Employee and co-worker were pulling a piece of conveyor belt that was 6 feet wide by 8 feet long and the employee lost his footing and then felt pain in his back. Employee was seen by a physician and removed from work.

Mostrando los 6Employee states that while he was traveling down the haul road from the refuse in the Cat 777 truck, he hit a hole in the road and jarred his neck and back.
Employee was per-shifting end loader and as he was walking around the machine and lost his footing and fell. As he fell, he tried to put his hands down and catch himself. In doing so he fractured and dislocated his wrist.
Employee received an abrasion to the leg on 4/1/2013, while transporting an electric motor on a carte. Employee was transported to CAMC, where he was treated and release at this time. On July 10, 2013 received medical documentation that ee was treated and diagnosed for cellulites with chronic ulceration at CAMC. EE was give antibiotics and taken off of work.
Was removing rust from steel beam with a 2 lb. hammer and missed intended target and mashed finger.
Employee change trailer caught fire from an apparent electrical malfunction. No injuires involved. Structure destroyed.
