Employee's work shift ended at 3:00 pm. At approx. 4:40 pm while still on mine property picking up plastic pipe for his personal use, he suffered a massive heart attack, per Medical Examiner at Wellmont Lonesome Pine Hospital in Big Stone, VA. The Security Guard witnessed him having chest pains and going unconscious. Will send copy of death cert. when available.

A roof fall occurred at the #21 crosscut, in the #2 entry spad number 2112. The fall was approximately 16 feet high by 80 feet long by 25 feet wide.
Employee slid a tire off the forks of a fork lift to install it on a shuttle car. It hit the wheel unit and bounced back and pinched his finger between tire and forks on fork lift.
Employee was shoveling underneath head drive when his finger came in contact with a piece of metal, resulting in 3 stitches being put on left right finger.
Installing roof strap, while installing roof strap it slipped down striking employee in left cheek and ear. He received fifteen stitches. (15 stitches).
IEE stated he was guilding a wire rope through the scrapper which was pulling belt through it.EE stated he had his right hand on motor and it slipped off causing his finger to get caught in belt pulley. Employee was going through a man door at an out-by area of the mine when he came in contact with a piece of draw rock, causing a cut to his right leg. Three stitches were required to close the cut.
Employee was attempting to move a rear end for a stinger personnel carrier, when he felt a sharp pain in his lower back. This incident happened on the surface, where the mantrips are parked.
The employee was working on bolting supplies on the bolter when he twisted his right hand. This twisting action caused a fracture of the 4th metacarpal bone of the right hand.

