While welding the bucket pin on the John Deere 844 wheel loader, a piece of slag dropped down, burnt through employee's jeans and went into their boot burning their foot.
Plant operator noticed the pipeline was plugging up. Operator and other employee went towards the booster to listen for material moving in the pipeline. They could hear material moving and kept walking towards the plant. They found where the plug was and told dredge operator to shut down. The pipe came apart at a rubber joint and the material and water flew out striking operator.
Employee was going to release a come-a-long on the ADT cab to lower it back down when EE slipped off the edge and fell 7' to the ground fracturing EE's left scapula.
A section of pipe was being removed from the plant dredge. The pontoons have a hook for a sheave that holds the electric cable. They had the cable removed and hung 2 sheaves on the pontoon hook. When hitting the pontoon w/ the workboat to separate the pipe and rubber elbow one of the sheaves flew off crushing employee's left thumb while they sat in the workboat.
Employee was working around hand grinder and got two pieces of metal in EE's eye. Employee was wearing all PPE. Glasses and face mask were being used. Employee thought metal got into eye when EE was finished and wiped away sweat from EE's brow.
Miner was changing a shaft sleeve on a pump at the plant when miner's hand with gloves get stuck in the V-bands and pulley.
Employee was removing a stuck bearing race off of a PTO shaft. Employee cut one side off and it freed the rear end of the shaft - it caught one burr of imperfection on the shaft and while hammering the imperfection off a piece of metal flew off and struck employee in leg.
The truck had a leak at the hydraulic hose beneath the cab. The seat and an access cover were removed to gain access to the hose that ran on top of the transmission. EE was straddling the transmission to replace the base when they slipped and fell on their tailbone. Sustained a fracture to the tailbone.
Employees were removing the dredge pump impeller shell. As they were removing the shell, the shell shifted and struck our employee in the stomach area. The employee experienced bruising in the stomach area. No other injuries.
Employee was exiting a Jonboat, when he stepped out of the boat his foot sank in the mud and lost his balance, fell and sprained his back.
Employee was driving along roadway at approx. 5:30 a.m. The vehicle he was driving went through a berm and landed in approx. 45' of water. Employee was found outside the vehicle.
Two mployees had placed a shaft into a workboat for transport. When one moved to side of boat, shaft shifted to side, overturning boat. The EE jumped to the dredge; as he tried to pull himself onto the dredge, the boat struck him in the left pelvic area.
