Mostrando los 22A customer truck driver stopped their truck at the entrance of the property. They got out of the truck and went to troubleshoot a vibrating noise. Then they were just behind the passenger side front wheel, the truck began to roll backwards. The truck rolled over the driver's foot and came to a stop on top of their foot, breaking a toe.
Employee was climbing up a Volvo A35G Haul Truck when they lost their footing and fell forward into the battery box on the truck hitting the bridge of their nose. The employee sought medical treatment and received stitches. They were released to full duty on the same day.

Employee was unloading some pipe from a truck. He picked up the pipe to load the bucket. When he picked up the pipe he felt a twinge and pain in lower back and left leg. Employee was on restrictive duty and as of 3/2/16 he is a Lost time. This incident was not reported at the time due to improper communication. This incident is reported late.
Employee was lifting a screen (less than 100lbs). Received help from another. After lifting the employee felt a sprain/strain in left arm. Employee was taken to the local clinic where they put on restrictive duty with a sling. Going back for checkup to re-evaluate.

He returned to 10/22/2013, due to pain, he went to the clinic to have knee examined. The doctor diagnosed torn right medial meniscus. He returned to work on restrictive duty on 10/22/2013.
IE and another EE were lifting a small hopper weighing about 125 pounds onto the tail pulley of the concrete sand stacker. One of the hopper legs struck IE on the left leg behind the knee, causing a deep laceration.
Employee was climbing a ladder and while on the second or third step the ladder slid off of the leg of the sand screw. The ladder and employee fell to the ground and he hit his left elbow on the concrete pier on the way down resulting in a chipped elbow.
