly bars from an elevated platform. While handing the welders the third grizzly bar the employee stated that when EE lifted it, EE felt a slight pain in the left knee. EE reported it to the supervisor and was asked if they needed medical care, the employee initially refused to seek medical treatment until the next morning.
Mine floor was previously wet and had since dried. Equipment was used in the area while the mine floor was wet leaving tire tracks. The employee failed to recognize the hazard while carrying 4 cable bolts and inadvertently stepped in one of the dried tire tracks straining their right knee.
Ice caused the slope car to derail. Employee was jacking the car up when he was struck by the jack handle. He received a cut to the right side of his face just below the right eye brow. The cut required five stitches and the employee was released to full duty.
XC-1 East Mains saturated top of roof fill above anchorage zone. This is due to Geological conditions.

Employee was loading miner cable on the hooks on the miner, he lost his balance and put his hand on the shoe to catch himself and at the same time,the miner head came down smashing his middle right finger.
Employee was removing a tilt cylinder from a TRS on a roofbolter. He removed the pin from cylinder and cylinder fell pinching the right index finger resulting in a laceration and slight fracture.
Injured person was shoveling on the slope belt near the tailpiece. The belt was running. Some rocks became caught between the top belt and the hold down roller, he tried removing the rocks with his shovel and got close to the running belt. His arm got caught between roller and belt.
The employee was examining the belt line and heard a noisy roller. He bent down to look at the roller and a piece of material ( coal or rock) came off of the belt hitting the employee in the left side of face, causing bruising and possible concussion.
Employee was jacklegging and when raising leg it stopped and suddenly raised upward striking employee in mouth and right side of face. This resulted in complex right lower lip laceration, right cheek laceration, depressed fracture anterior wall right Maxillary Sinus, fractured teeth
Building Stop Blocking. Air tugger fell causing a laceration to the front of leg down to ankle and impacted his foot.
Person was tightening an electrical ground with a hand wrench. The wrench slipped and the person dislocated his right shoulder. He manipulated his shoulder back into place. Note he had dislocated his right shoulder before.
Western Industrial employee did not report to his work area after lunch. He was found unconscious in a port a-john. Attempts by EMTs to revive him were unsuccessful and he was pronounced dead.
Employee states that he slipped on steel while installing grating in breaker bldg on top floor. His left foot slipped and his right leg hit steel injuring right knee. This injury was reported to this office late, hence our late filing. Employee worked modified duty 10/12/10 and was released to full duty on 11/23/10.
EE was holding steel aligning it with jackleg and jackleg jumped smashing his hand into the roof.
