Employee was dismounting a dozer the employee had been working on, and while dismounting the employee had stepped onto the pusharm step and slipped off between the track and pusharm. It had some oil residue on it, but the non-skid surface and grab irons were in place.

Hitachi EX3600 excavator was loading trucks when the engine/hydraulic compartments on the rear of machine caught fire. Operator exited the machine safely and fire departments were called to help extinguish the fire.
Employee was unclogging drain pipe at sediment pond 41E, while removing the pond floating boom from inlet of pipe, employee must have became engulfed in the draining water pulling ee into the drain pipe.
WHILE BACKED IN TO FUEL CAT EXCAVATOR, EMPLOYEE DIDN'T NOTICE BIG DRIED UP CLUMP OF DIRT ON THE GROUND OUTSIDE THE CAB OF SERVICE TRUCK, WHEN EE EXITED THE CAB EE TRIPPED OVER THE CLUMP OF MUD AND FELL ON TO EE FACE AND LEFT SHOULDER.
Employee was operating front-end loader in coal pit area when a rock became dislodged from the high wall and fell. The rock came through the right-hand door glass and struck the employee on the right hand. The results of the fall were the employee has 2 broken bones and a laceration on the palm of right hand.
Employee was outside of cab and reached inside to turn off roof-mounted air conditioner, and got middle and index fingers too close to the fan and it cut them on the tips.
Service truck driver stepped out of truck the wrong way and sprained ankle.

Teammate was assembling a 18.00R33 tire on front wheel of 769 cat rock truck. Teammate was using the boom and a hammer to install the bead seat band on the wheel and was swinging the hammer, when the handle broke off of the head of the hammer and struck the teammates finger creating a fracture to left index finger. He continued to work and finished the job.
Service truck had pulled out of the pit area and the head lights went dim. Service operator pulled to the top of the hill to check the problem with the head lights. Once at the top of the hill he was checking the lights and noticed the truck was smoking from the rear of the truck. The truck started to burn on the back.
The driver began backing onto the drill bench when he felt the brakes go soft. He reacted by turning the steering wheel of the truck abruptly to the left causing the truck to turn onto its side.
The blaster helper was driving Kenworth Truck up steep grade at mine site. The back wheels started to spin & bounce, causing drive shaft to break. Due to weight of material on truck, truck started sliding back down the steep grade. Blaster helper jumped from truck & sprained his left ankle. Which will result in the blaster helper being off work for 3 scheduled work days.