EE is the responsible person on the surface. The stacker belt was hung up, ee was running across the surface area to turn belt off. When he tripped and twisted his knee.
Rock fall on 001 section return. Located inby spad #717, on 1 Left off West Mains approximately 3,400 feet from the section. Fall was in the last row of open crosscuts and was between #5 & #6 entries. Approximate dimensions are 20 Ft. wide by 6 foot plus high by 100 ft. long. Fall will not be cleaned up, cribber off.
He was hanging a miner cable and said he felt a pull in the front of his left shoulder. He continued to work until July 17, 2011 when he went to an emergency room and was informed he may have a rotator cuff tear. He is off work pending an MRI.
Employee was assisting in repairing a link in the feeder chain on the 001 Section. Another employee was hitting the link with a hammer and a small piece of metal flew off and struck him between the eyes, above his safety glasses. After work he went to an emergency room where the metal was discovered and removed.
Employee stated that while exiting the scoop he got his left foot hung between the breaker lever and the seat. The employee stated this caused him to lose his balance and twist his left knee. Possible pulled muscle.
He was on the man trip preparing to go underground. The man trip did not go into reverse but stuck in forward. It was parked in front of the maintenance trailer. When the man trip moved it struck the towing tongue of the trailer, the sudden stop caused him to twist forcefully. His left ankle started bothering him 4 hours later, diagnosed with Fx of lower left fibula.
Employee stated that he was adjusting the pads on the T-Bar of a Fletcher Dual Head roof drill. He stated that he knocked out the pin that was holding the pad. He did this so he could move the pad. When he reached to move the pad, the pad set down on the t-bar catching his finger between the pad and the t-bar.
Employee stated that he was loading roof straps into the bucket of his scoop. He was using a hammer to break the metal binding straps that were holding the roof straps in a bundle. The employee stated that on one swing the hammer missed the binding strap and as a result his right ring finger hit the binding strap causing a laceration requiring 3 stitches.
Employee riding on back seat of mantrip. Jacket hood caught on belt guard at underpass. Employee was pulled off slow moving mantrip. Went to Emergency Room, returned to work with no restrictions. Saw Dr. on April 20 complaining of tingling in left hip and moderate shoulder pain. Taken off Work on 4-20-2011, presently scheduled for an MRI.
During a pre-shift a roof fall on the 002 section was discovered. Fall was approximately 20 ft. outby spad # 954 in the #4 entry. Fall was approximately 100ft. long, 18 ft. wide and 15 ft. high. Area has been cribbed off and supplemental support has been installed in the area surrounding the fall.
A roof fall on the 002 MMU in the #7 entry 140 feet outby S.S. 1011. Fall was located between the #22 and #23 crosscut. The fall was approximately 20 feet wide by 20 feet high by 70 feet long. The fall is cribbed off and will not be cleaned up.
EE was assisting in moving a section water pressure pump. He was securing a chain on a hook that was mounted on the ram of a scoop. As the ram retracted, the employees hand slipped behind the ram and was caught between the ram and the bucket frame. X-rays reveled in-line fractures to the right little finger and ring finger.
EE reported pain in his shoulder. He said he did not have an accident but the pain persisted. He saw a Dr. and was told to return to the neurosurgeon that he had been seeing for a previous injury to the shoulder. Worker Comp. denied this claim and ee remains off work while MRI and other tests are being performed.
Removing a rock from the Rock Box near the #3 belt drive. The rock flipped and caught his right hand between the rock and the frame. Taken to ER for x-rays and returned to work the next day. On 3-17-2011 he was informed that the x-rays showed a hairline fracture near the first joint of his right middle finger. Became medical reportable with restrictions on 3-17-2011.
Employee stated that he bent over to pick up the miner cable, when he felt a sharp pain in his back. He worked the remainder of the shift and decided to go to a doctor the next day. The doctor referred him to a specialist.
Employee was roof bolting. He felt pain in his chest and left arm, taken to hospital. Admitted for possible heart attack, released next day and told there was no cardiac event. He was diagnosed with a possible pulled muscle and given two days off. Returned to Dr. complaining of left arm pain and was told to get an MRI, employee off until given an MRI.
Employee stated he had moved the roof bolter cable and he said he was unaware that the operator was about to start the cable reel in order to take up the cable. When the cable moved it struck the employee caused him to lose his balance and fall. When he fell he landed on his self rescuer causing pain to the employees left side. He was told he had a contusion.
Employee was roof bolting. He stated that he twisted around to retrieve a roof bolt when he felt a pain in his back. He worked the rest of his shift and went to the emergency room on 12-21-2010 at 10:00 a.m. His was told that he had a pulled muscle.
Employee was walking down steps behind shop. His foot slipped in the gravel when he stepped off the last step. Employee fell against a scoop mounted rock duster. He bruised his lower back.
Employee stated on 11-22-2010 he threw a crib block (6"x6"x30") and felt a pain in his left shoulder. He did not report the incident. On 11-24 he stated a doctor told him he had a torn left shoulder rotator cuff. On 12-3 he reported the alleded injury to managment and filled out an accident report. He was off with pneumonia on 11-23 & 11-24 and for the holidays unitl 11-28.
Employee picked up the bottom stand while working on the belt structure.
Employee was changing out a bad structure on outside stacker belt...while lifting structure felt pain in his lower middle back.
Employee was operating pinner and was retracting a piece of starter steel from the top as the steel cleared the top he was struck by the steel hitting his right arm.

The employee was allegedly walking through a check curtain when he ran into the rib with his head (hard hat). Employee complained two days later about a stiff neck and decided to go to a doctor.
While bolting, employee had his right hand on the pizza pan. While he was trying to get the rotation hose out of the way, the lever was accidentally pushed causing the bolt to raise up, catching his right fingers between the pizza pan and the top.
EE was taking suction hose off near the pinner head. Another EE moved the pinner catching EE's middle finger in a pinch point. This mashed the finger requiring stitches. Note: Paper work got misplaced.
Employee was moving the roof bolter when he was caught between the roof bolter and the mine rib. This caused a compound fracture to the femur bone of the left leg. This required surgery for placement of a pin in the femur bone.
Employee was pulling on a lodged piece of belt structure. It loosened & mashed employee's fingers on his right hand. This required stitches to the right ring finger.
EE was roof bolting first cut at intersection, slag of rock fell between left rib and operator at #3 left break. EE was hit with the slab of rock causing serious injuries.
EE was pulling drill steel out of drill pot that was stuck. When the drill steel dislodged it struck the employee over his right eye causing a laceration requiring stitches. No lost time.

During exploration/rehabilitation pre-existing fall was observed @ spad 208 for 2 break inby. Per instructions from local MSHA office it was called to them.
